Upon my arrival, I was greeted by K. Sander, assistant director. I was accompanied by Ms. Sander as I completed a general walk-through of the indoor and outdoor areas, which consisted of five (5) classrooms, the kitchen, and the outdoor play environment. The children were observed during outdoor play, free choice activities, and completing routine care tasks. Today’s meal was observed and in compliance with the Meal Patterns. **Current License Status: Currently this center operates with a five-star license issued August 16, 2021. ** Annual Inspections: The last sanitation inspection was completed March 20, 2025, and received a ‘Superior’ classification with 4 demerits. The last fire inspection was completed on March 4, 2025. The last documented fire drill was conducted on September 29,2025 the last documented lockdown drill was completed on August 15, 2025, and the last documented playground inspection was completed on September 23, 2025. Prior to today’s visit, I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed as current/active. During today’s visit, a partial assessment of Child Care Requirements was conducted. The monitoring included: supervision of children, staff/child ratio, storage of hazardous substances, storage of medicine, adequate/ approved space, posted license, license restrictions, attendance logs, program records and safe sleep checks. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. Menu posted was not current. 10A NCAC 09 .0901(b) The violation cited during today’s visit was corrected therefore, a compliance letter is not needed. Prior to today’s visit, the 18-month compliance history for this facility was 95%. Technical Assistance/ Consultation: During today’s visit we discussed the QRIS Modernization Pathways. Upon my arrival, I was greeted by assistant director K. Sanders. Ms. Sanders stated that the owner, operator B. King, was interviewing a potential candidate and would be with me shortly. Ms. King arrived a short time later. Limited monitoring of the child care requirements occurred during today’s visit. The license was posted. I was accompanied by Ms. King as I completed a general walk-through of the indoor environment. I monitored the staff/child ratio, adequate/approved space, supervision, and interaction with children. A total of fifty eight (58) children were present. During today’s visit, I discussed the allegations with Ms. King. Ms. King was given the opportunity to share her perspectives regarding the information surrounding the allegations. I reviewed the program’s discipline policy, a child’s file, and a message from a parent on the ProCare application. Allegations: *There are concerns that a child was put in a compression vest without parents’ permission. *The center did not follow discipline policy. During today’s visit Ms. King stated that a compression vest was purchased and used with parents’ permission due to a child being over stimulated by the noise level and size of the classroom environment. Ms. King stated that she met with the parent in September about the compression vest. Ms. King showed me a message with the parent stating that she put the vest on the child in the morning because he was upset. The individual needs of each child that comes to the facility with a disability are looked at. The teachers give the child the opportunity to become acclimated to the environment. A child that is exhibiting behavioral challenges will use interventions such as picture cues, removal from classroom for short time periods, and quiet spaces in the classroom. Messages are being sent to parents as needed. Parent teacher conferences are held to discuss the child’s behavior. Ms. King stated that she provides parents with various resources to gain support for their children. Ms. King could not provide any documentation on the parent’s permission to use the compression vest. Ms. King stated that a child got aggressive over a period of time hurting other children and pushing over shelves. It was difficult for the teacher to shadow the child and care for the other children in the classroom. Ms. King asked the family to provide the child with additional resources. The family did not provide the resources. Ms. King disenrolled the child. Based on my observations and statements made by the staff, the allegation regarding there are concerns that a child was put in a compression vest without parents permission is SUBSTANTIATED. Based on the allegation of the center not following the discipline policy is UNSUBSTANTIATED. The following violations were cited during today’s visit Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A child’s developmental needs were being addressed via calming support modality provided by the center. Center staff confirmed verbal permission was granted prior to use, but documentation to support permission could not be provided. G.S. 110-91(10) In order to comply with the NC Laws and Rules, a compliance letter must be submitted to me by February 24, 2025. The compliance letter must address each violation, explain how it has been corrected, and how it will be maintained in the future. If the letter of compliance is not received by the date listed a return visit will be made to the child care center. As a reminder, educate your families on giving diaper creams and medicines to a staff member to ensure that medicines are properly stored. A closet was observed unlocked that contained a purse with perfume, lotion, and deodorant that all need to be inaccessible to children. As a reminder, all purses need to be locked and inaccessible to children. Pursuant to GS 110-90(1a) the Division establishes an annual fee for licensed child care facilities. The invoices for the fees are assessed based on your licensed capacity and mailed after the 1st of November every year. You will only receive one invoice, and payment is due in FULL within 30 days.
