    Name of Operation: FUNDAMENTALS CHILD DEVELOPMENT CENTER Facility ID: 96000609 Consultant: TAMEKA JOHNSON Operation Type: Center Case Number: 0425-200L Visit Date: 4/16/2025 Number Present: 55 Completed Date: 4/16/2025 Age: From 0 To 5 Total Minutes: 60 Time In: 11:05 AM Time Out: 12:05 PM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to investigate a complaint received by the Division of Child Development and Early Education (DCDEE) on April 16, 2025. Upon my arrival, I was greeted by the assistant director, Hope Wade. Limited monitoring of the child care requirements occurred during today’s visit. The license was posted. I was accompanied by Ms. Wade as I completed a general walk-through of the indoor environment. I monitored staff/child ratio, adequate/approved space, supervision, and access to water. A total of fifty-five (55) children were present. Allegations: *There are concerns that staff/child ratios are not being followed. *There is a concern that children do not freely have access to water throughout the day. During today’s visit, I discussed the allegations with Ms. Wade. She was given the opportunity to share her perspectives surrounding the allegation. Ms. Wade stated that the facility opens at 6am. There is one teacher in the classroom designated for three-year-old around 7:30am. The next teacher usually arrives at 8:00 am. I reviewed video footage for the day in question of the classroom designated for three-year-old at approximately between 7: 30 and 7:45. I observed one teacher in the classroom with twelve children. The ratio for three-year-old children is one (1) teacher to ten (10) children. Based on my observations and statements made by the staff, the allegation regarding staff/child ratios are not being followed is SUBSTANTIATED. Ms. Wade stated that a five-gallon cooler is made each morning with water for the classrooms designated for preschool, pre-kindergarten, and school age children. The classroom designated for one- and two-year-old's typically receives sippy cups of water. Pitchers of water are made to refill the sippy cups. During today’s visit I observed the coolers on the shelves in the designated classrooms. Based on my observations and statements made by the staff, the allegation regarding children not freely having access to water throughout the day is UNSUBSTANTIATED. The following violations were cited during today's visit. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. The classroom designated for three-year-old children was out of ratio. One teacher cared for twelve (12) children. 10A NCAC 09 .2818 Failure to comply with the Child Care Requirements puts children’s health and safety at risk. As a result of the staff/child ratio violation, a return visit will be made within two (2) weeks to ensure that compliance has been maintained. In order to comply with the NC Laws and Rules any violation cited today must be corrected immediately. A compliance letter must be sent to me by April 30, 2025. The letter must address each violation, explain how it has been corrected and how it will be maintained in the future. Required staff/child ratios differ by age. For example, a staff/child ratio of 1:10 for 3-year-old children means that one staff member must be assigned to supervise no more than 10 3-year-old children. The required ratio helps staff provide better supervision and care and provides children with more opportunities to develop social skills by allowing them to consistently interact with a smaller group of children and staff. Please review with each staff the staff/child ratio for each classroom they are assigned to. Prepare for more children by communicating with one another before the classroom meets ratio. Upon my arrival, I was greeted by assistant director Hope Wade. Limited monitoring of the child care requirements occurred during today’s visit. The license was posted. I was accompanied by Ms. Wade as I completed a general walk-through of the indoor and outdoor areas. A total of sixty-three (63) children were present. The children were observed during outdoor play, free choice activities and completing routine care tasks. Supervision was maintained in all classrooms during today’s visit and therefore in compliance. A compliance letter was received on March 17, 2025. The following violations were cited during today's visit: Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. The classroom designated for four and five year old had a combine total of 14 children to 1 caregiver. 