This visit was conducted by Turkessha Casterlow, Childcare Consultant with Renee Lynch, Director. Your program currently operates with a Notice of Compliance. The program’s compliance history was 96% prior to today’s visit. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children during free play, routine care, and playing in their outside learning environment. The last annual compliance visit was conducted on November 5, 2024. A sanitation inspection was completed on February 27, 2026, with a Superior classification. The last fire inspection was conducted on February 26, 2026. This facility reopened on Monday March 2, 2025. Please complete a fire drill, playground inspection, and emergency drill by March 31, 2026. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 9995 A violation was found for which there is no item number. An EpiPen is located at the facility for a child that does not have a completed permission to administer medication form. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before March 19, 2026. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation You may email the letter to turkessha.casterlow@dhhs.nc.gov. If you state in your letter that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action being taken by the DCDEE. Technical assistance discussed during today’s visit related to violations • Having written permission to administer an EpiPen is critical in child care to ensure immediate, lifesaving, and legally compliant care during anaphylactic emergencies when parents are unavailable. This documentation protects staff from liability, ensures the medication is given correctly, and guarantees that trained staff are prepared to act. CONSULTATION: We reviewed the rule changes for Environmental Health today. Demerits will be cited beginning 1/1/24 on new rules and rule updates. We discussed the protocol if a child enrolls with a religious exemption from immunizations, you must have documentation on file and the parent must have notified the local health department. If any enrolled child becomes sick with a reportable, communicable disease, the child cannot attend until the illness is no longer present. If staff member M. Kaulinis subs again, she must update her emergency contact form and annual health questionnaire and review the EPR and Emergency Medical Care plans.
