This visit was conducted by Turkessha Casterlow, Child Care Consultant, with Tracey Harris, Operator. Your program currently operates with Two Star license effective June 18, 2025. Restrictions include Daytime care only; Maximum of 5 preschool children at any time; Children in care on ground level only; Trampoline not in use during operating hours; No more than 2 infants under age 1. The program’s compliance history was 94% prior to today’s visit. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • ITS-SIDS • CBC Qualification • General Health Administering Medication • Storage of Hazardous Products • Storage of Medication • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation The license was observed, and the restrictions were found in compliance. Individuals living in the home were discussed and are recorded on the Verification of Required Information for Operator and Additional Caregiver form. Be reminded that everyone living in the home must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A walk-through of the home was completed today. Indoor and outdoor areas were observed. We tested the smoke detector during today’s visit, and it worked. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children during free play and group time. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, ITS-SIDS, and Criminal Background Checks. No medications were observed nor reported. Storage hazardous items were monitored today. A playground inspection was recorded on April 8, 2026. A fire drill was recorded on April 8, 2026. A shelter in place drill for the facility was recorded on February 9, 2026. There are no pets reported in the home. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The ABCMS system has not been updated with the current residents. G.S. 110-90.2 & .2703(r) 2022 Operator and/or staff members did not complete the required health and safety trainings within the specified time frame. Inspections identify hazards like loose parts, broken equipment, or tripping hazards before they cause accidents, and ensure surfaces have adequate cushioning to reduce impact from falls. Keeping infants safe with regular checks in child care is critical to prevent sleep-related deaths, such as SIDS and suffocation. Consistent monitoring allows caregivers to ensure the sleeping environment remains safe, that infants are on their backs, and that they are not overheating. Checks also provide a quick response if an infant shows signs of distress or an issue arises with the sleep space, which is vital because many sleep-related deaths occur in the first year of life. Keeping infants safe with regular checks in child care is critical to prevent sleep-related deaths, such as SIDS and suffocation. Consistent monitoring allows caregivers to ensure the sleeping environment remains safe, that infants are on their backs, and that they are not overheating. Checks also provide a quick response if an infant shows signs of distress or an issue arises with the sleep space, which is vital because many sleep-related deaths occur in the first year of life. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”. These training courses are free, and participants will receive training certificates when completed. You must have 10 ongoing training hours each year and only five (5) additional hours can be brought forward. • Preparing for North Carolina’s New QRIS System North Carolina’s Quality Rating Improvement System (QRIS) is being modernized to provide more flexible and equitable ways for programs to demonstrate quality. Soon you will begin to choose from three star-rating pathways: Program Assessment, Classroom & Instructional Quality Accreditation. This new approach allows programs to align their strengths with the pathway that best fits their operations. Now is the time to prepare. Programs should begin evaluating staff qualifications including updating staff education in NC Works, curriculum, and classroom interactions to determine the most suitable pathway. I observed two children present upon our arrival involved in different free play and breakfast activities. The temporary license was issued to this family child care home on 12/17/24 and expires on 6/17/25. The Verification of Required Information for Operator and Additional Caregiver form was completed during today's visit. The last fire drill was completed on April 15, 2025. The last outdoor play inspection was completed on April 24, 2025. In addition, the quarterly shelter-in-place was completed on February 24, 2025. The provider informed me during today's visit that she is in the process of signing up to complete EPR training with her local resource and referral agency. Once completed, I informed Mrs. Harris that the EPR plan needs to be completed in the risk management portal within four months after the completion of the training. The provider does not provide transportation. The license was posted along with the NC Summary of the Law, Emergency Number and Emergency Procedures. A first aid reference sheet was available. The daily schedule and activity plan were also posted and available for review. Attendance records are maintained in the center’s Brightwheel app. Keep this information on file for at least one year. COMPLIANCE HISTORY: Prior to today’s visit, the 18-month compliance history for this facility was 93%. I would recommend that you complete your training three months prior to the above date. *Emergency Preparedness and Response (EPR)-Again the provider has not completed this training as of yet but once completed you must be complete the template within four months of receiving the training (please read below): 10A NCAC 09 .1714 EMERGENCY PREPAREDNESS AND RESPONSE (b) Existing family child care home operators (operator or operators) shall complete the Emergency Preparedness and Response in Child Care training. Within one year of the effective date of a new license, the operator of a new family child care home shall have completed the Emergency Preparedness and Response in Child Care training. When the trained staff member leaves employment, the family child care home shall ensure that another staff member completes the required training within four months of the vacancy. Documentation of completion of the training shall be maintained in the operator's personnel file. (c) Upon completion of the Emergency Preparedness and Response in Child Care training, the operator shall develop the Emergency Preparedness and Response Plan. The Emergency Preparedness and Response Plan means a written plan that addresses how a child care facility will respond to both natural and man-made disasters, such as fire, tornado, flood, power failures, chemical spills, bomb threats, earthquakes, blizzards, nuclear disaster, or a dangerous person in the vicinity, to ensure the safety and protection of the children and additional caregivers. This plan shall be on a template provided by the Division available at https://rmp.nc.gov/portal/# completed within four months of completion of the Emergency Preparedness and Response in Child Care training, and available for review RATED LICENSE INFORMATION: I discussed with Mrs. Harris the rated license process. I informed Mrs. Harris of the Environmental Rating Scale Assessment, and the provider stated that she will be interested in having this completed. I did review the Rated License Request Form with Mrs. Harris during today's visit and we reviewed it. I will email this form to Mrs. Harris and she will initial and sign and email back to me by the end of March. I did discuss the FCCERS-R book with Mrs. Harris and informed her that she would need to get this book to assist her in preparing for the assessment. Lastly, I informed her to begin working on her reflective self-study (this was emailed). I informed her I would need this by the first week in April. The provider Mrs. Tracey Harris was present and assisted me during today's visit. I completed a general walk through of the daycare space, bathroom and outdoor play area. I observed three children preparing for lunch upon arrival. The temporary license was issued to this family child care home on 12/17/24 and expires on 6/17/25. The Verification of Required Information for Operator and Additional Caregiver form was completed during today's visit and reviewed with the provider). The fire drill was last completed on 1/24/25 and the outdoor play inspection was last completed on 1/11/25. The provider does not want to be approved for transportation at this time but if anything changes in the future, I informed Mrs. Harris that before taking the children in any vehicle, to please contact me so I can make sure the vehicle is approved with all inspections and all necessary paperwork for transporting is available in the vehicle. The license was posted along with the NC Summary of the Law. A first aid reference sheet was available, and the daily schedule was posted. Attendance records have been maintained since opening. Keep this information on file for at least one year. The provider and children's records were reviewed during today's visit.
