You, Kathy Burchette, Owner/Operator, assisted with today’s visit. Your program currently operates with a five-star license issued on August 6, 2018. The restrictions listed on your license include First and Second Shift; Maximum of 5 preschool children at any time; No more than two infants under age one year; Fireplace/woodstove not in use during operating hours. You stated that you have six (6) children enrolled and five (5) were present when I arrived. Children throughout the home were participating in routines, lunch, free play, and rest time during the visit. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and learning environment and found supervision and staff/child ratios to be in compliance Required Inspections: The last playground inspection was conducted on December 17, 2025. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on May 2, 2023. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The last fire drill documented was conducted on December 16, 2025. The last emergency drill documented was conducted on December 12, 2025. A checklist was used to note the requirements I monitored today. I reviewed the ABCMS Provider portal during the visit. There is currently no one listed under the Precious Memories roster. Files Reviewed: One (1) child’s file was reviewed during the visit. One (1) existing staff file was reviewed during the visit. The following violations were observed/documented during today’s visit: Violation Number Comment Rule 716 Electrical outlets not in use were not covered. I observed one outlet near the writing center that was not covered and was accessible to children. I also observed one outlet in the home living center that was not covered and was accessible to children. 10A NCAC .1719(a)(27) 1735 The daily schedule, activity plan, infant feeding plan, allergy posting, SIDS Sleep Chart/visual check were not maintained on file for a minimum of 30 days from the revision or replacement date. The daily schedule was not posted and was not available for review during today's visit. .1721((f)(4)(A) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. Currently there is no one listed under the roster for Precious Memories in the ABCMS system. G.S. 110-90.2 & .2703(r) 2008 Operator did not document the review of requirements found in .1700 and in G.S. 110, including Emergency Preparedness and Response Plan. The most recent review of your Emergency Preparedness Plan was conducted on January 7, 2025. .1729(a) (10) The violation(s) documented must be corrected immediately. On or before January 22, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Please send your letter to dinah.brown@dhhs.nc.gov. You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Prior to today's visit, your program maintained a compliance history of 97%. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Technical assistance was provided on the following: Electrical Outlets: I suggested that you monitor all spaces throughout the day to ensure all electrical outlets not in use are covered with a safety plug. In addition, check all electrical outlets at the beginning of the day. Required Postings/Daily Schedule: I suggested that you create a checklist to use each day, week, and month to use to ensure all required items are completed within the required timeframe and are posted in the locations where they are required. ABCMS: I discussed with you that you must notify the Division of all new child care providers who are hired or have moved into the child care facility within five business days by entering each staff member’s information into the Automated Background Check Management System (ABCMS) Provider Portal. I reviewed with you the ABCMS Provider Portal Access Guide document and provided you with a copy of the document during the visit. Emergency Preparedness: I suggested you set a reminder in your calendar at least twice per year for trained staff to review the Emergency Preparedness Plan to ensure all information is up to date. I suggested that you complete the annual review the EPR plan at the same time you review the Emergency Medical Care Plan. Consultation: We discussed the QRIS Modernization and the Pathway to the Stars. We reviewed all three (3) pathways, as well as the Education standards, Continuous Quality Improvement Standards (CQI), and Family and Community Engagement Standards. I encouraged you to visit the QRIS Modernization website to review information regarding the changes to the rated license process. Additionally, please ensure all staff members have a WORKS account, that all transcripts have been submitted, and all WORKS accounts are up to date. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. You, Kathy Burchette, Owner/Operator, assisted with the visit. On October 29, 2024, the following allegation was received. There is a concern that child care area is not maintained in a sanitary condition. When I arrived, I discussed the allegation with you. I observed the children playing independently in centers, diapering, and departure. There was a smell of urine present. You stated that a child that was being toilet trained had recently soiled himself and that you had changed the child and placed the soiled clothes in the child’s bag to go home with the parents at pick up today. We discussed your procedures for daily, weekly, and monthly cleaning and sanitizing of all space, equipment, and toys used by children. You stated that you wash sheets on the mats weekly unless they are soiled. You stated that if a mat sheet is soiled that you clean and sanitize the mat and put on a clean sheet. You stated that you clean the kitchen area daily after the children leave. You stated that you mop weekly with a damp mop. We also discussed your procedures for the disposal of soiled diapers and used wipes. You stated that you put soiled diapers and used wipes in a trashcan. You stated that all of the children in your care are currently in diapers and one is currently being toilet trained. You stated that you are no longer using a training toilet and are using a standard toilet with a seat insert. We discussed your procedures for the handling of children’s soiled clothes and cot sheets. You stated that you put soiled clothes in the child’s bag that they bring each day and send them home with parents daily. We reviewed the pets you currently have listed on FCCH and Centers Located in a Residence Verification of Required Information for Operator and Additional Caregivers Form. You stated that