Upon my arrival I was greeted by the cook, A. Forte. Ms. Forte stated that the director was out, and she would be back shortly. A checklist was used to note the requirements monitored today. I was unaccompanied as I completed a general walkthrough of the indoor environment which consisted of eight (8) classrooms, kitchen and the outdoor environment. The children were observed during outdoor play, teacher directed activities and completing routine care task. Today’s meal was in compliance with the Meal Patterns. ** License Status The facility is currently operating with a five-star license issued on July 15, 2024. ** Annual Inspections: The last sanitation inspection was completed September 3, 2025, and received a ‘Superior’ classification with 4 demerits. The last fire inspection was completed on April 15, 2026. The last documented fire drill was conducted on May 5, 2026, the last documented lockdown drill was completed on March 16, 2026, and the last documented playground inspection was completed on June 1, 2026. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, attendance, and safe sleep checks. I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. During today's visit the classroom for infants did not have a current lesson plan posted. GS 110-91(12); .0508(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff did not complete first aid training. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff did not complete CPR. .1102(d) To comply with the NC Laws and Rules, any violations cited today must be corrected immediately. A compliance letter must be sent to me by June 17, 2026. The letter must address each violation, explain how it has been corrected and how it will be maintained in the future. If the compliance letter is not received by the date listed a return visit will be made and the violations cited again. Upon my arrival I was greeted by the assistant director T. Cotton and Geraldine Spruill. A checklist was used to note the requirements monitored today. I was unaccompanied as I completed a general walkthrough of the indoor environment which consisted of eight (8) classrooms, kitchen and the outdoor environment. The children were observed during lunch, preparing for naptime and completing routine care task. Today’s meal was in compliance with the Meal Patterns. ** License Status The facility is currently operating with a five-star license issued on July 15, 2024. ** Annual Inspections: The last sanitation inspection was completed August 13, 2024, and received a ‘Superior’ classification with 4 demerits. The last fire inspection was completed on April 10, 2025. The last documented fire drill was conducted on June 3, 2025, the last documented lockdown drill was completed on March 5, 2025, and the last documented playground inspection was completed on June 3, 2025. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, attendance, and safe sleep checks. I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A tube of Aquaphor was expired 4/2025 .0803(12) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Two employee's Lakesha Parker and Aletha Leftdwrige had expired qualification letters. Technical Assistance/ Consultation: The qualification letter for Lakiesha Parker and Aletha Leftdwrige, lead teachers, expired on May 12, 2025, and May 15, 2025. I stated to the Ms. Spruill that Ms. Parker and Ms. Leftdwrige have fifteen (15) days from today’s visit to submit the necessary documents to obtain a new qualification letter. If no letter has been obtained by July 16, 2025, Ms. Parker and Ms. Leftdwrige may not return until such time that a new qualification letter is submitted. As a reminder, criminal background check qualification letters are valid for five years from the date of issuance. Teraesa Leak, Licensing Supervisor accompanied me on today’s visit. Upon our arrival we were greeted by the director G. Spruill and assistant director T. Cotton. We were accompanied by Ms. Spruill as we completed a general walkthrough of the indoor environment which consisted of eight (8) classrooms, and kitchen. The children were observed during eating lunch, preparing for naptime and completing routine care task. Today’s meal was in compliance with the Meal Patterns. ** License Status The facility is currently operating with a four-star license issued on September 10, 2021. ** Annual Inspections: The last sanitation inspection was completed August 29, 2023, and received a ‘Superior’ classification with 6 demerits. The last fire inspection was completed on April 17, 2024. The last documented fire drill was conducted on August 1, 2023, the last documented lockdown drill was completed on May 23, 2023, and the last documented playground inspection was completed on August 1, 2024. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed current/active. During today’s visit, a partial assessment of Child Care Requirements was conducted. The monitoring included: supervision of children, staff/child ratio, storage of hazardous substances, storage of medicine, adequate/ approved space, posted license, license restrictions, attendance logs, program records and a review of new administrator file. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence) Feeding schedule was not provided for one toddler age 12 months. 10A NCAC 09 .0902(a) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Technical Assistance/ Consultation: As a reminder, feeding plans are required for children aged 12 to 15 months. Adding feeding plans to all enrollment forms will be a good reminder for children that enroll in your facility at the age of 12 to 15 months. Although a violation was not cited, we discussed having cleanable covered garbage cans located in each classroom and removing the trash to an exterior garbage area at least daily to reduce the smell during diapering.
