Facility ID: 96000194 Consultant: TAMEKA JOHNSON Operation Type: Center Case Number: 1224-162L Visit Date: 12/16/2024 Number Present: 87 Completed Date: 12/16/2024 Age: From 5 To 12 Total Minutes: 75 Time In: 03:30 PM Time Out: 04:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to investigate a complaint received by the Division of Child Development and Early Education (DCDEE) on December 12, 2024. Upon my arrival, I was greeted by the director, William Batts. Limited monitoring of the child care requirements occurred during today’s visit. The license was posted. I was unaccompanied as I completed a general walk-through of the indoor and outdoor environment. I monitored staff/child ratios, adequate/approved space, supervision, and interaction with children. A total of eighty seven school age children (87) were present. Allegation: There is a concern that children are not adequately supervised. During today’s visit, I discussed the allegations with Mr. Batts. I also interviewed five (5) staff members. Each was given the opportunity to share their perspectives surrounding the allegation. Mr. Batts stated that there was a family that was causing challenges at the facility that he had to let go. However, it was not related to supervision. During today’s visit, I observed multiple children free flowing through out the indoor and outdoor space without a staff member accompanying the children. Based on my observations, the allegation regarding children not being adequately supervised is SUBSTANTIATED. The following violations were cited during today's visit. Violation Number Comment Rule 1424 School-aged children were not adequately supervised. Multiple children are free flowing throughout the indoor and outdoor space without being accompanied by a caregiver. .2506(d)(1-3) 1756 Enhanced staff/child ratios and group sizes were not met. Two groups of school age children age 5 and up were combined totaling twenty eight(28)children in one space. Exceeding the maximum group size permitted of twenty five (25). 10A NCAC 09 .2818 Failure to comply with the Child Care Requirements puts children’s health and safety at risk. As a result of the supervision violation, a return visit will be made within two (2) weeks to ensure that compliance has been maintained. In addition to a return visit, in order to comply with the NC Laws and Rules, a compliance letter must be submitted to me by December 30, 2024. The compliance letter must address each violation, explain how it has been corrected, and how it will be maintained in the future. If the compliance letter is not received by the date listed a return visit will be made to the child care center. The maximum group size permitted for school age children is 25. Please refer to your classroom staff to child ratio form for each classroom. Caregivers should know how many children they always have. Please review Child Care rule 10A NCAC 09 .2506 regarding general safety requirements. Adequate supervision is vital to the operation of a child care program. Active supervision always requires focused attention and intentional observation of children. Caregivers position themselves so that they can observe all the children: watching, counting, and always listening. They also use their knowledge of each child’s development to anticipate what they will do, then get involved and redirect when necessary. Children should never be left unattended. Caregivers should continuously scan the entire environment to know where everyone is and what they are doing. Specific sounds or the absence of sound may signify a reason for concern. By listening closely to children, you can immediately identify signs of potential danger. Assist in creating positive play experiences by being proactive, watching for environment or equipment risks, and responding to unsafe play behaviors and unsafe equipment use. Facility ID: 96000194 Consultant: TAMEKA JOHNSON Operation Type: Center Case Number: Visit Date: 12/12/2023 Number Present: 16 Completed Date: 12/12/2023 Age: From 4 To 5 Total Minutes: 145 Time In: 09:40 AM Time Out: 12:05 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor applicable child care requirements during your annual compliance visit. Upon my arrival I was greeted by lead teacher Penny Wiggins. A checklist was used to note the requirements monitored today. William Batts arrived a short time later. I was unaccompanied as I completed a general walk through of the four (4) buildings and the outdoor environment. The four buildings are under one license, one (1) is for NC Pre K, three (3) other buildings are for school age children. The children were observed in group time, teacher directed activities, engaging in free choice activities, and completing routine care tasks. There were sixteen (16) NC PreK children present during today’s visit. Today’s meal was in compliance with the Meal Patterns. ** License Status The facility is currently operating with a four-star license issued on April 16, 2019. ** Annual Inspections: The last sanitation inspection was completed December 12, 2022, and received a ‘Superior’ classification with 0 demerits. The last fire inspection was completed on November 10, 2023. The last documented fire drill was conducted on December 30, 2022, the last documented lockdown drill was completed on November 9, 2023, and the last documented playground inspection was completed on November 4, 2023. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed current/active. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, transportation, and attendance logs. I reviewed a sample of children’s records, staff files, all applicable program records, and NC Pre K requirements. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. A container of Vaseline was stored in the middle console of the bus that transport school age children. .2820(b) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. One staff did not complete annual staff development plan and evaluation 10A NCAC 09 .0514(f) To comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by December 26, 2023. The letter must address each violation explain how it has been corrected and how it will be maintained in the future, If the letter is not received by the date listed a return visit will be made to the child care center. During today's visit, I observed on the bus designated to transport school age children, a container of Vaseline in the middle console. The warning label on Vaseline states to keep out of reach of children. Please ensure that all over the counter medicines are stored in a locked cabinet and are inaccessible to children. During today’s visit, staff and training worksheets were not completed. Completing and reviewing staff and training worksheets will help you identify due dates and prevent violations. Keeping it updated will let you see at a glance when items are due. Best practice is to update the staff and training worksheet quarterly or when staff changes or change positions. Although a violation was not cited, we discussed the upholstery on bus license plate CKT 5420. I observed on the back of 3 seats exposed foam and torn fabric. Mr. Batts stated that he has a work order to fix the upholstery on the seats. He stated that children are not sitting in these seats on the bus until they are fixed.
