The FCCH operates with a 4 STAR rated license. You are licensed to care for 8 children ages 0-12 and of these 8 children, only 5 children can be preschool aged. Today's visit was conducted with Ingrid Sparrow, owner/operator. Upon arrival to the FCCH today, Ms. Sparrow was observed coming back from a quick trip to a store nearby. I discussed with Ms. Sparrow that she is not approved for transportation and Ms. Sparrow stated that she had e-mailed me her insurance information last year but I advised that I did not receive it. We discussed that I would review all paperwork and the transportation vehicle during my visit today for approval for transportation. There was one child present today and Ms. Sparrow stated that the child present is the only child enrolled at this time. Records: Provider, children’s and program records were monitored during today's visit. Arrival and departure times for the children were observed documented on a clipboard at the entrance to the FCCH. I also monitored the required posted documents for compliance. CPR and First Aid were current and on file. A current CBC Qualifying letter was on file for Ms. Sparrow. Ms. Sparrow stated there are no other household members. There are no infants enrolled at the FCCH at this time but Ms. Sparrow stated that she has been contacted by a potential client who is requesting care for an Infant. We discussed that your SIDS certification expired August 2025 and it will need to be renewed prior to caring for an Infant. Please send me a copy of the completion certificate once you have completed the class. Ms. Sparrow completed her Health and Safety trainings 10-12-24. Ongoing training was monitored and we discussed that during your last annual compliance visit which was conducted on 9-17-24, you needed 5.5 more hours for the 2023-2024 year. Those hours were not completed as verified today. You also have not completed any ongoing hours for the 2024-2025 year. You will have two weeks to complete your required ongoing hours for the 2024-2025 year. I encourage you to go into Moodle on the DCDEE website to take as many of the free trainings that are offered. Emergency telephone numbers were readily accessible and emergency procedures were posted. A written daily schedule was posted. The daily schedule reflected outside time. Fire drills, EPR drills and outdoor playground inspections were monitored and observed to be in compliance. The last fire drill was conducted 1125-25. The last EPR drill was conducted on 8-11-25. Your EPR plan was revised 10-18-24 and will need to be revised again no later than 10-18-25. The last outdoor playground inspection was 8-25-25. The FCCH operates on city water. Ms. Sparrow is waiting on test results for Lead Paint and Asbestos testing. Lead water testing occurred on 4-25-25. Ms. Sparrow has been communicating with DCDEE regarding issues pertaining to getting her FCCH account established in ABCMS. Child Care Space: The main area that the children use is a room on the front of the home. For outside play, the children use fenced back yard as well as a screened porch on the side of the home. Meals and snacks are eaten in the Kitchen. No safety concerns of the indoor or outdoor play areas were observed during my visit. Ms. Sparrow stated there are no pets in the home. The smoke detector and fire extinguisher were observed to be in working order during my visit today. Transportation requirements were monitored for approval of transportation. Ms. Sparrow drives a 2017 Ford Escape SUV with license plate 4HZG1RY! Insurance information was verified as current and on file. We discussed today that permission to transport needs to be updated. Transportation with any children enrolled in the FCCH cannot occur until this permission is on file. You will also need to make a “To Go” bag with identifying information for the child as well as a copy of your EPR plan. I did observe a booster seat for the child present in the car. No safety hazards were observed with the transportation vehicle. *****Violations****** Three violations regarding emergency information and recordkeeping and on-going/inservice training were observed today. A corrective action letter stating how each violation was corrected is due to me no later than September 17, 2025 and can be sent to me either via e-mail or paper mail to the address at the bottom of this report. Copies of training certificates must be submitted for proof of completion of on going training hours. Ms. Sparrow transported a child on 9-3-25 without written permission from the parent/guardian. .1723(5) 1301 Operator did not complete the required number of on-going training hours as specified in rule. Ms. Sparrow did not complete any hours for the 2024-2025 year as well as did not complete all training hours required for the 2023-2024 year. Ms. Sparrow did not take the required information for identification for the child who was transported on 9-3-25. .1723(15)(c ) Technical Assistance: Motor vehicle crashes are the leading cause of death in children two to fourteen years of age in the United States. It is necessary for the safety of children to require that the caregiver/teacher comply with requirements governing the transportation of children in care, in the absence of the parent/guardian. Each child transported must have emergency contact information on file along with a photograph of each child. This is helpful in identifying children in the event of an accident. *Other Information: *We also discussed free training which is offered on the MOODLE platform on the DCDEE website.
