Upon my arrival, I was greeted by K. Gray, administrator. A checklist was used to note the requirements I monitored today. I was unaccompanied as I completed a general walk-through of the indoor and outdoor areas, which consisted of four (4) classrooms, the kitchen and the outdoor play environment. The children were observed during free choice activities and completing routine care tasks. Today’s meal was observed and in compliance with the Meal Patterns. ** License Status: Currently this center operates with a five-star rated licensed issued October 19, 2017. ** Annual Inspections: The last sanitation inspection was completed June 17, 2025, and received a ‘Superior’ classification with 4 demerits. The last fire inspection was completed on November 12, 2024. The last documented fire drill was conducted on December 5, 2025, the last documented lockdown drill was completed on October 31, 2025, and the last documented playground inspection was completed on December 12, 2025. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed as current/active. During today’s visit, a full assessment of Child Care Requirements was conducted. I observed all required postings, safe sleep checks, and attendance logs. I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. EMC plan was not reviewed with staff annually. 10A NCAC 09 .0802(a) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. 10A NCAC 09 .0514(f) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. Staff did not review EPR plan annually .0607(e) The compliance letter must address each violation, explain how it has been corrected and how it will be maintained in the future, If the compliance letter is not received by January 20, 2025 or a return visit will be made to the child care center. Transportation was not monitored during today's visit due to the vehicle being stored at the Joyful Play Two site. During today’s visit, Ms. Gray stated that she only had 6 children enrolled at the facility. The facility will be closing in a few months. Children will be transitioning to the Joyful Play Two site. Technical Assistance and Consultation: During today’s visit, we discussed transportation requirements. I reviewed before children are transported, written permission from a parent shall be obtained that shall include when and where the child is to be transported, expected time of departure and arrival, and the transportation provider. Parents may give standing permission, valid for up to 12 months, for transport of children to and from the center not including off-premise activities. When children participate in off premise activities the following shall apply: (1) Children under the age of three shall not participate in off premise activities that involve children being transported in a motor vehicle. (2) When children are transported in a motor vehicle for off premise activities, the provisions in Rule .1003(c) through (i) and (k) of this Chapter shall apply. (3) Before staff members walk children off premises for play or outings, the center shall obtain written permission from the parent of each child to be included in such activities. (4) Parents may provide a written statement giving standing permission which may be valid for up to 12 months for participation in off premise activities that occur on a regular basis. (5) The center shall post a schedule of off premise activities in each participating classroom where it can be viewed by parents, and a copy shall be given to parents. The schedule shall be current and shall include the: (A) location of the activity; (B) purpose of the activity; (C) time the activity will take place; (D) date of the activity; and (E) name of the person(s) to be contacted in the event of an emergency. (6) Each time that children are taken off the premises, staff shall take a list of the children participating in the activity with them. Staff members shall use this list to check attendance when leaving the center, periodically when the children are involved in the activity, before leaving the activity to return to the child care center, and upon return to the center. A list of all children participating in the off premise activity shall also be available at the center. During today’s visit, we discussed that staff members and child care administrators are counted in meeting the staff/child ratios cannot concurrently perform food preparation or other duties that are not direct child care responsibilities. The children were observed during free choice activities and completing routine care tasks. I observed all required postings, attendance logs, safe sleep logs, transportation, and I reviewed a sample of children’s records, staff files and all applicable program records. Upon my arrival I was greeted by the lead teacher K. Lewis. Ms. Lewis stated that the director was working in the next classroom. I was unaccompanied as I completed a general walkthrough of the indoor environment which consisted of five (5) classrooms, kitchen and the outdoor environment. The children were preparing for lunch and completing routine care tasks. Today’s meal was in compliance with the Meal Patterns. ** License Status The facility is currently operating with a five-star license issued on October 19, 2017. ** Annual Inspections: The last sanitation inspection was completed June 29, 2023, and received a ‘Superior’ classification with 8 demerits. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed current/active. During today’s visit, a partial assessment of Child Care Requirements was conducted. The monitoring included: supervision of children, staff/child ratio, storage of hazardous substances, storage of medicine, adequate/ approved space, posted license, license restrictions, attendance logs, program records and transportation. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. The violations cited were corrected during today’s visit; therefore, no compliance letter is required. Prior to today’s visit the 18-month compliance history was 99%. Technical Assistance/ Consultation: During today’s visit, I observed a bag of open pretzels in a cubby outside of the kitchen. As a reminder, all snacks need to be sealed in an airtight container.
