This visit was conducted by Turkessha Casterlow, Childcare Consultant with Sherita Garrison, Assistant Director. Your program currently operates with a Five Star license. Restrictions include Daytime care only; Meets enhanced ratios; Meets enhanced space. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on February 25, 2026, and Kerley and King, INC was listed as current and active. The program’s compliance history was 94% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • ITS-SIDS • CBC Qualification • General Health • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation A walk-through of the facility was completed today. Indoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children during routine care, napping, and quiet activties. I observed materials and activities that were available to the children in the classrooms. The last annual compliance visit was conducted on September 11, 2025. A sanitation inspection was completed on November 25, 2025, with a Superior classification. The last fire inspection was conducted on September 25, 2025. Program records and required postings were monitored. A fire drill was conducted on February 10, 2026. A shelter in place drill was documented on December 9, 2025. An outdoor inspection was documented on February 11, 2026. Two new staff member was reported at this program. Files for new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The following violations were cited during today’s visit: Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Infants in Room 6 under the age of 1 did not have updated sleep checks. .0606(g) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Regular monitoring (ideally every 15 minutes) allows caregivers to check for safety hazards, identify health issues like over-heating or breathing difficulties, and monitor development. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Funny Daffer. The last annual compliance visit was conducted on October 8, 2024. A sanitation inspection was completed December 18, 2024, with a Superior classification. The last fire inspection was conducted October 11, 2024. Program records and required postings were monitored. A fire drill was conducted on August 25, 2025. A shelter in place drill was documented on June 5, 2025. An outdoor inspection was documented on August 4, 2025. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in locked cabinets. Storage and administration of medication were monitored. Medication authorization was monitored. You stated that you do provide transportation in the afternoon for school age children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported, and the bus log. I observed first aid kits and fire extinguishers on the vehicle. I monitored two vehicles today. I observed a 2010 Thomas Bus, Tag #ZXB-8335 and a 2009 STCR bus, Tag #Visions3. These vehicles were clean, in good repair, had working seatbelts, had adequate tire tread, and were current on inspection and insurance. Your next inspection date on this vehicle is due August 2026, and the insurance is valid through February 23, 2026. The following violations were cited during today’s visit: Violation Number Comment Rule 813 Electric fans were not mounted out of reach or did not have a mesh guard to prevent access. A fan in Room 4 was located in the reach of children on a shelf. Three other classrooms had the fans located up high. They were not mounted or had mesh coverings. 10A NCAC 09 .0604(d) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. There was a container of Banana Boat sunscreen, Boudreaux diaper cream and a prescribed Nystatin ointment without permission to administer forms located in the classroom. One child file that I reviewed did not contain the health professional information. .0802(c)(2) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. K. McCandless did not have a medical report on file. Accurate records help prevent the spread of illness, ensure children receive appropriate care, and provide a basis for meeting their physical and emotional needs. • Covering fans is important in child care to protect children from physical injury by preventing their fingers or small objects from contacting the moving blades and to improve air quality, as children are more vulnerable to airborne contaminants that can hinder lung development and trigger respiratory issues. • Staff in childcare need a medical report before starting work to ensure they are physically and emotionally fit to care for children, to prevent the spread of infectious diseases, and to comply with state regulations designed to protect the health and safety of children in their care. These reports indicate the staff member's ability to perform duties and screen for conditions like tuberculosis (TB), which could pose a risk to vulnerable children. