Upon my arrival I was greeted by the operation’s manager Davinia Robinson. I was accompanied by Ms. Robinson as I completed a general walkthrough of the indoor environment which consisted of six (6) classrooms, kitchen and the outdoor environment. The children were observed during outdoor play, teacher directed activities, and completing routine care tasks. Today’s meal was in compliance with the Meal Patterns. ** License Status The facility is currently operating with a three-star license issued on July 9, 2025. ** Annual Inspections: The last sanitation inspection was completed March 17, 2026, and received a ‘Superior’ classification with 5 demerits. The last fire inspection was completed on December 12, 2025. The last documented fire drill was conducted on April 7, 2026, the last documented lockdown drill was completed on April 13, 2026, and the last documented playground inspection was completed on April 3, 2026. Prior to today’s visit I reviewed the NC Secretary of State’s website and observed that the owner of this facility is listed current/active. During today’s visit, a partial assessment of Child Care Requirements was conducted. The monitoring included: supervision of children, staff/child ratio, storage of hazardous substances, storage of medicine, adequate/ approved space, posted license, license restrictions, attendance logs, program records. The following violations were observed, documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. Over the counter Tylenol had the child's name missing. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A tube of over the counter aquafor had an expiration date of 4/2025. .0803(12) 1756 Enhanced staff/child ratios and group sizes were not met. The classroom designated for one and two year old children was left out of ratio when the office manager left the classroom leaving one (1) teacher to seven (7) children. 10A NCAC 09 .2818 Failure to comply with the Child Care Requirements puts children’s health and safety at risk. As a result of ratio violation, a return visit will be made within two (2) weeks to ensure that compliance has been maintained. In addition to a return visit, in order to comply with the NC Laws and Rules, a compliance letter must be submitted to me by May 12, 2026. The compliance letter must address each violation, explain how it has been corrected, and how it will be maintained in the future. The manager takes one child with her out of the classroom to get paper towels. Leaving the other teacher with seven (7) children out of ratio. It is important that you all know how many children you have at all times and the ratios per each age group. During today’s visit violations were cited regarding administering medications. Giving medicines is a serious responsibility. It requires caregivers to have knowledge and skills. All caregivers and families must understand what is required of them to ensure that children are safe. Medicines can be serious to the health and wellness of children. They can also be dangerous. Having a policy on the safe administration of medication protects your program. Please review childcare rule regarding administration of medication section 10A NCAC 09 .0803. Technical Assistance/ Consultation: During today’s visit, I observed a tube of diaper cream with the expiration date of 2/2024. It is important that the dates on the diaper cream are checked. All medications, including topical ointment, require authorization to administer medication form. The forms must be completed in their entirety, including the full name of the child, dates the medication is to be administered, dosage, frequency and parent’s signature and date. Authorization forms are good for a period of twelve (12) months, so please be sure to include the expiration date. Any medication remaining after the course of treatment is completed, after authorization is withdrawn or after authorization has expired must be returned to the child’s parents. Any medication the parent fails to retrieve within 72 hours of completion of treatment, or withdrawal of authorization, shall be discarded. During today’s visit, I reviewed the fire inspection report. The fire inspection was initiated on December 5, 2025, due to two failed codes because emergency lights were inoperable and the exit lights were inoperable. Name of Operation: All My Children Child Care Center III Facility ID: 96000650 Consultant: BECKY JACKSON Operation Type: Center Case Number: Visit Date: 6/18/2025 Number Present: 14 Completed Date: 6/18/2025 Age: From 0 To 4 Total Minutes: 180 Time In: 10:00 AM Time Out: 01:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of my unannounced visit today was to monitor your center for meeting applicable child care requirements during your Temporary License period. This is the third temporary time period visit for this facility. This facility is currently operating on a Temporary License effective from January 9, 2025 to July 9, 2025. Tameka Johnson, Child Care Consultant assisted with part of today’s visit. E. Muhammad, Administrator was not present when we arrived. The owner representative was present therefore the visit was conducted with her. Four classrooms are operating with 19 children enrolled and 14 children present. During today’s visit both the indoor and outdoor learning environments were monitored. The staff files were reviewed. A sampling of children’s files were also reviewed. The most recent fire drill was conducted on June 3, 2025 at 10:20am. The log reflected it took 3 minutes and 20 seconds to evacuate 15 children and 5 staff. The last outdoor inspection was completed on June 2, 2025. The most recent lockdown drill was conducted on June 5, 2025 at 10:30am. The most recent Sanitation inspection was conducted on March 17, 2025 with a Superior classification and 4 demerits. The children were observed engaged in free play, having snack, conducting routines (toileting and hand washing) and playing outdoors. The infants in care were observed playing with materials on the floor. The lunch being prepared today consisted of pizza, carrots, peaches, and milk. The following violations were documented today: Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. In Space # 1 the time of departure for June 17, 2025 had not been listed for three children. 