Upon my arrival, I was greeted by Ms. D. Tickle, the director. Ms. Tickle gave me the necessary paperwork I needed to complete today’s visit. I observed and documented the attendance. Thirty-five (35) children were present during today’s visit. They were observed playing in housekeeping, watching a movie, drawing, playing in the gym, listening to a story, using the bathroom, looking at books and napping. Limited licensing requirements were monitored. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: proper storage of medications; use of approved space, permit posted; permit restrictions, new staff files CPR and First Aid. Three (3) violations were cited during today’s visit. Violation Number Comment Rule 862 The EMC plan was not reviewed with all staff annually and whenever the plan was revised. The EMC plan was not reviewed with two new staff. 10A NCAC 09 .0802(a) 1825 All staff did not review the center's EPR Plan during orientation and/or on an annual basis with the trained staff. Documentation of the review was not maintained on file. Two staff did not review the center's EPR Plan during orientation. .0607(f) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. If I do not receive your letter by March 14,2024/or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: I discussed with Ms. Tickle that the staff members need CPR and First Aid Trainings by specific dates. Please refer to your previous staff/training works form. Thank you for your time. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000). A checklist was used to note the requirements I monitored today. This center operates with a GS-110 Notice of Compliance effective October 8, 2009. Upon my arrival to the facility, I was greeted by Ms. R Overman, the assistant administrator. Ms. Overman informed me that Dana tickle, the administrator was on vacation. Ms. Overman assisted me during my visit today. All indoor and outdoor spaces used use by the children was observed today. All required items were posted. All medications and hazardous items were out of the children's reach. Thirty-eight (38) children were present during today’s visit. I observed the children playing with available toys, listen to a story, watching a movie, washing their hands in preparation for lunch eating lunch and napping. A sanitation inspection was conducted on August 21,2023. This facility received a superior rating with three (3) demerits. The fire inspection was conducted on July 18,2023. The original inspection will be mailed to Raleigh with this documentation. Staff and children files were monitored during today’s visit. The staff and training worksheets were updated during today’s visit. Your signature on these forms serves as verification the information provided is accurate and complete. This facility does not transport the children. Two (2) violations were cited during today’s visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. One child's medication authorization has expired. 10A NCAC 09 .0803(4)(6-9) 1322 A written statement from each child's parent giving standing permission which may be valid for up to twelve months for participation in off premise activities that occur on a regular basis was not available. Five children did not have updated permission to participate in off premise activities. .1005(b)(4) The above violations must be corrected immediately, and a letter must be sent to me no later than September 14,2023 stating how each item was corrected.
