You, Jordan Bentley, Program Coordinator, assisted with today’s visit. Your program currently operates with a five (5) star license effective November 13, 2023. Daytime care only; School age children only; Playground area does not meet child playground safety standards; Meets enhanced ratios; Meets enhanced space. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in routines, free play, snack, and departure during the visit. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on February 27, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The last fire drill documented was conducted on December 31, 2025. The last emergency drill documented was conducted on October 31, 2025. A checklist was used to note the requirements I monitored today. I reviewed the ABCMS Provider portal during the visit. Five (5) staff members are listed as employees of your facility. I confirmed all current staff members are included in your facility portal. Files Reviewed: Five (5) children’s files were reviewed during the visit. One (1) new staff file and one (1) existing staff file were reviewed during the visit. The following violations were observed/documented during today’s visit: Violation Number Comment Rule 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. A staff member employed on December 8, 2023, did not have emergency information on file for review. .0701(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A staff member employed on April 24, 2006, did have a current First Aid certificate of completion on file for review. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A staff member employed on April 24, 2006, did not have a current CPR certificate of completion on file for review. .1102(d) 1445 All group leaders were not at least 18 years of age, have a high school diploma, and/or had not completed Basic School Age Care (BSAC) training. A staff member employed on November 10, 2025, did not have a current BSAC certificate of completion on file for review. .2510(c) The violation(s) documented must be corrected immediately. On or before January 28, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Staff should monitor the checklist throughout the year to ensure all required information and training is present, current, and on file for review. BSAC Training: I suggested that all staff complete the Staff File Checklist annually, which includes BSAC training. CPR/First Aid Training: I suggested that all staff complete the staff file checklist annually, which includes CPR/First Aid Training. We reviewed all three (3) pathways, as well as the Education standards, Continuous Quality Improvement Standards (CQI), and Family and Community Engagement Standards. I encouraged you to visit the QRIS Modernization website to review information regarding the changes to the rated license process. Upon arrival, the purpose of today’s visit was reviewed with you, Christina Alford, Program Coordinator. Your last annual compliance visit was conducted on February 8, 2024. This program currently operates with a Five star rated license effective November 13, 2023. License restrictions are Daytime care only; School age children only; Playground area does not meet child playground safety standards; Meets enhanced ratios, Meets enhanced space. A walk-through of all licensed indoor and outdoor space was conducted during today’s visit. You, Christina Alford, assisted me with today’s visit. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during individual center activities, teacher directed activities, handwashing, toileting, gross motor play, lunch, and rest and digest. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on April 26, 2023. Sanitation Inspection: The last sanitation inspection was completed on February 21, 2024. A “superior” classification was issued with four (4) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last shelter-in-place drill was completed on July 30, 2024, and recorded. The last fire drill was completed on July 30, 2024, at 1:00pm and recorded. A checklist was used to note the requirements I monitored today. Files Reviewed: 6 (six) staff files were reviewed today. The following violations were observed during the walk through today. I discussed these violations with you in detail at the end of the visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed on April 26, 2023. 10A NCAC 09 .0304(a) 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. In Space #1 one child was not signed out on August 6, 2024. 10A NCAC 09 .0302(d)(4) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. No medical authorization paperwork was on file for a child who had an emergency medication on site. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) Technical Assistance: Fire Inspection: I recommend that staff contact the fire inspector at least one month prior to the last inspection expiration date to ensure that an annual inspection is completed to maintain compliance with all child care rules and requirements. Arrival and Departure: I recommend staff review all arrival and departure sheets daily to ensure each child enrolled in the program has been signed in and out at the correct time. This process will ensure the safety of all children enrolled in the program and compliance with all child care rules and requirements. Medications: I recommend that staff be instructed to require parents/guardians complete all medication paperwork, including medication authorization, medication administration, and a medical care plan (if applicable), in its entirety before accepting any medication for a child enrolled in the program. Consultation: Prior to today’s visit, your compliance history was ninety-seven (97) percent as of August 15, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Lead Testing: Child Care Rule .0601(f)- Safe Environments Child Care Rule Effective January 1, 2024 Rule Amendment- This rule was amended to include the specific requirements that child care centers must be free of asbestos hazards. This is in addition to the existing requirement that they be free of lead poisoning hazards. In addition to the updates to this rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816, require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The program is free, and the first step is to watch one of the pre-enrollment webinars, which are available at www.cleanwaterforUSkids.org/carolina or paste https://bit.ly/3CK-webinar All licensed child care programs must register by May 1, 2024, at www.cleanwaterforUSkids.org/carolina and watch one of the pre-enrollment webinars. This program currently operates with a Five star rated license effective November 13, 2023. License restrictions are Daytime care only; School age children only; Playground area does not meet child playground safety standards; Meets enhanced ratios, Meets enhanced space. A walk-through of all licensed indoor and outdoor space was conducted during today’s visit. You, Christina Alford, assisted me with today’s visit. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during individual center activities, teacher directed activities, handwashing, toileting, gross motor play, lunch, and rest and digest. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on April 26, 2023. Sanitation Inspection: The last sanitation inspection was completed on February 21, 2024. A “superior” classification was issued with four (4) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last shelter-in-place drill was completed on July 30, 2024, and recorded. The last fire drill was completed on July 30, 2024, at 1:00pm and recorded. This incident report shall include: (1) facility identifying information; (2) the child's name; (3) date and time of the incident; (4) witness to the incident; (5) time the parent is notified of the incident and by whom; (6) piece of equipment involved, if applicable; (7) cause of injury, if applicable; (8) type of injury, if applicable; (9) body part injured, if applicable; (10) where the child received medical treatment, if applicable; (11) description of how and where the incident occurred, and the First Aid received; and (12) steps taken to prevent reoccurrence. This report shall be signed by the person completing it and by the parent, a copy given to the parent or a parent signature declining a copy and the report maintained in the child's file.