10A NCAC 09 .0302(d)(4) 428 A current activity plan was not posted for each group of children for reference. The activity plan for the group in Space #4 was dated June 2, 2025 to June 13, 2025. The plan for the group in Space #6 was not dated. GS 110-91(12); .0508(a) All violations must be corrected immediately. Please send me a compliance letter (written statement) describing how and when the above violations were corrected. The compliance letter must be received no later than July 2, 2025. The compliance letter should be sent to me via email, from the email address on file with the Division as the center’s official email address. Contact me if you have questions regarding correction of the violations documented during today’s visit. If you are unable to correct all violations within the specified timeframe, send a compliance letter regarding violations that were corrected and a specific plan to correct the remaining violations. An extension will need to be requested from management. Child care licensing requirements were established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. I explained that you have access to the on-line version of the Child Care Rule Book, as well as a hard copy of the rules. I recommended that you visit the Division of Child Development's website (www.ncchildcare.nc.gov) on a regular basis to find out what's new in childcare. You could also find any needed Child Care forms by going on the web site and clicking on the "Provider" tab and then clicking on the "Provider Documents" tab. Technical Assistance and Consultation: The activity plans must be current and must reflect the use of Foundations as well as any other curriculum being used. If the goals from Foundations are not listed on the activity plans, the staff should be prepared to explain how they use this tool in their activity planning. The plans should have the dates listed to ensure they are current. The time of arrival and departure must be noted each day for each child. Parents may complete the information, however staff should ensure it has been completed. I encourage you to have staff complete the document rather than parents to ensure it is being completed and is accurate. The fall zones for all stationary equipment should be kept free of grass and weeds. The surfacing material should be fluffed to ensure it remains resilient. All equipment that is too heavy for one person to easily move is considered stationary. The trike-go-rounds also must have surfacing, or must be removed from the outdoor play area. The edges of the trike path may become tripping hazards over time. I encouraged you to either maintain the dirt so it is level with the edges of the path or paint the inner and outer edges of the path to ensure visibility and prevent hazards. Keys should not be stored in the locks or doorknobs. This is not considered to be locked storage. There were no hazards present in these areas today, however staff should not get in the habit of storing the keys inside the locks. They should be removed and placed out of the reach of children. RATED LICENSE: A rated license application has been provided for you. If you are interested in applying for a Two through Five Star Rated License at the end of the temporary license, please return this form to me no later than June 30, 2025. Failure to submit the application form will result in a One Star License being issued at the end of the temporary time period. We discussed that we do not have time for a valid Environment Rating Scale Assessment to be conducted before the end of the temporary time period. We also discussed that one lead teacher does not currently have the North Carolina Early Childhood Administration Credential. You stated you were in the process of hiring a lead teacher for this group. We also discussed that two staff members do not have status letters. I asked that they check on the processing of their information though the WORKS system. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. ANNUAL COMPLIANCE VISIT: Your center will be due for an annual compliance visit prior to January 9, 2026. Additional visits may be conducted by the Child Care Consultant prior to that date. I encouraged you to keep an up to date staff and training worksheet available for any monitoring visit that may be conducted. You will need to submit this document to your consultant in preparation for your annual compliance visit. You will need to have an annual fire inspection by December 4, 2025. It is your responsibility to contact the local fire inspector to request this inspection. An annual sanitation inspection will be due by March 17, 2026, however more frequent visits may be conducted. The sanitation and fire inspections must be submitted to your consultant within 7 days. I also reminded you that the water must be tested for lead every three years. ON-GOING TRAINING: Training hours will be monitored for compliance during your annual compliance visit. Training hours are required to be recorded on the on-going training log with certificates attached to the log for verification. HEALTH & SAFETY TRAINING: All staff must complete Health & Safety training topics within one year of hire. CPR/First Aid and Recognizing and Responding to Suspicions of Child Maltreatment are due within the first 90 