You, Ashley Waugh, Lead Teacher, assisted with today’s visit. Your program currently operates with a five-star license issued on September 23, 2025. The permit restrictions listed on your license including Daytime Care Only; Certified Developmental Day Program; Enhanced ratios reduced by 1 in each group; Meets enhanced ratios; Meets enhanced space were in compliance. The Secretary of State website was checked on May 11, 2026, and your business, Caldwell County Schools, was not listed. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in routines and free play during the visit. Required Inspections: The last fire inspection was conducted on April 15, 2026. The last sanitation inspection was conducted on April 28, 2026. A “superior” classification was issued with four (4) demerits noted on the grade card. The last playground inspection was conducted on April 24, 2026. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on March 10, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The last fire drill documented was conducted on April 21, 2026. The last emergency drill documented was conducted on March 4, 2026. NC Pre-K and Developmental Day Requirements were reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Your program uses the Creative Curriculum. I reviewed the ABCMS Provider portal during the visit. I confirm that seven (7) current staff members are included in your facility portal. Files Reviewed: Two (2) children’s files were reviewed during the visit. One (1) existing staff file was reviewed during the visit. A checklist was used to note the requirements I monitored today. The following violation was observed/documented during today’s visit: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In space #2 I observed one aerosol can of Barbasol Shaving Cream in an unlocked cabinet accessible to children. .2820(b) You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Prior to today's visit, your program maintained a compliance history of 98%. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Technical assistance was provided on the following: Hazardous Products: All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled must be stored in a locked storage room or cabinet. I suggested that you monitor all areas of the child care facility each day to ensure all hazardous products are stored appropriately. I suggested that you remind staff to refrain from storing the key in the lock on cabinets or closets containing hazardous products or materials required to be kept in locked storage. I also suggested that you check all storage cabinets and closets to ensure the lock is operating properly. Remind staff of appropriate storage of hazardous products and appropriate locations and locks to be used. Locked storage rooms and cabinets shall include those which are unlocked with a combination lock, electronic or magnetic device, keypad, key, or equivalent locking device. Keys and electronic or magnetic unlocking devices shall be kept out of the reach of a child and shall not be stored in the lock. Toxic substances shall be stored below or separate from medications and food. Any other product that is labeled "keep out of reach of children" and does not have any other warnings on the label shall be kept inaccessible to children when not in use, but is not required to be kept in locked storage. A product shall be considered inaccessible to children when stored on a shelf or in an unlocked cabinet that is mounted a minimum vertical distance of five feet above the finished floor. One new staff member was hired since the last visit. The following violations were observed/documented during the visit: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. On January 14, 2026, one child was not signed out. On January 20, 2026, one child was not signed out. 10A NCAC 09 .0302(d)(4) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. A staff member employed on January 2, 2026, did not have a signed shaken baby acknowledgement form on file for review. .0608(d)(1-4) The violation(s) documented must be corrected immediately. On or before February 5, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Technical assistance was provided on the following: Arrival/Departure: I suggested that you designate one staff member to monitor the arrival and departure records at designated times each day to ensure all children have been signed in and signed out. Required Paperwork: I suggested you create an enrollment packet and signature sheet for all required acknowledgements to have each parent complete during the enrollment process. I also suggested you use the children’s file checklist found on the DCDEE website under the “Provider” tab to ensure all required documentation is on file. I provided you with a copy of the children’s file checklist during the visit. Consultation As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. One new staff member was hired since the last visit. I also suggested you use the children’s file checklist found on the DCDEE website under the “Provider” tab to ensure all required documentation is on file. I provided you with a copy of the children’s file checklist during the visit. Consultation As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated.