you only have the two dogs listed on the FCCH and Centers Located in a Residence Verification of Required Information for Operator and Additional Caregivers Form and that they both stay in kennels in the kitchen area that is not accessible to the children. Based on my observations and your statements there was evidence that the child care area is not maintained in a sanitary condition; therefore, the allegation for “There is a concern that that the child care area is not maintained in a sanitary condition” was confirmed and therefore substantiated. There were 4 children present ages 1 to 3 during today’s visit. The following violation was observed during the walk through today. I discussed the violation with you in detail at the end of the visit. Violation Number Comment Rule 804 Soiled diapers were not placed in a covered, leak-proof container which was not emptied and cleaned daily. I observed a soiled diaper in a small gray trashcan with out a lid located between the kitchen area and the child care area. .1725(a)(6)(D) Technical Assistance: I recommended that you add a disinfectant/deodorizer to the mop water when you mop. I recommended that you dispose of soiled diapers and used wipes in the trashcan beside the front porch outside not accessible to children. I recommended that you bag children’s soiled clothes and place them in a container outside on the front porch not accessible to children. I recommended that you remove the soiled cot sheet from the area that the children use or place it in container with a lid labeled soiled items out of the reach of children. I recommended that you use a disinfectant daily after all the children have departed for the day. Consultation: Prior to today’s visit, your compliance history was ninety (90) percent as of November 4, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Compliance Letter: You must correct the violation found during today's visit immediately. Inspections/Emergency Drills: Emergency Drills/Playground Inspections: The last shelter-in-place drill was completed on June 3, 2024, and recorded. The last playground inspection was completed on September 2, 2024, and recorded. The last fire drill was completed on August 9, 2024, at 2:00pm and recorded. A checklist was used to note the requirements I monitored today. Files Reviewed: One (1) staff files were reviewed today. The following violations were observed during the walk through today. I discussed these violations with you in detail at the end of the visit. Violation Number Comment Rule 904 Documentation of the operator’s emergency medical care plan to follow in the event of a child medical emergency, was not on file and available for review. No Emergency Medical Care plan was posted for review. .1713 1931 Telephone numbers for the fire department, law enforcement office, emergency medical service, and poison control were not posted in a location visible in the home. No Emergency Telephone Numbers were posted for review. .1719(a )(12) 1940 Hazardous cleaning supplies and other items that might be poisonous e.g. toxic plants, were not out of reach or in locked storage when children were in care. I observed several poison oak plants on the playground. One small plant was on the ground in the middle on the right side next to the fence several plants were pushing through the chain linked fence onto the playground area. .1719(a)(6) Technical Assistance: Emergency Information: I recommended that all required information be current and posted at all times to ensure the safety of children and maintain compliance with all child care rules and requirements. Playground Safety: I suggested that a daily playground walk through take place to ensure the area is free of hazards that would be dangerous to children such as toxic plants, broken toys, spiders, bee nests and snakes to ensure the safety of children during outdoor play. Consultation: Prior to today’s visit, your compliance history was eighty-five (85) percent as of September 4, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Lead Testing: Child Care Rule .0601(f)- Safe Environments Child Care Rule Effective January 1, 2024 Rule Amendment- This rule was amended to include the specific requirements that child care centers must be free of asbestos hazards. This is in addition to the existing requirement that they be free of lead poisoning hazards. In addition to the updates to this rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816, require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. Children were observed participating in group story time, free play activity, transitions and feeding. The infant children was engaged in feeding and personal care routines. You reported that you are not currently serving any children on 2nd shift. The FCCH/Centers Located in a Residence Operator’s Statements of Responsibility form and Verification of Required Information for Operator and Additional Caregivers form were completed during the visit. Children’s records and your staff file were reviewed during the visit. The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was 81 percent as of February 5, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The following violation(s) were observed today: Violation Number Comment Rule 905 Record of monthly fire drills, giving the date, time of day and length of time to evacuate, and operator's signature, were not available. No fire drill log was available for review. .1721 (e )(2) 920 Records were not made available for review. A record for one infant child enrolled in care was not available for review. G.S. 110-91(9) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios in compliance. Children were observed participating in group story time, free play activity, transitions and feeding. The infant children was engaged in feeding and personal care routines. You reported that you are not currently serving any children on 2nd shift. The FCCH/Centers Located in a Residence Operator’s Statements of Responsibility form and Verification of Required Information for Operator and Additional Caregivers form were completed during the visit. Children’s records and your staff file were reviewed during the visit. The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was 81 percent as of February 5, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios in compliance. You reported that you are not currently serving any children on 2nd shift. The program’s compliance history was 81 percent as of February 5, 2024.
