Upon my arrival, I was greeted by the assistant director Ms. Shuquale Royal. Ms. Ruby McKinzie, director, arrived short time later. I was unaccompanied as I completed a general walk-through of the indoor and outdoor areas, which consisted of four (4) classrooms, the kitchen, and the outdoor play environment. The children were observed during outdoor play and completing routine care tasks. Today’s meal was observed and in compliance with the Meal Patterns. **Current License Status Currently this center operates with a three-star rated license issued May 25, 2022. ** Annual Inspections: The last sanitation inspection was completed July 14, 2025, and received a ‘Superior’ classification with 6 demerits. The last fire inspection was completed on July 17, 2025. The last documented fire drill was conducted on November 4, 2025, the last documented lockdown drill was completed on October 27, 2025, and the last documented playground inspection was completed on November 5, 2025. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed as current/active During today’s visit, a partial assessment of Child Care Requirements was conducted. The monitoring included: supervision of children, staff/child ratio, storage of hazardous substances, storage of medicine, adequate/ approved space, posted license, license restrictions, attendance logs and program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. Two cans of aerosol sunscreen was stored in an unlocked cabinet. .2820(b) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. An inhaler was not stored in it's original labeled container. .0803(2)(a) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A tube of diaper cream had an expiration date of 2/2024. .0803(12) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Safe sleep checks were not documented in compliance with visually checking on sleeping infants. .0606(g) To comply with the NC Laws and Rules, any violations cited today must be corrected immediately. A compliance letter must be sent to me by December 17, 2025. The letter must address each violation, explain how it has been corrected and how it will be maintained in the future. If the compliance letter is not received by the date listed a return visit will be made and the violations cited again. Technical assistance/ Consultation: During today’s visit violations were cited regarding administering medications. Giving medicines is a serious responsibility. It requires caregivers to have knowledge and skills. All caregivers and families must understand what is required of them to ensure that children are safe. Medicines can be serious to the health and wellness of children. They can also be dangerous. Having a policy on the safe administration of medication protects your program. Please review childcare rule regarding administration of medication section 10A NCAC 09 .0803. During today’s visit, I observed that sleep checks were not completed documented since November 17, 2025, for infant children. Please retrain the infant staff on the safe sleep policy and the importance of following it. We discussed hanging a clipboard on each end of the crib so that the it will be in view. The staff/child ratio for one year olds is one caregiver to six children. Ms. Royal stated that she was in the classroom with the other caregiver. She left the classroom to open the door for me. She planned to move the one year back to the other classroom but did not think about it at the time. As a reminder, classrooms should be in ratio at all times. Failure to maintain ratios puts the safety of children at risk. Due to this violation a return visit will occur in the near future. In addition, to comply with the NC Laws and Rules, any violations cited today must be corrected immediately. A compliance letter must be sent to me by January 22, 2024. The letter must address each violation, explain how it has been corrected and how it will be maintained in the future. A staff member did not complete an annual health questionnaire. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. One staff member did not complete emergency information at least annually. .0701(a) All violations cited during today’s visit was corrected; therefore, a compliance letter is not required. Prior to today’s visit, the 18-month compliance history for this facility was 80%. Technical Assistance/ Consultation: During today’s visit I observed, diaper cream ointment stored in cubbies on a shelf in the classroom designated for infants. The vertical distance of the cubbies were not five feet above the finished floor. As a reminder, a product shall be considered inaccessible to children when stored on a shelf or in an unlocked cabinet that is mounted a minimum vertical distance of five feet above the finished floor.