days of hire. After initial completion all topic areas must be completed every five years. A training log is available on the DCDEE website. This training information will be reviewed by a DCDEE consultant during a monitoring visit. Annually you must review the EPR plan online and print the review form for your records. You must review the EPR plan and the EMC plan with all staff on an annual basis. I recommend doing this at the same time everyone completes an annual health questionnaire and emergency information. This would also be a good time to do annual staff evaluations and staff development plans. Annual Licensing Fee: Pursuant to GS 110-90(1a) the Division establishes an annual fee for licensed child care facilities. The invoices for the fees are assessed based on your licensed capacity and mailed after the 1st of November every year. You will only receive one invoice and payment is due in FULL within 30 days. The fall zones for all stationary equipment should be kept free of grass and weeds. The surfacing material should be fluffed to ensure it remains resilient. All equipment that is too heavy for one person to easily move is considered stationary. The trike-go-rounds also must have surfacing, or must be removed from the outdoor play area. The edges of the trike path may become tripping hazards over time. I encouraged you to either maintain the dirt so it is level with the edges of the path or paint the inner and outer edges of the path to ensure visibility and prevent hazards. Keys should not be stored in the locks or doorknobs. This is not considered to be locked storage. There were no hazards present in these areas today, however staff should not get in the habit of storing the keys inside the locks. They should be removed and placed out of the reach of children. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. ANNUAL COMPLIANCE VISIT: Your center will be due for an annual compliance visit prior to January 9, 2026. Additional visits may be conducted by the Child Care Consultant prior to that date. The owner representative was present therefore the visit was conducted with her. Four classrooms are operating with 19 children enrolled and 14 children present. During today’s visit both the indoor and outdoor learning environments were monitored. The staff files were reviewed. A sampling of children’s files were also reviewed. The most recent fire drill was conducted on June 3, 2025 at 10:20am. The log reflected it took 3 minutes and 20 seconds to evacuate 15 children and 5 staff. The last outdoor inspection was completed on June 2, 2025. The most recent lockdown drill was conducted on June 5, 2025 at 10:30am. The most recent Sanitation inspection was conducted on March 17, 2025 with a Superior classification and 4 demerits. This is the second temporary time period visit. This facility was issued a temporary license from January 9, 2025 to July 9, 2025. Restrictions on the license include daytime care only. Fourteen children were present today. The administrator was not on site and was unable to assist with the visit. The staff member in charge was identified upon my arrival and was notified of the purpose of the visit. The Administrator was reached by phone, and the exit conference was conducted with her via phone and with the staff member in charge in person. All approved indoor spaces were monitored today. Children throughout the facility were napping. The infants and children one year of age in care were observed in bouncy seats and on the floor interacting with the caregivers. The afternoon snack being prepared consisted of tropical fruit, vanilla wafers, and apple juice. No new staff have been hired since the most recent monitoring visit. Therefore staff files were not monitored today. The most recent fire drill was completed on May 5, 2025, at 9:12 am. The log reflected that it took 3 and one-half minutes for 15 children and 5 adults to exit the building safely. The most recent shelter-in-place drill was conducted on March 21, 2025, at 11:10am. The most recent outdoor inspection was completed May 1, 2025. The most recent sanitation inspection occurred on March 17, 2025, with a superior classification and four demerits. The following violations were documented today: Violation Number Comment Rule 802 Safe pick-up and delivery procedures were not communicated to parents and/or were not posted where they can be seen by the parents. Upon my arrival I observed that the door from the lobby to the classroom areas was locked. No one was present at the desk or in the office for assistance. I rang the provided doorbell three times with no answer. I entered the office to access the classroom area after the third doorbell was not answered. I observed that the two staff members present were being counted in staff/child ratio and were unable to answer the door without leaving children unsupervised. The staff member in charge stated two staff members were on break. I reminded you that access must be provided at all times for both inspectors as well as parents. I highly encouraged you to review the procedures for ensuring access is provided at all times. Reminders and Technical Assistance The rules regarding the naptime exception for staff/child ratio were reviewed today. For groups of children two years of age and older, one person may remain the room, while the other staff member is on call. In order to be on call the staff member must be within a natural calling distance without the use of cell phones, walkie-talkies, or other devices. They may not be outside the building in order to be considered on call. Each staff member may be on call for only one group at a time. Activity plans must be developed using North Carolina Foundations for Early Care and Learning. I encouraged you to use the codes provided