Name of Operation: GRANITE FALLS ELEMENTARY PRE-K Facility ID: 14000419 Consultant: DINAH BROWN Operation Type: Center Case Number: Visit Date: 1/14/2026 Number Present: 23 Completed Date: 1/14/2026 Age: From 3 To 5 Total Minutes: 225 Time In: 11:00 AM Time Out: 02:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for a routine unannounced visit. You, Donna Bumgarner, Lead Teacher , assisted with today’s visit. Your program currently operates with a five (5) star license effective January 9, 2023. The permit restrictions were in compliance including Daytime care only; Certified Developmental Day; Enhanced ratios reduced by one; Meets enhanced ratios; Meets enhanced space A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in routines, lunch, group time, free play, rest time, and departure during the visit. Required Inspections: The last fire inspection was conducted on October 27, 2025. The last sanitation inspection was conducted on August 25, 2025. A “superior” classification was issued with zero (0) demerits noted on the grade card. The most recent playground inspection was conducted on December 15, 2025. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on March 3, 2023. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The most recent fire drill was conducted on December 17, 2025. The most recent emergency drill was conducted on November 20, 2025. I reviewed the ABCMS Provider portal during the visit. Six (6) staff members are listed as employees of your facility. I confirmed all current staff members are included in your facility portal. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training, and Criminal Background Checks. No new staff members have been hired since the last visit. The following violations were observed/documented during the visit: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. One December 17, 2025, one child was not signed out in space #1. On January, 6, 2026, one child was not signed out in space #1. On January, 6, 2026, one child was not signed out in space #2. 10A NCAC 09 .0302(d)(4) 608 Children did not wash their hands upon arrival at the center, after each visit to the toilet, before eating, before and after water activity play, after outside play, and after handling animals or animal cages. Children in space #1 did not wash hands before lunch. 15A NCAC 18A .2803(c) The violation(s) documented must be corrected immediately. On or before January 28, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Please send your letter to dinah.brown@dhhs.nc.gov. You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Prior to today's visit, your program maintained a compliance history of 99%. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Technical assistance was provided on the following: Arrival/Departure: I suggested that you designate one staff member to monitor the arrival and departure records at designated times each day to ensure all children have been signed in and signed out. Handwashing: I reviewed handwashing requirements with you and staff during the visit. You may access resources regarding handwashing routines by visiting www.ncrlap.org. The NC Health and Safety Resource Center designs posters to improve health and safety practices in early care and education programs. Early care and education programs can download posters or order them free of cost at https://healthychildcare.unc.edu/resources/posters/. Child care center employees shall ensure that children wash their hands as follows: (1) upon arrival at the child care center; (2) after each diaper change or visit to the toilet; (3) before eating meals or snacks; (4) before and after water play; (5) after being outdoors; and (6) after handling animals or animal cages. (d) Except when the action that necessitates handwashing is diapering and before eating meals or snacks, hand antiseptics may be used in lieu of handwashing while a child is outdoors, provided that the child's hands are washed when the child returns indoors. Consultation As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. We discussed the QRIS Modernization and the Pathway to the Stars. We reviewed all three (3) pathways, as well as the Education standards, Continuous Quality Improvement Standards (CQI), and Family and Community Engagement Standards. I encouraged you to visit the QRIS Modernization website to review information regarding the changes to the rated license process. Emergency Drills/Playground Inspections: The last lockdown drill was completed on September 4, 2024, and recorded. The last playground inspection was completed on September 4, 2024, and recorded. The last fire drill was completed on August 29, 2024, at 8:30am and recorded. A checklist was used to note the requirements I monitored today. Files Reviewed: Five (5) staff files were reviewed today. Four (4) children’s files were reviewed today. The following violation was observed during the walk through today. I discussed this violation with you in detail at the end of the visit. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The most recent sanitation inspections were completed on May 30, 2023, and September 3, 2024. 10A NCAC 09 .0304(b) Technical Assistance: I recommended that you contact environmental health at least one month prior to the expiration of the current sanitation inspection to ensure a sanitation inspection is completed annually in order to maintain compliance with all child care rules and requirements. Consultation: Prior to today’s visit, your compliance history was ninety-nine (99) percent as of September 19, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Lead Testing: Child Care Rule .0601(f)- Safe Environments Child Care Rule Effective January 1, 2024 Rule Amendment- This rule was amended to include the specific requirements that child care centers must be free of asbestos hazards. This is in addition to the existing requirement that they be free of lead poisoning hazards. In addition to the updates to this rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816, require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The program is free, and the first step is to watch one of the pre-enrollment webinars, which are available at www.cleanwaterforUSkids.org/carolina or paste https://bit.ly/3CK-webinar All licensed child care programs must register by May 1, 2024, at www.cleanwaterforUSkids.org/carolina and watch one of the pre-enrollment webinars. Your last annual compliance visit was conducted on October 5, 2023. You, Donna Bumgarner, Lead Teacher, assisted with the visit. Your program currently operates with a five-star license effective January 9, 2023. The permit restrictions were in compliance including daytime care only, enhanced ratios reduced by one, Certified Developmental Day, meets enhanced ratios and meets enhanced space. Fire Inspection: Your last fire inspection was conducted on April 23, 2024. The facility was approved for daytime care only. Sanitation Inspection: The last sanitation inspection was made on May 30, 2023, by Logan Little, Environmental Health Specialist. A “superior” classification was issued with six (6) demerits noted on the grade card. NC Pre-K and Developmental Day: NC Pre-K and Developmental Day requirements were monitored and in compliance. Your last monthly fire drill was conducted on April 9, 2024 at 8:40am. Your last emergency shelter-in -place/lockdown drill was conducted on March 6, 2024. Your last monthly playground inspection was conducted on April 22, 2024. Children were observed during teacher lead activity, outdoor gross motor play, toileting, handwashing, transition to lunch and nap/rest. A checklist was used to note the requirements I monitored today. The following violations were observed: Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A lockdown drill was conducted on September 9, 2023 and again on March 6 2024. .0604(u);.0302(d)(8) Technical Assistance: Emergency Drills: Shelter-in-place and lockdown drills are required to be conducted and documented every three months to comply with child care rules. A Lockdown drill was conducted on September 12, 2023, and again on March 6, 2024. In addition to following and participating in your school’s emergency drills, I recommend using the Emergency drill log to conduct and document either a shelter-in-place or lockdown drill every three months to ensure compliance with child care rules. Consultation: I recommend that you customize your safe arrival and departure procedures to be specific to your facility as to the exact routine/schedule and procedures that children and staff use to ensure the safety of children as they arrive and depart at the beginning and end of each day. Prior to today’s visit, your compliance history was one hundred (100) percent as of April 29, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score.
