You, Tori Lingerfelt, Program Coordinator, assisted with today’s visit. Your program currently operates with a five (5) star license effective November 9, 2023. The permit restrictions were in compliance including daytime care only, school age children only, playground area does not meet child care playground safety standards, meets enhanced ratios, meets enhanced space. You have earned seven (7) points in education, six(6) points in program standards and zero (0) quality points for a total of thirteen (13) points. The Secretary of State website was checked on March 31, 2026. Caldwell County Schools was not listed. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today; all licensed and approved space was monitored. I observed children in the indoor and outdoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in routines, free play, snack, outdoor gross motor play, and departure during the visit. You received the results of your most recent lead water testing showing no levels of lead above the allowed amount, on February 5, 2024. Per the facility data on the Clean Classrooms for Carolina Kids website, your facility is exempt from lead-based paint and asbestos testing. Emergency Drills: The last fire drill documented was conducted on March 27, 2026. The last emergency drill documented was conducted on March 27, 2026. I reviewed the ABCMS Provider portal during the visit. I confirmed three (3) current staff members are included in your facility portal. File Reviewed: One (1) existing staff file was reviewed during the visit. No new staff have been employed since the last monitoring visit. Rated License: Phyllis Crump, Administrator, has chosen to meet the requirements for Pathway #1, Program Assessment. I received your Application for Assessment for a Rated License for Centers on March 25, 2026. You stated that you have completed the three-month self-study that began in November 2025. As a result of the self-study and mock SACERS assessments that you had conducted from an outside agency, your program is receiving supplemental materials. You stated that you will send the self-study, facility and individual CQI plans, Family and Community Engagement Standards, Staff Education and Experience Worksheet, and the Rated License Assessment Request Review form to me by April 13, 2026. I will submit your request to NCRLAP for your Environment Rating Scale at Baton Wraparound when your request for review has been received. Individual CQI Plans must be completed annually and a copy placed on file for review. One violation was observed/documented during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. On March 19, 2026, four children were not signed in and one child was not signed out. On March 20, 2026, one child was not signed out. 10A NCAC 09 .0302(d)(4) Technical assistance was provided on the following: Arrival/Departure Records: I suggested that you designate one staff member to monitor the arrival and departure records at designated times each day to ensure all children have been signed in and signed out. You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4) (c). Prior to today's visit, your program maintained a compliance history of 97%. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me at least thirty days prior to any changes occurring. A checklist was used to note the requirements I monitored today. I reviewed the ABCMS Provider portal during the visit. Your facility is listed, and three staff members are listed under your facility portal. Files Reviewed: Three (3) children’s files were reviewed during the visit. One (1) new staff file and one (1) existing staff file were reviewed during the visit. The following violations were observed/documented during today’s visit: Violation Number Comment Rule 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. A staff member employed on May 8, 2024, did not complete and have a certificate of training on file. .1102(g) 1898 Staff did not complete the health and safety training within one year of employment. One staff member employed on May 8, 2024, did not complete and have certificates for trainings on file for review. .1102(a) The violation(s) documented must be corrected immediately. On or before September 30, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Additionally, I suggested you use the staff file checklist found on the DCDEE website under the “Provider” tab to ensure all required documentation is on file. I also provided you with a copy of the staff file checklist during the visit. I suggested you set a calendar reminder to ensure trainings for new staff members are completed in a timely manner. I also suggested you use the Health and Safety Training Record to keep track of required trainings and to ensure trainings are completed on time. Upon arrival, the purpose of today’s visit was reviewed with you, Tori Lingerfelt, Director. Your last annual compliance visit was conducted on October 10, 2023. Your program currently operates with a five (5) star license effective November 9, 2023. The permit restrictions were in compliance including daytime care only, school age children only, playground area does not meet child care playground safety standards, meets enhanced ratios, meets enhanced space. You have earned seven (7) points in education, six(6) points in program standards and zero(0) quality point for a total of thirteen (13) points. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on March 18, 2024. Sanitation Inspection: The last sanitation inspection was completed on September 24, 2024. A “superior” classification was issued with four (4) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last lockdown drill was completed on September 12, 2024, and recorded. The last fire drill was completed on September 12, 2024, at 3:00pm and recorded. A checklist was used to note the requirements I monitored today. Files Reviewed: Two (2) staff files were reviewed today. Four (4) children’s files were reviewed today. The following violation was observed during the walk through today. I discussed this violation with you in detail at the end of the visit. Violation Number Comment Rule 104 Center has not passed required sanitation inspection and received an approved or superior rating. The most recent sanitation inspections were dated August 28, 2023 and September 24, 2024. 10A NCAC 09 .0304(b) Technical Assistance: Required Inspections: I recommended that you contact the environmental health inspector at least one month prior to the expiration of the current sanitation inspection posted to maintain compliance with all child care rules and requirements. Consultation: Prior to today’s visit, your compliance history was ninety-four (94) percent as of September 24, 2024. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Lead Testing: Child Care Rule .0601(f)- Safe Environments Child Care Rule Effective January 1, 2024 Rule Amendment- This rule was amended to include the specific requirements that child care centers must be free of asbestos hazards. This is in addition to the existing requirement that they be free of lead poisoning hazards. In addition to the updates to this rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816, require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The program is free, and the first step is to watch one of the pre-enrollment webinars, which are available at www.cleanwaterforUSkids.org/carolina or paste https://bit.ly/3CK-webinar All licensed child care programs must register by May 1, 2024, at www.cleanwaterforUSkids.org/carolina and watch one of the pre-enrollment webinars. Upon arrival, the purpose of today’s visit was reviewed with you, Tori Lingerfelt, Director. Your last annual compliance visit was conducted on October 10, 2023. Your program currently operates with a five (5) star license effective November 9, 2023. The permit restrictions were in compliance including daytime care only, school age children only, playground area does not meet child care playground safety standards, meets enhanced ratios, meets enhanced space. You have earned seven (7) points in education, six(6) points in program standards and zero(0) quality point for a total of thirteen (13) points. A walk-through of the facility was completed today, You, Tori Lingerfelt, Director, assisted me with today's visit. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during transitions, independent free choice center play, snack, handwashing, and departure. Inspections/Emergency Drills: Fire Inspection: The last fire inspection was completed on March 18, 2024. Sanitation Inspection: The last sanitation inspection was completed on September 24, 2024. A “superior” classification was issued with four (4) demerits noted on the grade card. Emergency Drills/Playground Inspections: The last lockdown drill was completed on September 12, 2024, and recorded. The last fire drill was completed on September 12, 2024, at 3:00pm and recorded. Currently, this facility operates with a five-star child care license issued on August 29, 2017, with the following restrictions noted on the license: daytime care only, school age children only, enhanced ratios reduced by 1, seven point group size reduced by 1, playground area does not meet child care playground safety standards, meets enhanced space and meets enhanced ratios. A walk-through of the facility was completed today, Space #7 was in use during the visit. I conducted the walk-through alone, while you were engaged in arrival activities with the children. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children were observed during arrival, afternoon snack, departure, and indoor free play. Any violation(s) documented may impact the compliance history score. Technical Assistance: • Item #840: Potentially hazardous items and items under pressure must be kept in locked storage. I suggested that you conduct daily safety checks to ensure the cabinet located in the teacher area is kept locked at all times. You corrected this violation by placing the lock on the cabinet and securing the lock on the cabinet. Rated License: You previously had the SACERS-U assessment conducted on May 1, 2023. A rated license visit was needed to complete the rated license reassessment. The facility will now qualify for seven points in education and six points in program standards for a total of thirteen points and a five-star child care license.
