This facility operates with a Three (3) Star License issued June 1, 2018, earning two (2) points in Program Standards, four (4) points in Staff Education, and one (1) quality point. The last sanitation inspection was completed November 24, 2025, with a “Superior” classification. The last fire inspection was completed September 5, 2025, and the program was approved for day-time care and nighttime care. The last annual compliance visit was completed on May 6, 2025. You, D. Kidd, owner/operator, were present visit and ready to assist. The facility contact information was verified as correct. Twenty-seven (27) children were present during today’s visit, ranging from age one (1) to five (5) years. The children were observed in teacher directed activities, free choice activities, eating lunch, toileting and handwashing routines and naptime. Lunch consisted of turkey and gravy, rice, broccoli, applesauce, and milk. The following requirements were monitored during today's visit: Staff/Child Ratio, Supervision, Specialized trainings, Storage of Hazardous Products, Storage of Medication, Adequate/Approved Space, General Safety, Staff Records, Program Records, and License Posted and Permit Restrictions. I used a checklist as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to your program. A computer generated visit summary was reviewed and left with you at the conclusion of the visit. The following violations of child care requirements were documented. Violation Number Comment Rule 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Pierce did not have a staff health questionnaire on record for review. .0701(d) Child Care Licensing Requirements are established to ensure a safe, healthy, and developmentally appropriate environment for all childcare facilities. Therefore, it is important for you to be knowledgeable of all licensing requirements that apply for your facility. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. COMPLIANCE HISTORY Prior to today's visit, this facility has maintained an 18-month compliance score of 95%. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. Note, other Pathways to the Profession of Early Childhood Teacher still include: • EDU 119 – Introduction to Early Childhood Education: This 12-week course introduces the foundations of culturally responsive, equitable and inclusive early childhood education. Available at many N.C. Community Colleges with online, evening, or in-person options. Upon successful completion of EDU-119, the student earns the North Carolina Early Childhood Credential and qualifies to be a lead teacher in a child care center. • Test-Out Option for EDU 119: An early childhood professional who has gained knowledge through work and lived experience and/or professional development may be able to receive Credit for Prior Learning by taking a challenge exam for EDU119. The last sanitation inspection was completed April 8, 2024 with a “Superior” classification. The last fire inspection was completed August 21, 2024, and the program was approved for day-time care and nighttime care. A copy of the most recent fire inspection was submitted today. The last Annual Compliance visit was completed on June 11, 2024. You, D. Kidd, owner/operator, were present visit and ready to assist. The facility contact information was verified as correct. Nineteen (19) children were present during today’s visit. The children were observed in naptime. The following requirements were monitored during today's visit: Staff/Child Ratio, Supervision, Specialized trainings, Storage of Hazardous Products, Storage of Medication, Adequate/Approved Space, General Safety, Staff Records, Program Records, and License Posted and Permit Restrictions. I used a checklist as a basic monitoring tool to assess compliance with all applicable childcare requirements pertinent to your program. A computer-generated visit summary was reviewed and left with you at the conclusion of the visit. The following violation of child care requirements was documented and corrected during the visit. The provisional status has been extended through August 6, 2023, to give the provider more time to make repairs to the septic system. This facility operates with a Three (3) Star License issued June 1, 2018, earning two (2) points in Program Standards, four (4) points in Staff Education, and one (1) quality point. The last Annual Compliance visit was completed on August 3, 2022. The last sanitation inspection was completed January 9, 2023, with a “Provisional” classification. I received documentation from the local sanitation inspector regarding the provisional status of the sanitation inspection. According to the documentation received, the center can safely operate with children in attendance. The provisional status has been extended through August 6, 2023, to give the provider more time to make repairs to the septic system. The last fire inspection was completed August 29, 2022, and the program was approved for day-time care and nighttime care. You, Ms. D. Kidd, owner/operator, was present on this visit and ready to assist. The facility contact information was verified as correct. There were Twenty-seven (27) children present during today’s visit. The children were observed in teacher directed activities, outdoor play, handwashing and toileting routines, and lunch time. Lunch consisted of hot dogs, baked beans, peaches, rolls, and milk. All interactions with the children were positive and nurturing. I used the Annual Compliance Monitoring Checklist for Child Care Centers and the Child Care Center Item