Name of Operation: WEST VIEW ELEMENTARY Facility ID: 51000629 Consultant: PAULA DAVIS Operation Type: Center Case Number: Visit Date: 4/24/2025 Number Present: 31 Completed Date: 4/24/2025 Age: From 3 To 5 Total Minutes: 120 Time In: 09:45 AM Time Out: 11:45 AM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor compliance with applicable child care requirements during a routine unannounced visit. I monitored the indoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation placard, evacuation plan, safe arrival and departure procedures, first aid chart, menus, daily schedules, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of today's visit. The storage of medication and hazardous products was also monitored for compliance. No new staff member were hired since the 9-26-24 annual compliance visit. Therefore, no staff files were reviewed during the visit. A fire drill was documented as completed on 3-25-25 at 10:20 am with an evacuation time of forty eight seconds. A shelter-in-place drill was documented as completed on 2-24-25 at 2:15 pm. An outdoor inspection was documented as completed on 3-24-25. An approved fire inspection was completed on 9-17-24. A sanitation inspection was last completed on 2-14-25 with zero demerits and a superior classification. The children were observed eating lunch and preparing for nap and napping during the visit. For lunch the children had beef tacos with a salad with lettuce, tomatos, cheese and ranch dressing and guacamole and milk. A five star child care center license was issued to you on 11-14-19 with seven points in education, five points in program and one quality point. Based on information provided and on observations made, you continue to meet the requirements previously met to obtain the license. During the visit, we discussed that the hold harmless status for rated license has now been extended until our new rated license rules are implemented. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. An outlet in space 501 was not being used and was not covered. 10A NCAC 09 .0604(c) 1757 A valid qualification letter was not on file and available to review at the facility. An individual went into the classroom who did not have a current qualification letter. G.S. 110-90.2(b) & (d) & .2703(e) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. There were two cups of iced coffee in space 501 and two bottles of flavored water in space 122. .0901(i) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Two staff members did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 30 days of hire as required. .1102(g) Today an individual went into space 501 to assist and maintain staff/child ratios. The individual did not have a current qualification letter. The individual only stayed in the classroom for a short period of time and then the assistant returned to the classroom. ****Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules.**** Please send me a written statement describing how and when the above violations were corrected. The written statement must be received no later than 5-7-25. Please note that if your letter is mailed, a hand written signature is required. As an alternative, compliance letters can now be submitted via email. I recommend emailing your letter to reduce the time needed to ensure my receipt of your information; however, if you do not have access to email, please ensure that your letter is mailed in a timely fashion and that you allow for any possible delays in the mail service.
