You, Sherry Bolen, Owner/Operator, assisted with today’s visit. Your program currently operates with a four-star license issued on August 30, 2021. The restrictions listed on your license include daytime care only, maximum of five (5) preschool children at any time, garage door must be open when children in care on basement level and serves no more than two children less than one year of age. The Secretary of State website was checked on April 23, 2026. Country Care Child Care Home was not listed. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the FCCH was completed today; all licensed and approved space was monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the FCCH were participating in rest time, routines, free play, snack, outdoor gross motor play, and departure during the visit. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on March 15, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The last fire drill was conducted and conducted on April 21, 2026. The last emergency drill was conducted and documented on April 23, 2026. I reviewed the ABCMS Provider. I verified that the two people living in your home are listed. A checklist was used during today’s visit to monitor compliance. Files Reviewed: One child's file was reviewed during the visit. One (1) existing staff file was reviewed during the visit. Rated License: Sherry Bolen, Owner/Operator, has chosen to meet the requirements for Pathway #2 for a FCCH, Classroom and Instructional Quality. I received your Application for Assessment for a Rated License for a FCCH on April 23, 2026. You stated that you will send your facility CQI plan and Professional Development Plan (PDP) and Family and Community Engagement Standards, to me by May 15, 2026. Individual CQI Plans must be completed annually and a copy placed on file for review. One violation was observed/documented during today’s visit. Violation Number Comment Rule 716 Electrical outlets not in use were not covered. I observed one electrical outlet that was not covered and was accessible to children located behind the door between the mirror and cabinets approximately three and one halt to four feet from the floor. 10A NCAC .1719(a)(27) Technical assistance was provided on the following: Electrical Outlets: I suggested that you monitor all spaces throughout the day to ensure all electrical outlets not in use are covered with a safety plug. Instruct staff to check all electrical outlets at the beginning of the day. You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Prior to today's visit, your program maintained a compliance history of 97%. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. Your program currently operates with a four-star license issued on August 30, 2021. As we continue expanding our training offerings, DCDEE has established a new email address and phone number for Moodle support. To get help with Moodle, email at DCDEE_Moodle_Support@dhhs.nc.gov or call (919) 814-6326. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted.
