    Name of Operation: The Caring Nest Family Childcare Facility ID: 92004090 Consultant: LEANNE SIMPKINS Operation Type: Family CC Home Case Number: Visit Date: 7/8/2026 Number Present: 3 Completed Date: 7/8/2026 Age: From 2 To 2 Total Minutes: 95 Time In: 10:25 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to complete the first monitoring visit since the temporary license was issued June 1, 2026. The required postings were observed. Attendance was current and included arrival and departure times for each child. Three unrelated children were present today. One related school age child was also present. The provider was consoling a child that had stranger anxiety, assisting children using the restroom and handwashing. The other children had free play while other routine tasks were occurring. They also worked on an art project during the visit. Two children’s files were reviewed today. Two of the files were missing the medical, the children were enrolled June 1, 2026, so the medical should have been on file July 1, 2026. The medical is due within 30 days and the immunizations are due on the first day of enrollment. If parents have a medical from within the past year, that information can be provided. One other child did not have a file available but the child lives next door so the parent brought over the information. You should never have any children present that don’t have all required paperwork on file. Create a way to ensure all paperwork is on file the first day of attendance so you don’t have to worry about missing anything. Using the children’s file checklist for each file will be helpful as well. Document the date information was received so you know it is on file. Fire drills: 6/22/26 Outdoor inspections: 6/22/26 Incident log: No incidents have occurred since opening. Medication: No medication at this time. Lead water testing, lead paint testing and asbestos testing were completed prior to licensure. Capacity options: The facility is limited to 3 children. You can meet the capacity of option of having 3 children that are all different ages. (1-5 years old) Or you can meet the option of having 3 one year old children. Once the children turn two years old, you would be meeting the option of having 3 preschool age children. Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. Two unrelated children did not have a medical on file within 30 days of enrollment. GS 110-91(1); 10A NCAC 09.1721(a)(1) The violation cited today must be corrected immediately. Email me a corrective action letter stating when the violation was corrected, how the violation was corrected and how you plan to prevent repeat violations in the future.
