Name of Operation: Home Sweet Home Facility ID: 36000654 Consultant: JENNIFER H ROBERTS Operation Type: Family CC Home Case Number: Visit Date: 6/2/2026 Number Present: 0 Completed Date: 6/2/2026 Age: From 0 To 0 Total Minutes: 290 Time In: 10:10 AM Time Out: 03:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today's unannounced visit has been to monitor your compliance with all applicable child care requirements during the first temporary time period visit. You, Lareshia Caves, Operator, assisted me with the visit. This facility was issued a temporary license on March 4, 2026, with a restriction of daytime and overnight. The Secretary of State website was checked on June 2, 2026, and your business, Home Sweet Home Childcare llc, was active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, your consultant, at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. All designated indoor and outdoor spaces used for child care were monitored as well as the materials, equipment and required posted items for meeting all minimum licensing requirements. You currently do not have any children enrolled on first, second, or third shift. You have not enrolled any children since your temporary license was issued on March 4, 2026. I reminded you that you will need to complete a fire drill as soon as you enroll a child. You are required to complete monthly outdoor inspections. You completed the lead water test on May 5, 2025. You need to complete the lead-based paint and asbestos survey on the following website: cleanwaterforusclassrooms.org. You do not provide transportation. We discussed the Pathways to the Stars. I suggested that you continue to review the information regarding QRIS Modernization on DCDEE’s website under “What’s New.” You stated you plan to apply for a rated license and plan to meet the requirements for the Classroom and Instructional Quality Pathway. I provided you with the rated license application and applicable documents. Continue to review child care rule .3206 for information regarding the requirements for the Classroom and Instructional Quality Pathways. You must complete and submit these documents to me no later than July 1, 2026. You have established a DCDEE WORKS account including education evaluations completed. Documentation from Workforce on the results of the education evaluation was on file for review. I reminded you that programs with less than a three-star license will not be eligible to receive subsidy funding. The following violations were observed today: Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. Monthly checks for hazards on the outdoor play area were not completed in March, 2026, April 2026, or May 2026. 10A NCAC .1721(e)(5)(A-F) 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The ABCMS provider portal training had not been completed to establish a provider portal facility roster. G.S. 110-90.2 & .2703(r) Technical Assistance was provided on the following: Criminal Background Checks: There was no ABCMS provider portal facility roster completed as of today’s visit. Operators shall notify the Division of all new childcare providers or household members, as defined in G.S. 110-90.2(a)(2), who are hired within five business days. During the visit, you stated you had not completed the required Moodle training to obtain a provider code needed to move forward with this requirement. To correct this violation, you must complete the Moodle training, obtain a provider code and connect all household members to the ABCMS Provider Portal roster for this facility. I suggested that you add family members to the roster within five days of their entering/leaving the household. You completed the Moodle training during the visit and submitted your Power Form. I emailed you a copy of the ABCMS Provider Technical Assistance document during the visit. Outdoor Inspections: Monthly outdoor inspections must be completed every month even if children are not enrolled. I suggested that you place a reminder on your phone to alert you to complete an outdoor inspection. Reminders: Aerosol spray cans must be kept in locked storage when children are in care. We discussed conducting daily safety checks of the home once you enroll children. Your temporary license will expire after September 4, 2026. Child Care Rule .3206 (c) For a family child care home to earn a three-star rated license, the following requirements shall be met: (1) The operator shall meet education standards at the three-star level as set forth in Rule .3217 of this Section or higher. (2) The operator shall implement the family and community engagement standards set for in Rule .3220 of this Section for a three-star rated license. (3) The operator shall implement the continuous quality improvement standards set forth in Rule .3221 of this Section. (4) The operator shall implement a curriculum for all ages served that is: (A) aligned with the North Carolina Foundations for Early Learning and Development; (B) child focused; (C) developmentally appropriate; and (D) culturally and linguistically appropriate. (5) In curriculum planning the operator shall ensure modifications and adaptations are made to involve all children with special health and developmental needs. (6) The operator shall ensure child observations are conducted for each child enrolled in the family child care home. Child observations shall include evidence of the child's activities and may include the following: (A) anecdotal notes; (B) portfolio of child's work samples; (C) developmental screenings; (D) formative assessment tools; or (E) photographs, audio, pr video recordings with permission from the child's family. (7) The operator participates in one of the following activities regarding classroom and instructional quality practices: (A) annual completion of five hours of coaching or mentoring by a center administrator of a licensed child care center with a star-rating of three stars or higher, an operator of another licensed family child care home with a star-rating of three stars or higher, or a community resource partner; (B) annual completion of five on-going training hours, in addition to applicable requirements in Rule .1703(f) of this Chapter and Rule .3217 of this Section; or (C) annual completion of 0.5 continuing education unit, in addition to applicable requirements in Rule .1703(f) of this Chapter and Rule .3217 of this Section. (d) For a family child care home to earn a four-star rated license, the following requirements shall be met: (1) The operator shall meet