This visit was conducted by Turkessha Casterlow, Childcare Consultant with Wendy Oakley, Director. Your program currently operates with a Notice of Compliance. The program’s compliance history was 97% prior to today’s visit. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children eating snacks, playing in their outside learning environment, and nap time. The last annual compliance visit was conducted on September 4, 2024. A sanitation inspection was completed June 3, 2025, with a Superior classification. The last fire inspection was conducted on August 11, 2025. Program records and required postings were monitored. A fire drill was conducted on July 31, 2025. A shelter in place drill was documented on July 28, 2025. An outdoor inspection was documented on August 8, 2025. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. You stated that you do provide transportation for field trips. The vehicle was monitored, and all information was current. Registration and Inspection are due January 31, 2026, and insurance expires August 16, 2026. The following violations were cited during today’s visit: Violation Number Comment Rule 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The last EPR update on Risk Management was dated for September 23, 2021. .0607(e) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before September 10, 2025. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation You may email the letter to turkessha.casterlow@dhhs.nc.gov. If you state in your letter that corrections or changes have been made when they have not, it will be considered falsification of information and could lead to administrative action being taken by the DCDEE. Repeated violations, continued non-compliance, or failure to submit the compliance letter prior to the date given in this visit summary can also lead to administrative action being taken by the DCDEE. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance discussed during today’s visit related to violations: • Regularly updating an emergency preparedness and response (EPR) plan is crucial for childcare centers to ensure the safety of children and staff. Because emergencies are often chaotic and unpredictable, an outdated plan can cause critical delays and put lives at risk. A continuously evaluated plan ensures that a provider can respond quickly, calmly, and effectively to any situation. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • Remember to update your EPR Plan yearly to prevent archiving of your account. • The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. A checklist was used to monitor requirements today and a copy was left with you. The last annual compliance visit was conducted 9/12/23. Currently this program operates with a GS110 Notice of Compliance issued 9/19/23. The center director, Wendy Oakley, assisted me today. I conducted an assisted walk-through of the center. The last sanitation inspection was completed 6/14/24 with a ‘Superior’ classification and 7 demerits. The last fire inspection was conducted on 8/19/24. Children were observed playing outside, large and small group activities, and routine care activities. Transportation is being provided for summer field trips. The center is using a full-size school bus. The bus was not onsite to be monitored today. A full assessment of childcare requirements was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The facility's previous inspection was 8/7/23. The current inspection was conducted 8/19/24. 10A NCAC 09 .0304(a) 601 Refrigerator(s) did not maintain a temperature of 45 degrees F. or below. The temperatures in the refrigerators where children's lunches were stored were 48 degrees and 50 degrees. 15A NCAC 18A .2806(j)(2) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Violations cited during the visit were discussed with Ms. Oakley. The violation(s) documented above must be corrected immediately. CONSULTATION AND RESOURCES: I will need to schedule a time to monitor the bus used for transportation. Please provide days/times during the next two weeks when the bus will be on site. We discussed how to work with children exhibiting challenging behaviors. Region 12 Healthy Social Behaviors Coaches are Karissa Livingston karissal@childcareservices.org, Jesse Norris jessen@childcareservices.org, and Cindy Goff cindyg@childcareservices.org. They serve Alamance, Caswell, Durham, Franklin, Granville, Orange, Person, Vance & Wake counties. These coaches are available to assist with classroom observations and behavior plans. In addition, they provide proactive trainings for staff to ensure they are prepared when a challenging behavior arises. There is a challenging behavior hotline you may call to receive assistance. The number is 888-600-1685 option 1. I emailed a copy of the healthy social behaviors training calendar. These trainings are for you and staff. They are a great resource to assist with children and families. Staff member L. Bush has a provisional criminal background letter valid until 9/27/24. We reviewed requirements for a provisional letter. The staff member counts in staff/child ratio but cannot be left alone with children until a qualification letter is received. The criminal background letter or A. Evans is due by 1/7/25. The fire inspection must be conducted by the 364th day of the previous year’s inspection. You have one child enrolled with a religious exemption for immunizations. If a communicable disease is present in the facility, the child must be excluded until it is no longer present. It is best practice for the family to notify the local health department so they can receive additional guidance for the protection of their child’s health. CONSULTATION AND RESOURCES: We reviewed the rules for transportation today. You stated you are recording attendance during the field trips using Brightwheel application. Attendance needs to be recorded using a hardcopy and staff should use the Head Count sheet to do a name to face head count every fifteen minutes while on the field trip. Children’s emergency contact information with their photographs should be in a notebook and kept on the bus. It is recommended to have the emergency contact information for staff in the notebook as well. If any staff or children have a medical action plan, a copy must be in the transportation notebook also. We discussed the staff members who will need to complete CPR/First Aid by 8/31/24.
