    Name of Operation: WAGES BUCK SWAMP CENTER Facility ID: 96000619 Consultant: TAMEKA JOHNSON Operation Type: Center Case Number: Visit Date: 9/26/2024 Number Present: 49 Completed Date: 9/26/2024 Age: From 0 To 4 Total Minutes: 150 Time In: 10:30 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor applicable child care requirements during your annual compliance visit. Upon my arrival I was greeted by the center lead Damika Franklin. Ms. Franklin stated that the director was out today. A checklist was used to note the requirements monitored during today’s visit. I was accompanied by Ms. Franklin as I completed a general walkthrough of the indoor environment which consisted of six (6) classrooms, kitchen and the outdoor environment. The children were observed during outdoor play, center activities, completing routine care tasks. Today’s meal was in compliance with the Meal Patterns. ** License Status The facility is currently operating with a five-star license issued on April 30, 2022. ** Annual Inspections: The last sanitation inspection was completed March 7, 2024, and received a ‘Superior’ classification with 4 demerits. The last fire inspection was completed on March 13, 2024. The last documented fire drill was conducted on September 9, 2024, the last documented lockdown drill was completed on July 8, 2024, and the last documented playground inspection was completed on August 19, 2024. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings and attendance logs. I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Bottles were not labeled and dated for the appropriate child. 15A NCAC 18A .2804(d) 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. Weeds and grass growing through out the surfacing on the playground designated for preschool children. 15A NCAC 18A .2832(a) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Outlet cover missing in the classroom designated for infant children. 10A NCAC 09 .0604(c) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic bag accessible to the children in the classroom designated for two year old children. .0604(q) To comply with the NC Laws and Rules, any violations cited today must be corrected immediately. A compliance letter must be sent to me by October 10, 2024. The letter must address each violation, explain how it has been corrected and how it will be maintained in the future. If the compliance letter is not received by the date listed a return visit will be made, and the violations cited again. Technical Assistance/Consultation: Weeds and grass growing throughout the surfacing was observed on the playground designated for preschool children. Please ensure that the surfacing in and around the playground is resilient by ensuring that the mulch is fluff and refilled when it begins to move away from fall zones. Staff and training worksheets were submitted to me prior to the annual compliance visit, however, the information on the worksheets were incorrect. The staff and training worksheet is used to document the center's compliance with requirements for preservice staff qualifications, staff records, orientation, and annual training. It is important that this information is completed correctly to ensure that compliance is maintained. Best practice is to update this form quarterly. Please follow the staff and training worksheet instructions when completing this form. Pursuant to GS 110-90(1a) the Division establishes an annual fee for licensed child care facilities. The invoices for the fees are assessed based on your licensed capacity and mailed after the 1st of November every year. You will only receive one invoice, and payment is due in FULL within 30 days. Please closely monitor your mail and be on the lookout for your invoice. If you have not received your invoice by November 15th, you should contact the Division at 1-800-850-0829. Please do not delay making your payment and be aware that Payment Plans are NOT available. In addition, please be aware that failure to pay a license fee within 30 days of receipt will result in late charges. If payments are made late and late charges are applied, the fee as well as the late charge must be resolved. Upon my arrival I was greeted by the director D. Battle. I was accompanied by Ms. Battle as I completed a general walkthrough of the indoor environment which consisted of six (6) classrooms, kitchen and the outdoor environment. The children were observed during center activities, outdoor play and completing routine care tasks. Today’s meal was in compliance with the Meal Patterns. ** License Status The facility is currently operating with a five-star license issued on April 30, 2022. ** Annual Inspections: The last sanitation inspection was completed March 7, 2024, and received a ‘Superior’ classification with 4 demerits. During today’s visit, a partial assessment of Child Care Requirements was conducted. The monitoring included: supervision of children, staff/child ratio, storage of hazardous substances, storage of medicine, adequate/ approved space, posted license, license restrictions, attendance logs, and program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. The last documented safe sleep check was completed on March 28,2024. .0606(g) To comply with the NC Laws and Rules, any violations cited today must be corrected immediately. A compliance letter must be sent to me by May 17, 2024. The letter must address each violation, explain how it has been corrected and how it will be maintained in the future. If the compliance letter is not received by the date listed a return visit will be made and the violations cited again. Technical Assistance/Consultation: During today’s visit, I observed in the classroom designated for infants that the safe sleep log was not completed since March 28, 2024. As a result, a violation was cited. Please review childcare rule 10A NCAC 09 .0606 regarding safe sleep practices. It is important to follow safe sleep practices to reduce the risk of sudden infant death syndrome (SIDS) and other causes of sleep related infant death.
