    Name of Operation: Ms. Dana's Sunshine Academy Facility ID: 96000648 Consultant: RAMIE DOLL Operation Type: Family CC Home Case Number: Visit Date: 2/3/2025 Number Present: 4 Completed Date: 2/3/2025 Age: From 0 To 3 Total Minutes: 175 Time In: 09:45 AM Time Out: 12:40 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's visit was to monitor your program for compliance with applicable child care requirements during the first temporary time period visit. You, Dana Hosier, operator, were present and assisted me today. A temporary license was issued for this program on December 3, 2024, and is valid through June 3, 2025. The temporary license allows a first shift capacity of 6 children, ages 0 to 12 years old. Restrictions on the license include daytime and overnight care, children in care on ground level only, a maximum of 5 preschoolers at any time, and the fireplace/woodstove is not used during operating hours. You are also an affiliated provider with the local air force base. During this visit, we walked through the indoor and outdoor environments and completed a full assessment of the licensed space. A variety of age-appropriate learning materials were observed in the child care area. A total of four (4) children were present today, that included your own preschool aged child. The children ranged in ages from infancy to three years. The children played with toys and interacted with you today. You served them lunch during the visit which consisted of breaded chicken nuggets, blueberries, cucumbers, peaches, and milk. Program records were also monitored today along with children’s records. Both fire drills and outdoor playground inspections must be completed monthly and maintained on file for one year after completion and kept on file for review. None of your information has changed since the Initial Assessment visit other than you completed the Recognizing and Responding to Suspicions of Child Maltreatment training. The following violation was documented today: Violation Number Comment Rule 919 Accurate records were not maintained for all staff and children. A record was not available for review for the Operator's preschool aged children. The following were not on file for three children today: receipt of the Shaken Baby Syndrome and Abusive Head Trauma Policy, parent acknowledgement of the summary of NC Child Care Law, and a signed and completed Written Plan of Care. G.S. 110-91(9) All violations must be corrected immediately. Please send me a compliance letter stating specifically how the violation was corrected. The compliance letter must be received within the next two weeks and no later than February 17, 2025. You can email the compliance letter from the email address on file with the Division as the center’s official email address. Contact me if you have questions regarding correction of the violation documented during today’s visit. If you are unable to correct the violation within the specified timeframe, send a compliance letter regarding the violation that was corrected and a specific plan to correct the violation. If an extension is needed to correct any violations, prior approval must be given. You will need to email a request for an extension, if needed. A sample compliance letter was given to you today along with the visit summary to see how to create/develop a compliance letter. You can list the item number, specifically stating that the violation has been corrected, explaining when and how it was corrected. Compliance is not considered finalized until all violations have been corrected; therefore, if the compliance letter says an item will be corrected, it should be followed-up with another letter once it actually has been corrected. Please let me know if you have any questions regarding how to complete a compliance letter. TECHNICAL ASSISTANCE & CONSULTATION: CHILDREN’S RECORDS: Children's records consist of various documentation such as a child's medical and immunization history, emergency medical care information, written plan of care, and parental acknowledgements. This information is the basis for meeting each child's physical, emotional, cognitive and social needs. When enrolling children, all forms are needed on the first day with the exception of the medical and immunization reports, which must be received within 30 days of the enrollment date. Best practice is to obtain both the medical report and immunization record prior to enrollment. We discussed having two files containing all required information for the Air Force and one for required licensing documents/forms. This may help you to separate what is required for both agencies. Some of the information may be repeated but having two separate folders may better assist you to ensure all required information is received and on file. A medical report can be accepted if completed within the last 12 months prior to a child’s enrollment provided the information listed on DCDEE’s medical form is answered. Ensure the parent completes the top section of the Children’s Medical Report form. A Children’s Records Checklist is available to assist with compliance. The form can be printed from the Provider Documents and Forms tab of the Division’s website. We reviewed the Children’s Records checklist during a previous visit. Your daughter, who is preschool aged, is counted in your capacity and all required information must be completed and on file for her as well. MEDICATION INFORMATION: Ensure parents specify a beginning and an ending date to administer any type of medication such as diaper cream, sunscreen, lip balm, lotions, etc. You have developed a permission form for parents to complete to administer these types of medication on one form. A beginning date was specified by no ending date was specified. Today was the first day of enrollment for the children currently enrolled. Ensure parents specify an ending date when they return today. Permission to administer these types of medication can be given for up to twelve (12) months. We referenced a medication permission form from the Division of Child Development and Early Education’s website under the Provider Documents and Forms today. MONTHLY INSPECTIONS: Outdoor inspections must be completed and documented monthly on the Outdoor Playground Inspection Checklist for Family Child Care Homes form along with monthly fire drills. Completing a fire drill and monthly playground inspections on the same day is a great way to ensure compliance with this requirement. You can also set reminders with your phone or document when you plan to conduct a drill on a calendar. Ensure drills are conducted