10A NCAC 09 .2818 A compliance letter is due within two (2) weeks. By April 9, 2025, please submit a letter stating how the violation has been corrected and how compliance will be maintained in the future. Be sure to include the item number and the facility ID number. Due to the nature of the violation, a return visit will occur to ensure that compliance has been maintained. During today’s visit, I observed in the classroom designated for four- and five-year-old children 1 caregiver with 7 four-year-old and 7 five-year-old children. The ratio for four-year-old children is 1 caregiver to 13 children. As a reminder, your facility meets enhanced ratios. Review staff/child ratios with all staff. Ensure that there is a procedure in place for staff to communicate how many children they have. If a classroom struggles with having more children than caregivers reconfigure enrollment in each classroom. Please continue to visit the DCDEE website for the latest information and updates. Upon my arrival I was greeted by Leonard Stevens, owner. Hope Wade assistant director, and Candace Stevens owner/director, arrived a short time later. A checklist was used to note the requirements we monitored today. I was accompanied by Ms. Wade and Ms. Stevens as I completed a general walkthrough of the indoor environment which consisted of six (6) classrooms, kitchen and the outdoor environment. The children were observed during outdoor play, free choice activities, and routine care tasks. Today’s meal was in compliance with the Meal Patterns. ** License Status The facility is currently operating with a five-star license issued on September 19, 2022. ** Annual Inspections: The last sanitation inspection was completed September 3, 2024, and received a ‘Superior’ classification with 2 demerits. The last fire inspection was completed on July 17, 2024. The last documented fire drill was conducted on February 7, 2025, the last documented lockdown drill was completed on January 29, 2025, and the last documented playground inspection was completed on February 21, 2025. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed current/active. During today’s visit, a full assessment of Child Care Requirements was conducted. We observed all required postings, attendance, and safe sleep checks. I reviewed a sample of children’s records, staff files and all applicable program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 303 Children were not adequately supervised at all times. A caregiver was standing between the back door of classroom and outside allowing one child to wash hands and the other child to use the bathroom without being able to render assistance. .1801(a)(1-5) Failure to comply with the Child Care Requirements puts children’s health and safety at risk. As a result of the supervision violation, a return visit will be made within two (2) weeks to ensure that compliance has been maintained. In addition to a return visit to comply with the NC Laws and Rules, any violations cited today must be corrected immediately. A compliance letter must be sent to me by March 26, 2025. The letter must address each violation, explain how it has been corrected and how it will be maintained in the future. If the compliance letter is not received by the date listed a return visit will be made and the violations cited again. Although the teacher could hear and see the children, she could not render assistance. Please review child care rule 10A NCAC 09 .1801(a) (1-5) regarding supervision. During today’s visit, Ms. Stevens stated that mulch will be delivered to the facility on Monday, March 17, 2025, for the play structures on the preschool playground and the school age playground. Children will not play on the play structures until the mulch is distributed to both areas. Ms. Stevens will send a photo of the mulch around the play structure along with the invoice. Pursuant to GS 110-90(1a) the Division establishes an annual fee for licensed child care facilities. The invoices for the fees are assessed based on your licensed capacity and mailed after the 1st of November every year. You will only receive one invoice, and payment is due in FULL within 30 days. Technical Assistance/ Consultation: During today’s visit, I observed that the facility’s sanitation inspection expired July 5, 2023. Please contact the Environmental Health inspector immediately. I reviewed and explained that each operator must schedule a Sanitation and Fire inspection within 12 months of the center's previous inspections. You must submit the original of the completed Sanitation and Fire inspection report to me within one week of the inspection, on the state form provided by the Division. During today’s visit, I observed speech therapist, April Holland alone in a classroom with three (3) children. Ms. Holland did not have a current qualifying letter on file. Ms. Stevens directed Ms. Holland to complete speech services with the children in their classroom. As a reminder, all outside venders need to have a current qualifying letter on file before being left alone with children. Pursuant to GS 110-90(1a) the Division establishes an annual fee for licensed child care facilities. The invoices for the fees are assessed based on your licensed capacity and mailed after the 1st of November every year. You will only receive one invoice, and payment is due in FULL within 30 days. Please closely monitor your mail and be on the lookout for your invoice. If you have not received your invoice by November 15th, you should contact the Division at 1-800-850-0829. Please do not delay making your payment and be aware that Payment Plans are NOT available. In addition, please be aware that failure to pay a license fee within 30 days of receipt will result in late charges.
