Today’s visit was conducted with Brittany Dixon and Kenya Stewart, Lead Teachers. The PreK program is housed within Aversboro Elementary School. The Administrator of the program is Katie Lewis who operates as an off-site Administrator. There are two classrooms approved for child care. Program requirements The following items were observe posted: License, Safe Arrival and Departure Procedures, NC Summary of the Law, Emergency Medical Care Plan, Daily Schedule, Activity Plan, Staff/child ratios, Allergies, First Aid chart, Fire evacuation route, hand washing signs, and cot chart. Arrival and departure times are documented on clipboards in each classroom. The most recent menu which was posted in classroom #313 was dated January 2025. This is a violation of child care requirements pertaining to nutrition. All space used by the children were adequately heated and cooled. An incident log and incident reports are stored in the teacher’s office in a notebook During my visit today, I observed classroom #309 having circle time and discussing the special tie dye project they are doing tomorrow and what supplies each child needs to being to school. The children also had made airplanes which they were taking outside to fly. After circle time, the children transitioned outside. In classroom #313, I observed the children playing in centers. Later during my visit, the children cleaned up and transitioned to circle time and participated in a gross motor activity. I did observe during clean up from centers, the Instructional Assistant (IA) left the classroom for a brief time estimated to be about five minutes. The Lead Teacher was unaware of where the IA was and was unaware that the staff member had left the classroom. The IA arrived back to the classroom with two children, one which had just arrived to the school. Due to the fact that there were six children who were four years old and six children who were five years old totaling twelve children in a classroom with one staff member, this is a violation of child care requirements pertaining to NC PreK staff/child ratios. A staff-to-child ratio is a measure of the number of children for whom each childcare provider is responsible. Because younger children need more direct one-on-one interaction, response, and supervision, staff-to-child ratios are lower for younger children than for older ones. Ratio and group size are two factors that are critical to a child’s health, safety, and development. Ratios and group sizes help ensure that a child gets enough one-on-one attention from an adult who is available to take care of each child’s unique needs. This responsive caregiving is extremely important to a child’s social and emotional development, physical well-being, and overall learning. Criminal background qualification letters for all staff were observed current and on file. Program records were monitored for compliance. The last fire drill was conducted on 2-26-25 and the last EPR drill was conducted on 3-7-25. The most recent outdoor playground inspection on file was October 3, 2024. It is very important to document the monthly playground inspections as it serves as proof that the required inspections are being completed. I did not see a current copy of the most recent fire inspection on file. Please obtain it from the front office and e-mail to me. The last Sanitation inspection was conducted on 2-5-25 and a Superior rating was awarded with 3 demerits. Three violations of child care requirements pertaining to NC PreK staff/child ratios, program records, and nutrition were observed out of compliance today and must be corrected. A corrective action letter stating how each violation was corrected is due no later than April 10, 2025. I will also be conducting a follow-up visit to monitor staff-child ratios. Violation Number Comment Rule 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. A current menu was not posted in classroom # 313. The most recent menu posted was dated January 2025. 10A NCAC 09 .0901(b) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last documented outdoor playground inspection was 10-4-24. .0605(q) 1775 NC Pre-K program staff/child ratios and group sizes were not met. A staff member was observed leaving classroom #313 for an estimated period of five minutes leaving one staff member with six-four year old children and six-five year old children. .3009 Technical Assistance: *Today we discussed that there have been a lot of behavior issues with the children this school year. I observed three incidents of behavior outburst during my visit in which staff were taken away from activities with the children to diffuse the behaviors of the children acting out. The visit was conducted with Brittany Dixon and Kenya Stewart, Lead Teachers. The Administrator, Tiffany Stuart, was also observed doing observation in the classroom today. CLASSROOM OBSERVATIONS Upon my arrival, the children were observed playing in centers then transitioning to the outside playground. There are 2 classrooms approved for care. I observed the 23 children present. The children were cared for in the approved space with adequate supervision. Ms. Dixon and Ms. Stewart both stated they do not have any children on medications at this time ATTENDANCE Attendance was monitored and found to be in compliance. POSTINGS The license, menu, and allergy information was posted and found to be in compliance. The lesson plans were posted and found to be in compliance. Staff/Child ratios were posted and the classrooms were found to be in compliance with ratios. STAFF RECORDS Staff and training worksheets were completed by me during my visit. Staff records were reviewed and it was observed that staff files were missing numerous items required to be on site. Both staff stated that they have not had a chance to review staff files. Three staff did not have current Annual Health Questionnaires or Emergency Information on file. Two staff did not have current CPR and First Aid. Both of these staff did sign up for CPR and First Aid during my visit. Three staff did not have proof of review of the Shaken Baby Head Trauma Policy on file. None of the staff working with the NC PreK classroom had proof of review of the EPR/Emergency Risk Plan within last year on file. None of the staff had proof of annual review of Emergency Medical Care Plan on file. We discussed that Staff Files should be updated prior to the beginning of the new school year. Criminal Record letters were current and on file for all staff. Staff work 40 hours per week and are provided time to plan lessons and attend staff development opportunities. CHILDREN RECORDS Sixteen children’s files were monitored and were observed to be missing numerous pieces of required information. 0 of the 16 children’s files monitored had receipt of discipline policy, operational policies, summary of the law, notification of smoking and tobacco restriction, or prevention of Shaken Baby and Abusive Head trauma policy. None of the 16 children's files I reviewed had documented proof that the Abusive Head Trauma Policy was reviewed. .0608(b) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file.