at different times each month. HEALTH AND SAFETY TRAINING: Health & Safety (Moodle): The Child Care Development Fund Health and Safety in Child Care Trainings, developed by the North Carolina Child Care Health and Safety Resource Center, are FREE and are located on the MOODLE training platform. You will need to have an NCID - the same NCID that is used to log in to WORKS and/or the CBC Portal. These modules can be found by using the "all courses" function or searching for CCDF - Health and Safety in Child Care in the search box at the bottom of the main Moodle page. This series of trainings include 10 topic areas. CPR, First Aid, and Recognizing and Responding to Suspicions of Child Maltreatment must be completed within 90 days of licensure. The remaining topic areas must be completed within your first year of licensure, and every five years thereafter. I encourage you to use the Health and Safety training log to ensure all topic areas are covered. Once you complete the health and safety trainings, print your certificate and maintain on file for review. You completed the Recognizing and Responding to Suspicions of Child Maltreatment December 10, 2024. You completed CPR and First Aid training November 2, 2024. The remaining Health and Safety trainings will need to be completed by December 3, 2025. Health and Safety trainings are required to be completed within the first year of employment and every five years thereafter. EMERGENCY PREPAREDNESS AND RESPONSE TRAINING: (EPR) Training: When asked, you have not yet completed the EPR training. The EPR training is required to be completed within the first year of licensure, on or before December 3, 2025. Once completed, you will need to create an EPR plan in the Emergency Management portal within four months after completion of the training. Once completed, you should print it and have it available for review at your facility. You will need to continue to complete and document shelter in place or lockdown emergency drills every three months. ON-GOING TRAINING: The ongoing training hours (in-service) will be monitored at your first annual compliance visit based on when you were initially licensed. No training hours were monitored today. The requirements are based on education and experience. You are required to complete twenty (20) hours of training annually. You may count the health and safety trainings toward this requirement. Maintain copies of your training certificates on file for verification. You can also use the On-Going Training Record Log to document the trainings you complete and attach your certificates to the log. You can attend any trainings offered by any Child Care Resource and Referral Agency in North Carolina. You can complete any trainings offered from the Southwestern Child Development Commission as well. Rated License: We discussed that you may voluntarily request to apply for a Two through Five Star Rated License at the end of the Temporary Time Period. A Three to Five Star license is required to participate in the Subsidized Child Care Program. We discussed that the Star Rated License is composed of the Program Standards Component, the Staff Education Component, and a quality point. An accumulation of points will determine how many points will be earned for the Star Rated License. I will bring you a copy of the Family Child Care Home Initial Rated License application during the next temporary time period visit. Staff Education: Points in this component will be determined by the Early Childhood courses you have completed at the college level and child care experience. WORKS is a self-service applicant portal that will be used by an individual applicant to register and maintain their information to support education requirements. The WORKS portal is available on the DCDEE website and requires that the user maintain an individual North Carolina Identity Management (NCID) username and password. At this time, you have not created a WORKS account, therefore no education information has been submitted. When asked, you have not completed the North Carolina Early Childhood Credential or any other coursework via the Community College or University. We reviewed points you may be eligible for during today’s visit without the completion of the North Carolina Early Childhood Credential. Program Standards: Once the facility is in compliance with applicable child care requirements, no additional action is needed to meet one point in this component. In order to earn two or more points, you will need to complete the 3-month long self-study and self-assessment using the FCCERS-R rating scale tool. In order to earn three or more points, you must have the FCCERS completed by the North Carolina Rated License Assessment Project and score a minimum of 4 points. I referred you to the NCRLAP website, (www.ncrlap.org), for technical assistance and information regarding the 3-month self-study and self-assessment. Maintain documentation of completion of the three-month self-study on file for review and include dates when information is documented if you choose to complete the self-study. I provided you with a copy of the information regarding the three-month self-study during today’s visit. You can contact Wayne County’s Partnership for Children to request any on-site technical assistance and assistance with a self-study. We discussed the FCCERS is voluntary and if not completed during your initial Rated License assessment you can request it later. Quality Point: The various options for earning a quality point will be discussed during the next visit. COMPLIANCE HISTORY: It is very important that you maintain compliance with all applicable requirements at all times since you will be building a compliance history. In order to move to a star rated license at the end of the Temporary Time Period, you must have at least a 75% compliance history. Programs whose compliance history is below 75% could be issued an administrative action including denial of a license at the end of the Temporary License period. The violations documented today may affect your compliance history score. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. I encourage you to visit the DCDEE website and click on the “What’s New” tab to stay abreast of all updates. CHILD CARE RULES: You can access the NC Child Care rules from the Division of Child Development and Early Education’s website: https://ncchildcare.ncdhhs.gov, click on the “provider” tab, then “child care rules” tab. Download a copy of the NC Child Care rules onto your computer so you have access to the rules and can reference them as needed.
