    Name of Operation: Victory Promise Learning Center & Childcare Facility ID: 92004063 Consultant: LEANNE SIMPKINS Operation Type: Center Case Number: Visit Date: 2/6/2026 Number Present: 14 Completed Date: 2/6/2026 Age: From 0 To 4 Total Minutes: 255 Time In: 10:00 AM Time Out: 02:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to complete the second of three monitoring visits during the first six months of operation and to discuss the QRIS rated license information. Today’s visit was completed with the director, Shenetta McGill. Prior to visiting classrooms, we reviewed the Classroom and Instructional Quality pathway 2. Education is the starting point. I confirmed the director’s information in WORKS. She needed to click on the administrator position to be evaluated. She attempted to go into WORKS during the visit and she received an error message so she was unable to complete this step. I will verify staff education for the other staff after today’s visit. I will email you if other steps need to be completed. We reviewed the Family and Community engagement options and the CQI for the facility and individuals and I reviewed the application for rated license. The plan is to start with pathway 2 and then later transition to Program Assessment pathway one. We discussed using preparation for pathway one for the CQI for the facility. The CQI for individuals could include preparing for the ITERS-3 and ECERS-3 assessments. We visited the NCRLAP website to review trainings that can be used to meet the individual CQI. The teachers use the Experience Early Learning curriculum that is approved to use with Mother Goose. I monitored 4 classrooms today for staff/child ratios, group sizes, capacity, supervision, interactions, discipline, materials, activities, postings, and health and safety. Only one child had a topical product that was used during diaper changes. Permission to administer the product was current and complete. One child was stated to be allergic to bananas, fish and peanuts but the information was only documented by the parent, not the doctor. The parent completed a medical action plan, however, the child does not have emergency medications for this. The classroom for infants opened last week. Three infants under age one were present with two teachers. Three bottles were labeled with a child’s name but did not include the date. Be sure parents date the bottles before they leave during arrival time. Since two children are drinking breastmilk, there should be two bottle warmers, one for formula and one for breastmilk. You can use warm water to warm the bottles that contain breastmilk until you have the second bottle warmer. Be sure that the bottle warmer and the cord to the bottle warmer is not within children’s reach when washing hands or changing diapers. Move the refrigerator down to the corner of the room where the evacuation crib was today. Move the evacuation crib closer to the door. Feeding schedules were stated to be on the Brightwheel app. The feeding schedules must be posted or available in the classroom. You will print the feeding schedules to place in a notebook or post in the classroom. Safe sleep checks were completed as required. The safe sleep policy and safe sleep poster were posted in the classroom. A mother breastfed their child in the classroom. This is acceptable but you can also create another place if the mother is not comfortable in the classroom. Tummy time is provided for the infants under age one. A video was shared showing two of the youngest infants facing each other as they had tummy time. Children were observed having free play exploring materials. Teachers were interacting with the children as they played and the teachers asked open ended questions to encourage language. The teachers interacted with the children in a positive manner. A teacher was observed reading a book to the children that included sign language. The children seemed to enjoy this time. No concerns with the outdoor play area during today’s visit. The sand box cover had blown to the corner so it will need to be placed over the sandbox at the end of the day. Outdoor inspections occurred as required. Fire drills were conducted as required. Seven children's records were reviewed during today's visit. A children's record checklist was used to confirm applicable information was on file. Eight staff files were reviewed today. All files contained the required information. No incidents have occurred that needed to be documented on an incident report. Space 105 was turned into a sick child room and storage. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Bottles for one child did not have the date. Bottles for another child did not have the date or the name of the child. 15A NCAC 18A .2804(d) 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence)Written feeding schedules for 3 children under 15 months old were not in the classroom. The schedules were on Brightwheel. 10A NCAC 09 .0902(a) The violations cited during today’s visit must be corrected immediately or as soon as possible. Email me a corrective action letter that addresses the violations, how the violation was corrected and how you plan to prevent repeat violations in the future.
