    Name of Operation: KRYSTAL'S QUALITY CARE Facility ID: 92003296 Consultant: KIMBERLY WIMBERLY Operation Type: Family CC Home Case Number: Visit Date: 4/10/2025 Number Present: 4 Completed Date: 4/10/2025 Age: From 0 To 3 Total Minutes: 120 Time In: 09:45 AM Time Out: 11:45 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor the FCCH for NC child care requirements during your annual compliance visit. The FCCH operates with a 4 STAR rated license. You are licensed to care for 8 children ages 0-12 and of these 8 children, only 5 children can be preschool aged. Today's visit was conducted with Krystal Brown, owner/operator. Upon arrival to the FCCH today, the children were observed playing with various toys in the child care area. Ms. Brown was observed feeding the infant that is enrolled and was present. Caregiver interactions were observed to be warm and nurturing. Records: Provider and program records were monitored for compliance. Arrival and departure times for the children were observed documented on a clipboard at the entrance to the FCCH. I also monitored the required posted documents for compliance. CPR, First Aid and SIDS were all current and on file. I observed that Ms. Brown and her husband did not renew their criminal background check qualifications letters that expired 1-2-25. Ms. Brown stated that she thought they expired 2026 and would get working on the appointment today. For future purposes I suggest to put sticky notes up near the desk where the child care paperwork is stored as an easy reference to stay on top of expiration dates. Ms. Brown completed her Health and Safety trainings on 5-2-24. Ongoing training requirements were monitored and Ms. Brown will carry forward 2.0 hours to the 2025-2026 year. Emergency telephone numbers were readily accessible and emergency procedures were posted. A written daily schedule was posted. The daily schedule reflected outside time. Sleeping baby checks for all infants ages 0-12 months old were monitored and observed to be in compliance. Fire drills, EPR drills and outdoor playground inspections were monitored and observed to be in compliance. The last fire drill was conducted 3-18-25. The last EPR drill was conducted on 1-17-25 and a new one must be conducted this month in order to remain in compliance. The EPR plan was revised 4-30-24 and will need to be revised again no later than 4-30-25. The last outdoor playground inspection was 4-8-25. The FCCH operates on city water. Negative lead test water results were on file at the FCCH. Child Care Space: The main area that the children use is the lower level of the split-level floor plan home. There is an exit at grade. For outside play, the children use fenced back yard area. There is an above ground swimming pool in the back yard but it is in a separate fenced area not accessible to the children. Meals and snacks are eaten in the child care area. No safety concerns of the indoor or outdoor play areas were observed during my visit. The smoke detector and fire extinguisher were observed to be in working order during my visit today. Transportation requirements were not monitored due to the fact that Ms. Brown does not provide transportation. There are two dogs that are in the home and I observed vaccination records current and on file. Ms. Brown stated that she does not allow medications except on an emergency basis and there are no children on emergency meds at this time. *****Violations****** One violation of child care requirements pertaining to criminal background checks was observed out of compliance and must be corrected immediately. A correction letter stating that the violation has been corrected is due no later than 4-24-25. Once you have submitted all the required information to the CBC Unit, please let me know so that I can track it. Please also let me know if there are issues with getting an appointment for fingerprints. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The qualification letters on file for Ms. Brown as well as her husband expired 1-2-25.
