    Name of Operation: Childcare Network #236 Facility ID: 92003971 Consultant: KIMBERLY WIMBERLY Operation Type: Center Case Number: Visit Date: 4/16/2025 Number Present: 127 Completed Date: 4/16/2025 Age: From 0 To 5 Total Minutes: 165 Time In: 10:00 AM Time Out: 12:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor child care requirements during a routine unannounced visit. Today’s visit was conducted with Mauricia Montague, Administrator. I reviewed the NC Secretary of State’s website and the owner of the facility, Child Development Schools North Carolina, LLC is listed as current-active as of 4-16-25. Program requirements The following items were observe posted: first aid poster, license, no smoking sign, summary of the law and emergency care plan. The indoor environment was monitored for safety and no items of non compliance were observed. Daily arrival and departure times of the children are documented on an electronic program as the children enter the center. All space used by the children were adequately heated and cooled. An incident log is stored in the Director’s office. The last fire inspection conducted was 2-5-25. The last Sanitation inspection was conducted on 11-24-24. The last documented outdoor playground inspection was April 2024. Fire drills and EPR drills were current and on file. The last fire drill was conducted on 4-14-25 and the last EPR drill was conducted on 3-13-25. Staff Records: I monitored five new staff files who were hired since the last annual visit during my visit today. I observed that two staff members did not have current CPR or First Aid. Ms. Montague stated that both staff are enrolled for the next CPR/First Aid class scheduled for 4-29-25. I also monitored criminal background check qualifying letters for all staff and I did observe that one staff member had not renewed their criminal background check. You will have two weeks to get a new criminal background check on file for the staff member and if at the end of 14 days, a new one is not on file, the staff member will not be able to return to work until a new one is on file. Staff/Child Ratios and Supervision: During today's visit I observed the children playing in centers and some were having circle time. Other classrooms were outside playing on the playground. The Infants were being changed and having tummy time. One infant was observed sleeping on their back in a crib. Sleeping baby checks were monitored and observed in compliance for all infants ages 0-12 months. Supervision was observed being maintained. Staff/child ratios were also in compliance and space capacity did not exceed that which was allowed. Medications and Hazardous Products: All hazardous products were placed out of reach of the children. Ms. Montague stated they only allow emergency medications and those medications are kept out of reach of the children in the classrooms. Three items of non compliance regarding staff records pertaining to CPR and First Aid and criminal background checks were observed and must be corrected. A correction letter stating how each violation was corrected is due to me no later than April 30, 2025. The letter can be mailed or e-mailed to me at the address at the bottom of this report. Please also send a copy of the completion certificates with the correction letter. Please let me know once the staff member has submitted all the paperwork for the criminal background check so that I can track it. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One staff member did not have a current criminal background letter on file. G.S. 110-90.2(b) & .2703(n)&(o) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two newly hired staff members did not have a current First Aid certification on file. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Two newly hired staff members did not have current CPR certification on file. .1102(d) REMINDERS: *There is a Director Meeting scheduled on May 6, 2025 at 10:00am via Zoom. Invitations were mailed via e-mail so please don’t forget to attend! A link for the Zoom meeting will be e-mailed to you on May 5, 2025 so please be on the lookout. We will have someone from the CBC Unit to provide assistance with connecting your staff to your center through the ABCMS Portal. *Criminal background checks (CBC) are completed to protect the children from abuse and reduce liability risks. A CBC must be completed every five years. An expiration date is listed on each Qualification Letter. Submissions can be made six months before the expiration date. Operators will need to be proactive and make staffing decisions and arrangements in advance to avoid non-compliance with this and other DCDEE staffing requirements. Criminal background checks must be completed online. To complete online criminal background check applications, go to www.ncchildcare.nc.gov/general/dhhscrc.asp. The $26.50 payment is a part of the online criminal background check application process. Fingerprints must be completed through LIVESCAN at your local county sheriff’s office. I Mauricia Montague, director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Debbie Wiggins, lead director, and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On September 26, 2024, a staff member took a four-year-old child by their forearm to guide them to a different area of the classroom. The child fell to the floor and the staff member did not release the child’s forearm and pulled the child from the floor to a standing position.