Number Listing as a basic monitoring tool to assess compliance with all applicable child care requirements pertinent to this facility. A computer generated visit summary was reviewed and left with you at the conclusion of the visit. The following violations of child care requirements were documented. Violation Number Comment Rule 811 Potentially hazardous items including but not limited to power tools, nails, chemicals, propane stoves, lawn mowers, gasoline, or kerosene were not stored in locked areas, removed from the premises, or made inaccessible to children. Numerous bottles of bleach, Lysol and other cleaning supplies were observed in an unlocked storage shed in the outdoor play area. .0604(a) 824 Outdoor play area was not enclosed by fence with a minimum height of 4 feet. The top of the fence, less than six feet, was not free from protrusions. Some areas of the fence in the outdoor play area did not measure the required 4 feet in height. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. It is your responsibility to maintain compliance with the applicable laws and rules; always whether they have been monitored or specifically discussed with you in the past. The best way for you to ensure that you are meeting all requirements is to periodically review the childcare laws and rules on the Division of Child Development and Early Education (DCDEE) website. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before July 27, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. TECHNICAL ASSISTANCE/ CONSULTATION CHILDREN’S RECORD KEEPING One (1) child’s file did not have a statement with parent’s signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy. The Shaken Baby and Abusive Head Trauma policy should be reviewed and have a documented receipt in all files of children five (5) years old and younger. STAFF RECORD KEEPING Two (2) staff records did not have an updated Staff Emergency Information form on file for review. All staff should complete the Staff Emergency Information form annually and/or as information needs to be updated. Two (2) staff records did not have an annual Staff Evaluation/ Staff Development Plan on file for review. The Staff Development Plan/ Annual Staff Evaluation should be completed annually for each employee. One (1) staff did not complete required Health and Safety trainings within the required five (5) years of completing the previous training. Health and Safety trainings should be completed by staff every 5 years in accordance with the child care rule. PROGRAM RECORDS Shelter in place drill log was incomplete. The last shelter in place drill was documented March 2023. There should have been a drill practiced and documented June 2023. SAFE INDOOR/OUTDOOR ENVIRONMENT Two (2) tubs of sanitizing wipes were observed in the restroom, used by the school aged children on the countertop. All hazardous products should be stored in a locked cabinet or closet to prevent harm to the children. You locked the sanitizing wipes up during the visit. In an unlocked storage shed, in the outdoor play area, numerous bottles of bleach, Lysol and other cleaning supplies were observed. All storage sheds containing hazardous products must be locked at all times, while children are in attendance. You locked the storage shed during the visit. A copy of the Hazardous Items Storage for Child Care handout was left with you at the conclusion of the visit. OUTDOOR PLAY AREA The outdoor play area was not enclosed by fence with a minimum height of 4 feet. All areas of the fence in the outdoor play area should measure a minimum of 4 feet in height. REMINDERS If children over the age of three (3) years old participate in screentime, they should have access a maximum of 30 minutes per day and no more than a total of two and a half hours per week per child and screentime should be documented on a cumulative log or activity plan, and shall be available for review by the Division. Screen time is prohibited for children under the age of three (3). RESOURCES HEALTH AND SAFETY TRAININGS DCDEE has partnered with the NC Child Care Health and Safety Resource Center to offer health and safety trainings in MOODLE. These trainings will remain at no charge for staff. The contract with ProSolutions ended on September 30, 2020. Health and safety trainings are due every five (5) years, including the Recognizing and Responding to Suspicions of Child Maltreatment. The Recognizing and Responding to Suspicions of Child Maltreatment training must be completed via the Prevent Child Abuse NC website. The enrollment process and completion of course materials tend to work best with a windows laptop or desktop using Firefox, but other devices and Google Chrome should also work. You can access the health and safety trainings available in Moodle and Frequently Asked Questions by logging in using NCID and completing the following steps: • From the Moodle home page, students need to log-in with their NCID and password. The log-in is in the upper right corner of the screen. If you do not have a current log-in you will need to create one. • After log-in, the student should scroll to the bottom of the page. In the ‘Search courses’ box, the student should type in the short name “Year 2023 CCDF” and then click go. The student should not register for prior year courses as they are no longer available. • After completing the course search for “Year 2023 CCDF”, the student should be able to register for the course. • Once registered, the student will then need to use the “ADDITIONAL DIRECTIONS” for step-by-step instructions.