education standards at the four-star level as set forth in Rule .3217 of this Section or higher. (2) The operator shall implement the family and community engagement standards set forth in Rule .3220 of this Section for a four-star rated license. (3) The operator shall implement the continuous quality improvement standards set forth in Rule .3221 of this Section. (4) The operator shall implement a curriculum that has been approved by the Commission as defined in Rule .0102(13) of this Chapter for all ages served. A list of curricula that has been approved by the Commission is available at http://ncchildcare.ncdhhs.gov/Services/Licensing/Star-RatedLicense/Curriculum. (5) In curriculum planning, the operator shall ensure modifications and adaptations are made to involve all children with special health and developmental needs. (6) The operator shall conduct on-going formative assessments to gather information about each child's growth and skill development based on individual development and learning needs and maintain evidence of each child's on-going assessment. The operator shall use a formative assessment(s) that has been approved by the Commission, available at http://ncchildcare.ncdhhs.gov/Services/Licensing/Star-Rated-License/Curriculum. Evidence of formative assessments shall include documentation required or contemplated by the approved formative assessment and may also include: (A) anecdotal notes; (B) portfolio of child's work samples; (C) developmental screenings; or (D) photographs, audio, or video recordings with permission from the child's family. (7) The operator shall share the results of the formative assessment with families at least once annually. The manner in which the results are shared shall be convenient for the family such as by telephone, email, virtually, or in-person. (8) The operator shall complete training related to the curriculum and formative assessment tool that is used by center staff with children, in addition to applicable requirements in Rule .1703(f) of this Chapter. Verification of this training shall be maintained in the operator's personnel file. (9) The operator participates in one of the following activities regarding classroom and instructional quality practices: (A) annual completion of five hours of coaching or mentoring by a center administrator of a licensed child care center with a star-rating of four or five stars, an operator of another licensed family child care home with a star-rating or four or five stars, or a community resource partner; (B) annual completion of five on-going training hours, in addition to applicable requirements in Rule .1703(f) of this Chapter and Rule .3217 of this Section; or (C) annual completion of 0.5 continuing education unit, in addition to applicable requirements in Rule .1703(f) of this Chapter and Rule .3217 of this Section. Health and Safety Trainings: You will need to complete Health and Safety training and Emergency Preparedness and Response Training no later than March 4, 2027. As part of the Health and Safety Training requirements you must complete the following found in the child care requirements for family child care homes. See below. 10A NCAC 09 .1703 ON-GOING REQUIREMENTS FOR FAMILY CHILD CARE HOME OPERATORS (a) After receiving a license, family child care home operator shall: (1) Update the health questionnaire annually. The Division may request an evaluation of the operator's emotional and physical fitness to care for children when there is reason to believe that there has been a deterioration in the operator's emotional or physical fitness to care for children. This request may be based upon factors such as observations by the director or center staff, reports of concern from family, reports from law enforcement or reports from medical personnel. The Division may require the operator to obtain written proof that he or she is free of active tuberculosis when the Division determines that the operator was exposed to a person with active tuberculosis; (2) Renew First Aid training on or before expiration of the certification appropriate for the ages of children in care; (3) Renew CPR course on or before the expiration of the certification appropriate for the ages of children in care; (4) Renew ITS-SIDS training every three years from the completion of previous ITS-SIDS training; and (5) Complete Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of licensure. This training shall count toward requirements set forth in Paragraph (d) of this Rule. Recognizing and Responding to Suspicions of Child Maltreatment training is available at https://www.preventchildabusenc.org/. (b) Family child care home operators and staff members shall complete health and safety training within one year of employment, unless the operator or staff member has completed the training within the year prior to beginning employment or within the year prior to receiving a license. Health and safety training shall be in addition to the pre-licensing visit and new staff orientation requirements set forth in Rules .1702(d) and .1729(c) of this Section. The following persons shall be exempt from this requirement: (1) service providers such as speech therapists, occupational therapists, and physical therapists; and (2) substitutes who provide services for less than 10 days in a 12-month period. (c) The health and safety training shall include the following topic areas: (1) Prevention and control of infectious diseases, including immunization; (2) Administration of medication, with standards for parental consent; (3) Prevention of and response to emergencies due to food and allergic reactions; (4) Building and physical premises safety, including identification of and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; (5) Emergency preparedness and response planning for emergencies resulting from a natural disaster, or a man-caused event; (6) Handling and storage of hazardous materials and the appropriate disposal of biocontaminants; (7) Precautions in transporting children, if applicable; (8) Prevention of shaken baby syndrome, abusive head trauma, and child maltreatment; (9) CPR and First Aid training as required in Rule .1102(c) and (d) of this Chapter; (10) Recognizing and reporting child abuse, child neglect, and child maltreatment; and (11) Prevention of sudden infant death syndrome and use of safe sleeping practices. Compliance Letter: You must correct the violations found during today's visit immediately. Please send me a letter verifying compliance by June 16, 2026.
