Upon arrival, the purpose of today’s visit was reviewed with you, Taylor Beam, Program Coordinator. Your last annual compliance visit was conducted on December 16, 2024. Your program currently operates with a five (5) star license effective October 25, 2024. The permit restrictions were in compliance including daytime care only, school age children only, playground area does not meet child care playground safety standards, meets enhanced ratios and enhanced space. A walk-through of the facility was completed today, all indoor and outdoor areas used by children enrolled were monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during arrival, snack, transitions, toileting, handwashing, outdoor gross motor play, and departure. Required Inspections: The most recent fire inspection was completed on March 13, 2025. The last sanitation inspection was completed on December 17, 2024. A “superior” classification was issued with (4) demerits noted on the grade card. Emergency Drills: The last fire drill was completed on April 30, 2025, at 3:08pm and recorded. The last shelter-in-place/lockdown drill was completed on February 27, 2025, and recorded. Lead Water Testing: The most recent lead water testing results were completed on March 10, 2024. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. On April 29, 2025, one child was not signed out. On May 9, 2025, one child was not signed out. 10A NCAC 09 .0302(d)(4) Technical Assistance: Arrival/Departure: I suggested that a staff member review all arrival/departure sheets daily to ensure every child has been signed in and out each day and maintain compliance with child care rules. In addition to the updates to this rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816, require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. Upon arrival, the purpose of today’s visit was reviewed with you. You, Shannon Greene, Program Coordinator, assisted me with the visit today. Your last compliance visit was February 6, 2023. Dinah Brown, Child Care Consultant, accompanied me on the visit. Your program currently operates with a five (5) star license effective March 19, 2018. The permit restrictions were in compliance including daytime care only, school age children only, enhanced ratios reduced by 1, 7-point group size reduced by 1, playground area does not meet child care playground safety standards, meets enhanced ratios and enhanced space. You have earned seven (7) points in education, seven (7) points in programs standards and one (1) quality point for having reduced staff/child ratios by one in each group from the 7-point level as required in Rule .2818c for a total of a total of fifteen (15) points. Children were met when school was dismissed and brought to the cafeteria and space #5. We observed children during free choice activities, hand washing/bathroom, snack, transition to and from cafeteria, and departure. Supervision and staff/child ratios were in compliance. Two staff files were reviewed, including one existing staff file and one new staff file. One violation was cited and discussed with you. Children’s records were reviewed with no violations. The most recent sanitation inspection for your facility was conducted on December 6, 2023. A superior sanitation classification was issued with 4 demerits. The most recent fire inspection for your facility was conducted on November 1. 2023. The facility was approved for day time care. The latest fire drill was conducted on 12/8/23. One violation was observed and discussed with you. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed.
