Upon my arrival, I was greeted by the Director, Alexis Bell, she assisted me throughout the visit. I conducted a walkthrough of all classroom spaces, documented enrollment and attendance records, and completed observations. Children were observed participating in free play in their classroom and having group time. The last annual compliance visit was conducted on November 6, 2024. The center is currently operating with a 5-star license issued on May 8, 2024. The most recent sanitation inspection was conducted on June 24, 2025, with the center receiving a Superior rating. Fire drills and playground inspections were reviewed, and the most recent fire inspection was conducted on December 30, 2024. All staff files were monitored along with four children’s files. Your compliance history is currently 97% this percentage may change based on the results of today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. I observed the following violation during today’s visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference.In space YA2/NCPrek2, the activity plan was not posted for the NC Prek. GS 110-91(12); .0508(a) The violation was corrected during today's visit. No corrective action letter is required. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ , on the home page under the ‘Services’ tab. Technical Assistance Ensure current activities plan are posted. The Quality Rating Improvement System (QRIS) is being modernized to provide more flexible and equitable ways for programs to demonstrate quality. Starting later this year, providers will be able to choose from three star-rating pathways—Program Assessment, Classroom & Instructional Quality, or Accreditation—allowing you to align your strengths with the rating method that best fits your program. Now is the time to evaluate your current practices, staff credentials, curriculum use, and classroom interactions to determine which pathway is the best fit. Programs are encouraged to begin preparing for updates to staff qualification standards and assessment tools. While existing star ratings will remain valid for now, all new star ratings will be subject to the new system. Your program’s quality should reflect not just compliance—but a commitment to best practices, child development, and continuous improvement. Stay informed, stay proactive, and take advantage of technical assistance and training opportunities as they become available. The last annual compliance visit was conducted 11/6/24. Currently this program operates with a five (5) Star License, issued 5/8/24. The facility earned seven (7) points in staff education, six (6) points in program standards, and one (1) quality point for programmatic options met. Center director Alexis Bell assisted me today. I conducted an unassisted walk-through of the center. The last sanitation inspection was completed 7/25/24 with a ‘Superior’ classification and 0 demerits. The last fire inspection was conducted on 12/30/24. Children were observed playing in activity areas and routine care activities. The center has two NCPK classrooms with twenty-one children enrolled. The NCPK requirements were monitored during today’s visit. The program operates for 6.5 hours a day. A monthly breakfast and lunch menu was posted in each classroom. Two NC Pre-K children's files were monitored and each contained a health assessment within 30 days and evidence of each child's on-going progress. The teachers use the Creative Curriculum and activity plans were current and posted. Staff/child ratio of 1:9 or less is being maintained. The parents receive a monthly newsletter, receive updates through Teaching Strategies, and they are able to volunteer throughout the school year. The teachers also have parent meetings and conferences three times a year. The staff noted on the Program Consultant Site Visit information screen was correct. The current ECERS-R score was received 10/10/19. The facility received a score of 5.63. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. The medical report for A. Kimes was received after the hire date. Her hire date was 12/17/24 and the report date was 4/10/25. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. The TB test on file for A. Kimes was older than twelve months on date of hire. .0701(a) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. A checklist was used to monitor the childcare requirements today, and a copy was left with you. The is the facility’s first annual compliance visit after a change of ownership. Currently this program operates with a five (5) Star License, issued 5/8/24. The facility earned seven (7) points in staff education, six (6) points in program standards, and one (1) quality point for programmatic options met. Center director Aimee McNeil is no longer employed. Her last day was 10/21/24. Assistant director Sheila Jeffries assisted me today. I conducted an assisted walk-through of the center. The last sanitation inspection was completed 7/25/24 with a ‘Superior’ classification and 0 demerits. The last fire inspection was conducted on 1/25/24. Children were observed in large group activities, playing outside, eating breakfast, playing in activity areas, and routine care activities. The center has two NCPK classrooms with twenty-two children enrolled. The NCPK requirements were not monitored during today’s visit. Transportation is provided. The facility has two minibuses (FHD-4026 and HMY-1266). Both were available and monitored today. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. The facility had a change of ownership. When I arrived, I was greeted Amber Carter. The Administrator, Aimee McNeal arrived during the visit. I explained the purpose of my visit. I conducted an assisted walkthrough of the facility. The last sanitation inspection was conducted 1/24/24 with a ‘Superior’ classification and 2 demerits. The last fire inspection was conducted 1/25/24 and was approved for day-time care only. During today’s visit, the children were observed playing outdoors, playing in activity areas, eating lunch, resting, and routine caregiving activities. The center has two classrooms with thirty-four NCPK children enrolled. The NCPK requirements were monitored during today’s visit. The program operates for 6.5 hours a day. A monthly breakfast and lunch menu was posted in each classroom. Four NC Pre-K children's files were monitored and each contained a health assessment within 30 days, a DIAL screening within 90 days, and evidence of each child's on-going progress. The teachers use Creative Curriculum and activity plans were current and posted. Staff/child ratio of 1:9 or less is being maintained. The parents receive a monthly newsletter, and they are able to volunteer throughout the school year. The teachers also have parent meetings and conferences three times a year. The staff noted on the Program Consultant Site Visit information screen was correct. The current ECERS-R score was received 10/10/19. The facility received a score of 5.63. The facility uses three mini-buses for transportation. Two buses, FHD 4026 and EDA 9507 were on site and monitored today. The red bus was in the repair shop today. The following items were observed in compliance during today’s visit: license posted, summary of law posted, staff:child ratio posted and observed in compliance, supervision, sign in/out sheets current, attendance taken daily, daily schedule posted, menus posted, lesson plans posted, and daily schedule posted. The temporary license was issued 1/6/24 and expires 7/6/24. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. The concrete footing was exposed on the basketball goal post, the tetherball pole, one pole of the installed pergola, and under the half barrel attached to the mud table. .0605(j) 1123 All vehicles used to transport children were not free of hazards. You stated there are concerns about children with behavioral challenges trying to run out of classroom YA1 and creating a supervision or staff/child ratio issue. We discussed adding a gate at the entryway for classroom YA1 to alleviate this concern. Following today’s visit, your center will receive a new permit once your corrective action letter is received. The rated license points will be based upon the previous license issued prior to the change of ownership.
