This visit was conducted by Turkessha Casterlow, Childcare Consultant with Tara Stephens, Administrator. Your program currently operates with a Four-Star license effective September 17, 2019. Restrictions include Daytime care only; No cooking allowed in building; Meets enhanced ratios. The program’s compliance history was 96% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CBC Qualification • CPR/First Aid • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children eating snacks, participating in free play, and outside activities. No new staff were reported at this program. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, Special Training, and Criminal Background Checks. No medications were observed nor reported. Storage hazardous items were monitored today. A playground inspection was recorded August 11, 2025. A fire drill was recorded on August 11, 2025. A lock down drill for the facility was recorded on August 11, 2025. The most recent fire inspection was on March 3, 2025. I observed a sanitation inspection was last conducted on September 4, 2024, earning a Superior classification. You stated that you do provide transportation in the afternoon for 21 children. During today’s visit, I monitored emergency information and bus logs. I observed first aid kits and fire extinguishers on the vehicles. I monitored two vehicles today. I observed a 2007 Doge Carvan, Tag #FLA-2676. I also observed a 2016 Chevy Express, Tag #KHC-9375. The vehicles were clean, and in good repair, they had working seatbelts, the Chevy had adequate tire tread. We discussed the thread on the front passenger side of the Dodge. The vehicles were current on inspection and insurance. Your next inspection date on this vehicle is due for the Dodge by February 28, 2026, and the Chevy by July 31, 2026. and the insurance is valid through November 5, 2025. The following violations were cited during today’s visit: Violation Number Comment Rule 1125 Before children were transported, written permission from a parent was not obtained that included when and where the child was to be transported, expected time of departure and arrival, and the transportation provider. Permission to transport forms did not contain all the required information. .1003(i)(j) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before September 9, 2025. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation You may email the letter to turkessha.casterlow@dhhs.nc.gov. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance discussed during today’s visit: • Signed transportation forms are crucial in child care to establish clear communication, ensure child safety through detailed information and consent, and reduce liability for both the center and parents by documenting permissions, emergency procedures, and transportation details. These forms help prevent miscommunication, ensure children are transported safely and by authorized individuals, and provide vital information in emergencies. • Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • Double check with Sanitation regarding scheduling to ensure that your next sanitation inspection does not become delinquent. • Be sure to place your vehicle insurance cards in each of your transportation binders. Make sure the binders are on the vehicle while children are being transported. • All children that are transported should have written and signed permission forms from the parents acknowledging the location where the child is being picked up and where the child is being dropped off. The form should include the driver’s name, times the child will be transported, and emergency contact information for the parent. • Preparing for North Carolina’s New QRIS System North Carolina’s Quality Rating Improvement System (QRIS) is being modernized to provide more flexible and equitable ways for programs to demonstrate quality. Beginning later this year, providers will be able to choose from three star-rating pathways: Program Assessment, Classroom & Instructional Quality Accreditation. This new approach allows programs to align their strengths with the pathway that best fits their operations. Now is the time to prepare. Programs should begin evaluating staff qualifications including updating staff education in NC Works, curriculum, and classroom interactions to determine the most suitable pathway. Updates to staff qualification standards and assessment tools are expected, and while current star ratings remain valid for now, all new ratings will be issued under the revised QRIS system. Your program’s quality should reflect not only compliance, but also a strong commitment to best practices, child development, and continuous improvement. Providers are encouraged to take advantage of available technical assistance and training opportunities. A provider meeting will be held on September 29, 2025, from 1:00–3:00 PM at Impact Alamance. Please plan to attend this meeting to learn more about the new QRIS system. As you prepare for the QRIS be sure to double-check that all your staff’s education is listed in NC Works. The link is located on the DCDEE website under Services, select the drop-down box and select DCDEE Works. At the completion of the visit, this visit summary was reviewed with and provided to you. You were provided with an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Childcare licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. A checklist was used to note the requirements I monitored today, and a copy was left with you. The last annual compliance visit was conducted 3/6/23. Currently this program operates with a four (4) star license, issued 9/17/19. The facility earned four (4) points in staff education, five (5) points in program standards, and one (1) quality point form educational options met. Tara Stevens, program coordinator, assisted me today. I conducted an assisted walk-through of the center. The last sanitation inspection was completed 10/17/23 with a ‘Superior’ classification and 05 demerits. The last fire inspection was conducted on 3/10/23. Children were observed playing in activity areas, eating lunch, and routine care activities. Children did not attend school today due to the elections. Transportation is provided. The center uses three vans to transport. One Ford van HAD 7731, one Ford van TEY 1256, and one Dodge van FLA 2676. The vans were not on site today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. The emergency information form on file for all staff was dated 1/19/23. .0701(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. The training for M. Lawson expired 12/23. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. The training for M. Lawson expired 12/23. .1102(d) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Staff member T. Stevens was short 2.5 hours and M. Lawson was short 4.5 hours. Copies of the training certificates must accompany the corrective action letter .1103(a) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. The exclusion requirements for COVID have been updated by the CDC. The new guidelines state a person testing positive must exclude but can return after24 hours fever free with no medications and symptom improvement. Your next fire inspection must be completed by March 9, 2024. A copy must be sent to me within seven days. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The date of the last inspection was 2/8/23. The remainder of the health and safety trainings must be completed by 8/25/24. As a reminder, any health and safety trainings completed prior to date of hire are only valid if less the twelve months old on date of hire. The CPR/First Aid for M. Lawson expires 12/23.
