The license was posted. Upon my arrival I was greeted by assistant director Wendy Smith. Ms. Smith stated that Mr. Batts, owner/operator, will be at the facility shortly. I was accompanied by Ms. Smith as I completed a general walk-through of the indoor and outdoor environment. I monitored staff/child ratios, supervision, schedules, activity plans, incident reports, and safe environment. A total of seventy-three (73) children were present. Allegations: *There are concerns that children are not adequately supervised. *There are concerns that staff/child ratios are not being met. *There are concerns regarding the facility not being a smoke-free environment. * There are concerns regarding incident reports not being completed. * There are concerns regarding a safe environment. During today’s visit, I discussed the allegations with Ms. Smith and Mr. Batts. Ms. Smith stated that they have not had any complaints or concerns from staff or parents. During today’s visit, in the classroom designated for four- and five-year-old children, I observed two (2) children in the bathroom. The teacher was standing in the doorway of the classroom and the playground. There were two additional teachers outside with 16 four- and five-year-old children outside. Ms. Smith told the teacher to come in to supervise the two (2) children in the bathroom. On one of the playgrounds designated for four- and five-year-old children I see a child crying because of an injury. The teacher takes the child inside to offer first aid and to ensure the child was okay. I later asked if an incident report was written. The teacher had not filled out an incident report. I later walk past the classroom designated for three-year-old children. I overheard a teacher explaining a bite to a guardian that was picking up the child. Once the teacher had completed the conversation and the family had gone home, I asked the teacher for a copy of the incident report. The teacher stated that the grandparent does not usually want the incident report and that an incident report will be written up and given to the mother in the morning. Based on my observations and statements made by the staff, the allegation regarding concerns that children are not adequately supervised is CONFIRMED. The allegation regarding incident reports not being completed is CONFIRMED. The allegation regarding the facility not being a smoke-free environment is UNCONFIRMED. The allegation regarding staff/child ratios is not being met is UNCONFIRMED. The allegation regarding a safe environment is UNCONFIRMED. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 303 Children were not adequately supervised at all times. Two (2) children were observed in the bathroom unsupervised. .1801(a)(1-5) 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. Incident reports were not completed each time a child was injured. .0802 (e) Failure to comply with the Child Care Requirements puts children’s health and safety at risk. As a result of the supervision violation, a return visit will be made within two (2) weeks to ensure that compliance has been maintained. In addition to a return visit, a compliance letter must be submitted to me by June 17, 2026. The compliance letter must address each violation, explain how it has been corrected, and how it will be maintained in the future. If the compliance letter is not received by the date listed a return visit will be made to the child care center. The supervision rule states staff must be positioned in an indoor and outdoor environment to maximize their ability to hear or see the children at all times and render assistance. Please review child care rule 10A NCAC 09 .1801(a) (1-5) regarding adequate supervision. Having more than one classroom outside at a time can cause a challenge when supervising children. A caregiver left alone to care for children should be up and actively supervising. Active supervision always requires focused attention and intentional observation of children. Caregivers position themselves so that they can observe all the children: watching, counting, and always listening. They also use their knowledge of each child’s development to anticipate what they will do, then get involved and redirect when necessary. Children should never be left unattended. Caregivers should continuously scan the entire environment to know where everyone is and what they are doing. Specific sounds or the absence of sound may signify a reason for concern. By listening closely to children, you can immediately identify signs of potential danger. Completing incident reports in child care centers is essential to ensure child safety, maintain transparent family communication, and fulfill legal licensing requirements. Detail documentation protects children, staff and the facility by establishing accurate record of events. Tracking incidents helps administrators identify behavioral patterns, spot environmental hazards, and adjust supervision to prevent future accidents. Upon my arrival I was greeted by floater Sonya Olmstead. Ms. Olmstead stated that director, Mr. William Batts, was not available today. The assistant director, Ray Batts went to get supplies and will be back shortly. I was accompanied by Ms. Olmstead as I completed a general walkthrough of the indoor environment which consisted of seven (7) classrooms, the kitchen, and the outdoor play environment. Four (4) of the classrooms are designated for NC Pre-Kindergarten. The children were observed eating breakfast, during free choice activities, teacher directed activities and completing routine care tasks. Today’s meal was in compliance with the Meal Patterns. ** License Status The facility is currently operating with a four-star license issued on August 1, 2019. ** Annual Inspections: The last sanitation inspection was completed December 17, 2025and received a ‘Superior’ classification with 4 demerits. The last fire inspection was completed on November 8, 2025. The last documented fire drill was conducted on January 14, 2026, the last documented lockdown drill was completed on December 2, 2025, and the last documented playground inspection was completed on July 11, 2023. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed current/active. During today’s visit, a partial assessment of Child Care Requirements was conducted. The monitoring included: supervision of children, staff/child ratio, storage of hazardous substances, storage of medicine, adequate/ approved space, posted license, license restrictions, attendance logs, and program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. During today’s visit we discussed that all lesson plans need to be posted in each classroom. If there is a challenge with printing lesson plans, ensure that all the teacher know and have access to printing. During today’s visit we discussed room arrangement. As a reminder, please ensure that teachers are able to see and hear children with a clear 360 view for supervision. Ensure there are no blind spots in the classroom allowing teachers to monitor all children from any position. We discussed prepping food before it is served to the children. As a reminder, knives used for cutting fruit and vegetables should be used in the kitchen to prevent children from getting hurt. There are children approved knives that can be used if you all are interested in allowing children to prep food for an activity. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. The children were observed eating breakfast, doing free choice activities, teacher directed activities, outdoor play, and completing routine care tasks. Today’s meal was observed and in compliance with the Meal Patterns. **Current License Status Currently this center operates with a four-star rated license issued August 1, 2019. ** Annual Inspections: The last sanitation inspection was completed December 12, 2023, and received a ‘Superior’ classification with 6 demerits. The last fire inspection was completed on November 10, 2023. The last documented fire drill was conducted on August 20, 2024, the last documented lockdown drill was completed on June 14, 2024, and the last documented playground inspection was completed on September 13, 2024. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed as current/active. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, attendance, and safe sleep logs. I reviewed a sample of children’s records, staff files and all applicable program records and NC Pre-K requirements. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Please review child care rule 10A NCAC 09 .0803 regarding administering medication in a child care center. It is important to ensure the right medication and dosage is given to the correct child to maintain safety. We discussed options to choose from to ensure the safety of children with allergies. Please remember to update allergy lists quarterly or as new children are enrolled, whichever comes first. I recommended that the lists include pictures of children so that those children with allergies are easily identifiable to floaters and/or substitutes. During today’s visit I observed children in the classroom designated for three year old children eating lollipops. The teacher stated that because it was wet outside, she gave them a lollipop for a special treat. As a reminder, food should not be given to children as a reward. We discussed alternative to gross motor play when the playground is wet. When children are in care and weather conditions permit, there shall be outdoor time. Today’s meal was in compliance with the Meal Patterns. ** License Status The facility is currently operating with a four star rated license issued on August 1, 2019. ** Annual Inspections: The last sanitation inspection was completed December 12, 2022, and received a ‘Superior’ classification with 4 demerits. The last fire inspection was completed on November 10, 2022. The last documented fire drill was conducted on June 30, 2023, the last documented lockdown drill was completed on April 11 , 2023, and the last documented playground inspection was completed on July 11, 2023. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed current/active. During today’s visit, a partial assessment of Child Care Requirements was conducted. The monitoring included: supervision of children, staff/child ratio, storage of hazardous substances, storage of medicine, adequate/ approved space, posted license, license restrictions, attendance logs, and program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. Menus in two classrooms were not updated. 10A NCAC 09 .0901(b) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Outlet covers were missing in two classrooms. 10A NCAC 09 .0604(c) The violations cited was corrected during the visit; therefore, no compliance letter is needed. Upon my arrival I was greeted by the lead teacher W. Wiggins. Ms. Wiggins stated that Mr. Batts was on a field trip with school age children at the second site. Mr. Batts arrived a short time later. I was unaccompanied as I completed a general walkthrough of the indoor environment which consisted of seven (7) classrooms, kitchen and the outdoor environment.
