Upon my arrival, I was greeted by the assistant director Chasity Silverthorne. A checklist was used to note the requirements I monitored today. I was accompanied by lead teacher Marylee Bass as I completed general walk-through of the indoor and outdoor areas, which consisted of eight (8) classrooms, the kitchen, and the outdoor play environment. The children were observed during outdoor play, a teacher directed activity, free choice activities and completing routine care tasks. Today’s meal was observed and in compliance with the Meal Patterns. **Current License Status Currently this center operates with a four-star rated license issued November 11, 2021. ** Annual Inspections: The last sanitation inspection was completed September 18, 2025, and received a ‘Superior’ classification with 7 demerits. The last fire inspection was completed on December 15, 2025. The last documented fire drill was conducted on February 27, 2026, the last documented lockdown drill was completed on February 27, 2026, and the last documented playground inspection was completed on February 27, 2026. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed current/active. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, attendance logs, and sleep checks. I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. A crack was observed along the border of the playground play structure designated for preschool children. 10A NCAC 09 .0601(a) 893 A copy of the safe sleep policy was not given to and/or explained to the parent of each child on or before the first day the infant attended the center. A copy of the safe sleep policy was not given to one child on the first day. 10A NCAC 09 .0606(c) The violations cited during today’s visit were corrected therefore a compliance letter is not needed. Technical assistance/ Consultation: During today’s visit I observed a crack along the border of the playground play structure designated for preschool children. This could present a potential safety hazard for children during play, as a child could trip, catch clothing, or be injured. The area was repaired during the visit. As a reminder, check to ensure potential items such as cracks and holes are caught during playground inspections. During today’s visit, staff and training worksheet were not complete. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. I was accompanied by Child Care Consultant, Tanya Herring. Upon our arrival, we were greeted by lead teacher, B. Bass. Ms. Bass stated that the assist director, C. Silverthorne was in the kitchen preparing lunch. Ms. Silverthorne stated that the owner/operator, T. Smith was on the way. Ms. Smith arrived a short time later. Limited monitoring of the child care requirements occurred during today’s visit. The license was posted. We were unaccompanied as we completed a general walk-through of the indoor and outdoor areas. A total of sixty seven (67) children were present. The children were observed during outdoor play and completing routine care task. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. Eleven (11) one year old children were on the playground with one teacher. Leaving the classroom out of ratio. GS 110-91(7);.0713(a-d) Failure to comply with the Child Care Requirements puts children’s health and safety at risk. As a result of staff/child ratio violation, a return visit will be made within two (2) weeks to ensure that compliance has been maintained. I asked the caregiver where the other teacher was. She stated she is in the classroom. The minimum staff/ child ratio for one year old children is one (1) teacher to six (6) children. The classroom was out of ratio. The teacher in the classroom stated that she had five (5) children with her and completed changing diapers. She had to go back into the classroom to grab something. As a reminder, staff/child ratio have to be maintained at all times. Staying in ratio allows caregivers to focus on the needs of the children. The rest of the children were outside with the other teacher. Although the teacher could hear and see the child, she could not render assistance. I went to the office to get assistance. Please review child care rule 10A NCAC 09 .1801(a) (1-5) regarding supervision. I observed an infant sleeping in a bouncer during today’s visit. The caregiver stated that she was waiting for the crib sheets to come out of the dryer. Infants aged 12 months or younger are prohibited from sleeping in sitting devices, including car safety seats, strollers, swings, and infant carriers. Infants that fall asleep in sitting devices shall be moved to a crib, bassinet, play pen, mat, or cot. A recommendation is to purchase enough sheets so that if a set is in the wash another set can be used on the cribs. As a reminder, please review child care rule 10A NCAC 09 .0606 regarding safe sleep practices. Staff and training worksheets need to be completed annually. Completing and reviewing this document will help you identify due dates and prevent violations. Keeping it updated will let you see at a glance when items are due. Today’s meal was in compliance with the Meal Patterns. ** License Status The facility is currently operating with a four-star license issued on November 11, 2021. ** Annual Inspections: The last sanitation inspection was completed October 2, 2023, and received a ‘Superior’ classification with 15 demerits. The last fire inspection was completed on September 28, 2022. The last documented fire drill was conducted on October 3, 2023, the last documented lockdown drill was completed on October 13, 2023, and the last documented playground inspection was completed on October 13, 2023. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed current/active. During today’s visit, a partial assessment of Child Care Requirements was conducted. The monitoring included: supervision of children, staff/child ratio, storage of hazardous substances, storage of medicine, adequate/ approved space, posted license, license restrictions, attendance logs, and program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division.