in foundations on your activity plan documents to demonstrate use of this tool. Otherwise, staff will need to be able to explain how the tool is used to develop the plan. Sand and water, science and nature, and music and movement activities must be provided for all groups of children ages three and older at least once per week. RATED LICENSE: Today we again discussed the two-component rated license process. The Application for Assessment for a Two Component Star Rated License was provided today. I asked that you return the form as soon as possible. We reviewed that staff education must be submitted to WORKS and evaluated before I can determine how many points the facility will earn in Education Standards. If staff are hired and their education is not evaluated via WORKS, I cannot determine points for Education for that person and this can affect the points earned for this facility in the Education Standards component of the Rated License. You can access the WORKS portal and information pertaining to WORKS via DCDEE’s website and click on the “Services” tab, the “DCDEE WORKS”. There are tutorials available to assist a person with creating their WORKS account. The quality point form was also provided today. All points for the Rated License will be determined during the final temporary time period visit. A third and final temporary time period visit will be conducted in the near future. Muhammad, Administrator was not present when I arrived. One of the lead teachers identified herself as the person in charge and reached Ms. Muhammad by phone. She arrived and assisted with today’s visit. Three classrooms are operating with 9 children enrolled and 7 children present. During today’s visit, infants were observed being held by the caregiver and playing on the floor. The remaining groups were observed having engaged in free play activities and creating art projects. There are currently no school age children enrolled. Lunch today consisted of taco salad with creamed potatoes, mixed fruit, and milk. According to the North Carolina Secretary of State website, your corporation All My Children Family Resource Center is current and active. Your last fire drill was completed February 27, 2025 at 9:10am. The log reflected it took two minutes and thirty seconds to evacuate 3 children and 4 adults. The last outdoor inspection was completed March 10, 2025. The most recent sanitation inspection was conducted on March 17, 2025 with a Superior classification and 4 demerits. The most recent fire inspection was conducted December 4, 2024. I monitored your classroom environments, outdoor playgrounds, a sampling of staff and children's records and required center records. I used the North Carolina Child Care Requirements and Compliance Listing as basic monitoring tools during the visit. Your classrooms have a variety of developmentally appropriate learning materials and equipment and the activity centers are well arranged. The license was posted along with the NC Summary of the Law and safe arrival and departure procedures. The emergency medical care plan and emergency telephone numbers were also posted. Activity plans and daily schedules were posted. No concerns with supervision were observed during the visit. Three children’s records were reviewed today and were in compliance. Six staff files were also reviewed today. Using a checklist helps verify information on file and easily identifies missing information. The facility’s Temporary License was issued January 9, 2025 and is valid through July 9, 2025. Restrictions on the permit include a first shift capacity of 146 children ages 0 to 12 years old. E. Muhammad, Administrator was not present when I arrived. One of the lead teachers identified herself as the person in charge and reached Ms. Muhammad by phone. She arrived and assisted with today’s visit. Three classrooms are operating with 9 children enrolled and 7 children present. During today’s visit, infants were observed being held by the caregiver and playing on the floor. The remaining groups were observed having engaged in free play activities and creating art projects. There are currently no school age children enrolled. Lunch today consisted of taco salad with creamed potatoes, mixed fruit, and milk. According to the North Carolina Secretary of State website, your corporation All My Children Family Resource Center is current and active. Your last fire drill was completed February 27, 2025 at 9:10am. The log reflected it took two minutes and thirty seconds to evacuate 3 children and 4 adults. The last outdoor inspection was completed March 10, 2025. The most recent sanitation inspection was conducted on March 17, 2025 with a Superior classification and 4 demerits. The most recent fire inspection was conducted December 4, 2024. I monitored your classroom environments, outdoor playgrounds, a sampling of staff and children's records and required center records. I used the North Carolina Child Care Requirements and Compliance Listing as basic monitoring tools during the visit. Your classrooms have a variety of developmentally appropriate learning materials and equipment and the activity centers are well arranged. The license was posted along with the NC Summary of the Law and safe arrival and departure procedures. The emergency medical care plan and emergency telephone numbers were also posted. Activity plans and daily schedules were posted. No concerns with supervision were observed during the visit. Three children’s records were reviewed today and were in compliance. Six staff files were also reviewed today. Using a checklist helps verify information on file and easily identifies missing information. The following violations of child care requirements were observed today: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months.
