You, Christianie Dor-Lampton, Owner/Co-Administrator, assisted me with the visit until 1:18pm. The visit was completed with Marie Dor, Co-Administrator. The Secretary of State website was checked prior to the visit on December 17, 2025, and your business Early Visions Academy, LLC, is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. Currently this center operates with a temporary license issued on August 14, 2025, with the following restriction: Daytime care only. During this second Temporary Time Period visit, the classrooms were monitored as well as the playgrounds, materials, equipment and required posted items for meeting all minimum licensing requirements. Children were observed during nap/rest time, transitions, and snack. The Emergency Preparedness and Response Plan was completed on November 19, 2025. A Shelter-in-place drill was conducted on October 22, 2025, at 10:30am. The last monthly fire drill was completed on December 11, 2025, at 11:50am. The last playground inspection was completed on December 1, 2025. Sanitation Inspection: A sanitation inspection was completed on October 17, 2025. A superior classification was issued with two demerits noted on the grade card. A sample of three children’s files were reviewed. Staff files were previously reviewed. A menu was posted and met all the requirements with the Meal Patterns for Children in Childcare. The daily schedule was posted. We discussed the Classroom and Instructional Quality Pathway. You will need to submit the Application for Assessment for a Rated License for Centers to me on or before January 2, 2026. I provided you with a copy of the Family and Community Engagement Standards, Continuous Quality Improvement Plan (CQI) Facility, and the Continuous Quality Improvement (CQI) and Professional Development Plan (PD) Individual. I will review and obtain a copy of the completed Family and Community Engagement Standards and completed Continuous Quality Improvement Plan (CQI) Facility during the third temporary time period visit. We reviewed staff education and the lead teacher currently meets the education level for four stars. To meet the education level for five stars, the lead teacher will need to request a Competency Evaluation by January 2, 2026. I provided you with information regarding the competency evaluation overview. I discussed with you that programs with less than a three-star license will not be eligible to receive subsidy funding. The following violation was observed and corrected during the visit. Violation Number Comment Rule 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. One incident report dated October 28, 2025, was not maintained in the child's file. The incident report was maintained in a the file with the incident log. .0802 (e) Technical assistance was provided on the following: Incident Reports Incident Reports must be maintained in the child’s file. I suggested that you review the Children’s File Checklist with staff to make sure that staff know where to maintain incident reports. This violation was corrected during the visit. To correct the violation, the incident report was placed in the child’s file. Consultation was provided on the following: -Small acorns were observed on playground #2 and will need to be removed prior to enrolling children under three years of age. You currently do not have any children under three years of age enrolled. Reminders: Any observed violations cited during visits will negatively affect the center's Compliance History Score. Your program must maintain a compliance score of at least 75% as required by G.S. 110-90 (4)(c). Your temporary license will end after February 14, 2026. We will also review the Application for a Two Component Star Rated License and the Rated License Assessment Request Form, if applicable, during your next visit. If you plan to choose Pathway #1, I suggested that you begin the three-month self-assessment. The self-assessment can be completed on NCRLAP.org. All new and existing staff must establish a DCDEE WORKS account and have their education evaluations completed by Workforce by December 4, 2025. The lunch menu for October 16, 2025, listed turkey lunchmeat, wheat bread, tator tots, green beans, and milk; however, children were served ham, wheat bread, mozzarella cheese stick, Cheez Doodles, green beans, clementines, and milk. I suggested you review your menus daily and prior to serving a meal or snack to the children to ensure your menu is accurate. Orientation The date, October 1, 2025, was documented for each orientation topic to be completed within the first two weeks of employment; however, the hours of orientation received were not documented on the Documentation of Staff Orientation form for M. Dor. The Documentation of Staff Orientation form reviewed in N. Townsend’s file was blank and did not document the dates orientation was completed or the number of hours received. I suggested when you use the Documentation of Staff Orientation form, that you document the date and the number minutes/hours of orientation you provided at the time the topic area was discussed/reviewed with staff. Consultation was provided on the following. • As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. I also suggested that you register staff now for any upcoming Webinar trainings listed on the ncrlap.org website. The menu that was posted in space #2, and dated October 13, 2025 through October 17, 2025, listed the following food items for lunch on October 16, 2025, turkey lunchmeat, wheat bread, tator tots, green beans, and milk. 10A NCAC 09 .0901(b) 1067 Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. Two staff, with hire dates of October 1, 2025, did not have documentation on file that six clock hours of training in the required topic areas were received within the first two weeks of employment. .1101(a)(b) Technical assistance was provided on the following: Menus Prior to serving a meal or snack, ensure the items listed on the menu are served to the children or update the menu with any changes prior to serving the meal or snack to the children. The lunch menu for October 16, 2025, listed turkey lunchmeat, wheat bread, tator tots, green beans, and milk; however, children were served ham, wheat bread, mozzarella cheese stick, Cheez Doodles, green beans, clementines, and milk. I suggested you review your menus daily and prior to serving a meal or snack to the children to ensure your menu is accurate. Orientation The date, October 1, 2025, was documented for each orientation topic to be completed within the first two weeks of employment; however, the hours of orientation received were not documented on the Documentation of Staff Orientation form for M. Dor. The Documentation of Staff Orientation form reviewed in N. Townsend’s file was blank and did not document the dates orientation was completed or the number of hours received. I suggested when you use the Documentation of Staff Orientation form, that you document the date and the number minutes/hours of orientation you provided at the time the topic area was discussed/reviewed with staff. Consultation was provided on the following. • As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. You must complete this task immediately. • Baby wipes stored in plastic packaging must be stored in a locked cabinet, locked container, or stored at least five feet from the floor surface to ensure the plastic packaging is not accessible to children in attendance that are under three years of age. Parent’s Choice Baby Wipes stored in the plastic packaging were observed on the paper towel dispenser in the bathroom in space #2. Children in attendance today were three years of age and four years of age. General Information Regarding the Environment Rating Scale Assessment. ECERS-3 If you choose to have the Environment Rating Scale completed, I suggested that you visit the website, www.ncrlap.org, for supplemental materials that staff can use when preparing for the Environment Rating Scale Assessment. Additional notes to the Early Childhood Environment Rating Scale Third Edition can be obtained on this website. NCRLAP also offers Webinar trainings for staff. I also suggested that you register staff now for any upcoming Webinar trainings listed on the ncrlap.org website. Contact your Regional Child Care Resource and Referral office for further technical assistance with the Environment Rating Scales. You can contact Gaston/Lincoln Partnership for Children. Compliance Letter: You must correct the violations found during today's visit immediately. Please send me a letter verifying compliance by October 30, 2025. Please include in that letter each violation number and explain in detail how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your corrective action plan letter, that corrections have been made when they have not, it will be considered falsification of information. Reminders: Any observed violations cited during visits will negatively affect the center's Compliance History Score. Your program must maintain a compliance score of at least 75% as required by G.S. 110-90 (4)(c). Your temporary license will end after February 14, 2026. You stated the facility enrolled children and began operating on October 1, 2025. As a reminder, the temporary license ends six months from the date it was issued, and extensions are not given for the temporary license.