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • We discussed making a medication binder for the front office to keep track of the children’s medication forms. • A. Wilson will need CPR/First Aid by 11/7/25 and Health and Safety Training by 8/7/2026. E. Summers has until 5/5/26 to complete Health and Safety Training. • The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. Programs should begin evaluating staff qualifications including updating staff education in NC Works, curriculum, and classroom interactions to determine the most suitable pathway. Updates to staff qualification standards and assessment tools are expected, and while current star ratings remain valid for now, all new ratings will be issued under the revised QRIS system. Your program’s quality should reflect not only compliance, but also a strong commitment to best practices, child development, and continuous improvement. This visit was conducted by Turkessha Casterlow, Childcare Consultant with Sherita Garrison and Valerie Morris, Co-Directors. Your program currently operates with a Five Star license. Restrictions include Daytime care only; Meets enhanced ratios; Meets enhanced space. The license was observed, and the restrictions were found in compliance. The NC Secretary of State website was reviewed on September 10, 2025, and Kerley and King, Inc was listed as active and not current. Mrs. Morris went on the Secretary of Staff website and updated the information. The program’s compliance history was 91% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children in free play, playing in their outside learning environment, and lunch. I observed materials and activities that were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Funny Daffer. The last annual compliance visit was conducted on October 8, 2024. A sanitation inspection was completed December 18, 2024, with a Superior classification. The last fire inspection was conducted October 11, 2024. Program records and required postings were monitored. A fire drill was conducted on August 25, 2025. A shelter in place drill was documented on June 5, 2025. An outdoor inspection was documented on August 4, 2025. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in locked cabinets. Storage and administration of medication were monitored. Medication authorization was monitored. You stated that you do provide transportation in the afternoon for school age children. During today’s visit, I monitored emergency information, permission to transport forms for all children being transported, and the bus log. I observed first aid kits and fire extinguishers on the vehicle. I monitored two vehicles today. I observed a 2010 Thomas Bus, Tag #ZXB-8335 and a 2009 STCR bus, Tag #Visions3. These vehicles were clean, in good repair, had working seatbelts, had adequate tire tread, and were current on inspection and insurance. Your next inspection date on this vehicle is due August 2026, and the insurance is valid through February 23, 2026. The following violations were cited during today’s visit: Violation Number Comment Rule 813 Electric fans were not mounted out of reach or did not have a mesh guard to prevent access. A fan in Room 4 was located in the reach of children on a shelf. Three other classrooms had the fans located up high. They were not mounted or had mesh coverings. 10A NCAC 09 .0604(d) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. There was a container of Banana Boat sunscreen, Boudreaux diaper cream and a prescribed Nystatin ointment without permission to administer forms located in the classroom. The medication was removed from the classroom by the co-director. 10A NCAC 09 .0803(1)(a & b) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Several staff members did not update their health questionnaire within the year timeframe. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Emergency information forms for several employees were last completed more than a year ago. .0701(a) 1314 Emergency information did not name child's health care professional. One child file that I reviewed did not contain the health professional information. .0802(c)(2) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. K. McCandless did not have a medical report on file. The staff member went and picked up the report during the visit. .0701(d) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before September 25, 2025. Technical assistance discussed during today’s visit related to violations: • Obtaining health information for children in childcare is vital for ensuring their well-being and safety, allowing caregivers to respond effectively to emergencies, manage chronic conditions, and facilitate communication between parents and healthcare providers. Accurate records help prevent the spread of illness, ensure children receive appropriate care, and provide a basis for meeting their physical and emotional needs. • Covering fans is important in child care to protect children from physical injury by preventing their fingers or small objects from contacting the moving blades and to improve air quality, as children are more vulnerable to airborne contaminants that can hinder lung development and trigger respiratory issues. • Staff in childcare need a medical report before starting work to ensure they are physically and emotionally fit to care for children, to prevent the spread of infectious diseases, and to comply with state regulations designed to protect the health and safety of children in their care. These reports indicate the staff member's ability to perform duties and screen for conditions like tuberculosis (TB), which could pose a risk to vulnerable children. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • We discussed making a medication binder for the front office to keep track of the children’s medication forms. • A. Wilson will need CPR/First Aid by 11/7/25 and Health and Safety Training by 8/7/2026. E. Summers has until 5/5/26 to complete Health and Safety Training. • The process of notifying the Division of new employees has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new childcare providers working who were hired or moved into the childcare facility within five business days. Beginning later this year, providers will be able to choose from three star-rating pathways: Program Assessment, Classroom & Instructional Quality Accreditation. This new approach allows programs to align their strengths with the pathway that best fits their operations. Now is the time to prepare. Programs should begin evaluating staff qualifications including updating staff education in NC Works, curriculum, and classroom interactions to determine the most suitable pathway. Updates to staff qualification standards and assessment tools are expected, and while current star ratings remain valid for now, all new ratings will be issued under the revised QRIS system. Your program’s quality should reflect not only compliance, but also a strong commitment to best practices, child development, and continuous improvement. Providers are encouraged to take advantage of available technical assistance and training opportunities. A provider meeting will be held on September 29, 2025, from 1:00–3:00 PM at Impact Alamance. TECHNICAL ASSISTANCE: DISCIPLINE: 10A NCAC 09 .1803 PROHIBITED DISCIPLINE IN CHILD CARE CENTERS (a) No child shall be subjected to any form of corporal punishment by the owner, operator, director, or staff of any child care center. For purposes of this Rule, "staff" shall mean any regular or substitute caregiver, any volunteer, and any auxiliary personnel, including cooks, secretaries, janitors, maids, or vehicle drivers. The following shall apply at all child care centers: (9) No child shall ever be yelled at, shamed, humiliated, frightened, threatened, or bullied; (b) Discipline practices shall be age and developmentally appropriate. MEDICATION/PERMISSION TO ADMINISTER - Before assuming responsibility for administration of prescription medicine, facilities must have written confirmation of orders from the prescribing health professional that includes clear, accurate instructions and medical confirmation of the child’s need for medication while in the facility. Caregivers/teachers should not administer medication based solely on a parent’s/guardian’s request. Proper labeling of medications is crucial for safety (1). Both the child’s name and the name and dose of the medication should be clear. Medications should never be removed from their original container. All containers should have child resistant packaging. Potential side-effects are usually included on prescription and OTC medications if the packaging is left. This facility operates we have 5-Star License which was issued on January 25, 2019. The last Annual Compliance Visit was conducted on October 24, 2023. Upon arrival to the facility, I was greeted by the Ms. Sharita, Administrator. I stated the purpose of my visit and Ms. Sharita assisted me on a walkthrough of the facility. Attendance was taken and observations were made. The children were observed in outdoor free play, indoor free play, teacher directed activities, routines, having lunch and resting. The last fire inspection was conducted on October 11, 2023 and was approved for daytime care only. The last sanitation inspection was conducted on April 24, 2024. The facility achieved a superior classification. Limited monitoring was conducted this date. The permit was posted, restrictions maintained. I monitored supervision, staff child ratio, program records, criminal background checks, program postings, and emergency trainings, including CPR/First Aid, IT SIDS. The following violations were observed, cited, and technical assistance was provided. The last Annual Compliance visit was conducted on November 3, 2022. The facility currently operates with a Five-Star Child Care license that was issued on January 25, 2019. The last Sanitation Inspection was completed on July 18, 2023, with a "Superior" classification and (4) demerits. The last Fire Inspection was conducted on October 11, 2023, and the facility was approved for daytime care only. The last Fire Drill was conducted on September 12, 2023 The last Lock Down/ Shelter-in-Place was on July 11, 2023. The last Playground Inspection was conducted on October 19, 2023. Upon arrival, I was greeted by the director, Valerie Morris. I stated the purpose of my visit and was assisted on a walkthrough of the facility where attendance was documented, and observations made. The children were observed participating in routines, indoor free play, and outdoor activities. A checklist was used to note the requirements I monitored today. The North Carolina Department of the Secretary’s website was used to check the status of the corporation on October 20, 2023. The facility’s program status is current and active. During today's visit, I completed a full assessment of applicable Child Care Requirements; license posted, restrictions maintained, staff/child ratio and adequate approved space. Program Records, Criminal Background Checks were also monitored. Staff and Children’s files were monitored as well as Emergency Drills, Medication, Playground Inspections and Transportation. The following three vehicles used to transport children were monitored today. All vehicles are insured through West Bend Mutual Insurance and is good 2/23-2/24. Vehicle 1. Chevrolet minibus Tag # VISIONS5 registration expires 3/31/24. Vehicle 2. Chevrolet minibus Tag # ZXB-8335 registration expires 8/31/24. Vehicle 3. Ford bus Tag # VISIONS3 registration expires 8/31/24. The following violations were observed during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On the large preschool playground there was a large sharp rock behind a dinosaur climber. A Corrective Action Letter must be drafted with the name of your facility, the facility identification number, and your signature.
