Name of Operation: ABC DAYCARE CENTER Facility ID: 41002360 Consultant: ELIZABETH CHILDRESS Operation Type: Center Case Number: Visit Date: 5/28/2024 Number Present: 5 Completed Date: 5/28/2024 Age: From 1 To 4 Total Minutes: 105 Time In: 09:00 AM Time Out: 10:45 AM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of this visit was to monitor compliance during an Annual Compliance Full visit. I completed an Annual Compliance Monitoring Checklist during today’s visit. This facility operates with a Four- Star License issued 4/22/2020. The facility earned 7 points in Staff Education and 2 points in Program Standards, 1 Quality point (All lead teachers have at least 5 years EC work experience in no more than 2 different centers). The last Annual Compliance Full Visit was conducted on 8/2/2023. The last Sanitation inspection was conducted on 5/6/2024 and the facility earned a "superior" classification with 2 demerits. The last Annual Fire inspection occurred on 1/24/2023 and the facility was approved for daytime care only. M. Parks, Administrator/Owner, Teachers and five children were present during the visit. I observed in Space # 1 and Space #2 while the administrator/owner and teacher was attending to the individual needs of the children, story time, and later during outside activities. There was one violation of Child Care requirements observed during this visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed on 01/24/2023. 10A NCAC 09 .0304(a) The violation documented above must be corrected immediately. A signed and dated letter of compliance must be emailed to me by 6/7/2024, describing how and when the violation was corrected and how compliance will be maintained in the future. Please remember your letter should include facility name, facility ID#, and original signature. Transportation: Motor vehicle crashes are the leading cause of death in children two to fourteen years of age in the United States. It is necessary for the safety of children to require that the caregiver/teacher comply with requirements governing the transportation of children in care, in the absence of the parent/guardian. You stated that you do not provide transportation during today's visit. Per our conversation today, the outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. Since children are exploring it is very important to provide a safe environment, free of debris. A designated staff member should verify daily before children go outdoors that the area is safe for children to use. Each month, the outdoor play areas should be inspected and documented on the form provided by the Division of Child Development. All vines that grow on fences, tall grass and lots of places in the outdoor spaces may contain snakes and other vermin. You may contact your local pest control to help with keeping snakes away. Please refer to your first aid posters and your first aid trainings for signs to look children who may come in contact with these types of things. and how to care for from snakes, insects and other hazard vermin. You were provided an opportunity to ask questions regarding the implementation of Child Care Rules.
