A checklist was used to note the requirements I monitored today. During the visit the Administrator Ms. C. Fisher accompanied me on the walk- through of the facility and assisted with the visit. Prior to today’s visit I viewed your corporation status as current-active on the Secretary of State website. This program currently operates with a four-star license issued March 12, 2024. I reminded you to make sure all staff’s education has been submitted to WORKS to be evaluated. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, summary of NC Child Care Law, were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. The staff and children’s records were also reviewed today. Files for new staff were monitored with a random selection of returning staff members. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. A random selection of children’s files was reviewed, and the children’s record worksheet was completed. The lesson plans were on fire prevention week. The children were observed playing in activity areas and later outside on the playground. The lunch menu met the meal pattern requirements. Your last fire drill was completed 9/11/25 and your last monthly outdoor playground inspection was completed 9/22/25. Your last EPR drill was a shelter-in-place completed 9/25/25. I reminded you that an EPR drill must be completed every three months and keep all paperwork on file for review. The fire inspection was dated 3/7/25. Your last sanitation inspection was completed 6/23/25 with a superior rating. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed. Once your inspections have been completed, mail or email them to me. I recommend that you place your due dates on a calendar or spreadsheet to help you maintain compliance. The following violations were documented during today’s visit. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months.Two staff members did not have a medical report on file prior to employment that was signed by a health care professional. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. One staff member did not have negative TB test on file prior to employment. .0701(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two staff members did not complete first aid training within ninety days of employment as required by the rule. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Two staff members did not complete CPR training within ninety days of employment as required by the rule. .1102(d) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. One staff member did not sign the policy prior to caring for children. .0608(d)(1-4) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member did not complete the training within ninety days of employment as required by the rule. .1102(g) The violations must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By October 21, 2025, please send me a letter addressing the violations, how the violations were corrected, and how compliance will be maintained in the future. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. Technical assistance regarding the following items were reviewed with you today. I reminded you the health and safety trainings have been relocated from Pro-Solutions to Moodle on the DCDEE website and they are free of charge. I discussed various other training resources, including those online and in-person. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded the health and safety training courses are required to be completed every five years. During today’s visit one staff member did not complete the health and safety training within the five year renewal timeline to meet compliance with child care requirements. During today’s visit I explained you can renew a criminal background check up to six months ahead of time. I recommend that you place your due dates on a calendar or spreadsheet to help you maintain compliance. Please be reminded prior to employment a staff member must have a completed medical assessment and a negative TB test less than one year old. Today I reviewed with you if new staff do not have a valid CPR /First aid they must be completed within ninety days of employment. I reminded you to begin thinking of which pathway you might choose and to start working towards the requirements for the pathway. During today’s visit I completed a walk-through of the facility with Ms. Fisher/Administrator who also assisted with today’s visit. The group size, supervision, ratio, capacity and approved space were in compliance. The restrictions, license and emergency care plan were posted. First Aid/CPR, storage of hazardous materials and the NC Child Care Summary of the Law were posted. I monitored the program requirements, furnishings and equipment, outdoor area, and medications. The infants and toddlers were observed playing with toys with one staff member. The preschool children were observed in morning gouptime and later playing outside on the playground. The theme of the lesson plans was birds. The lunch menu consisted of ravioli, sliced apples, pinto beans, bread, and milk. During the visit you stated you had hired a new employee 4/22/25 since my last visit. The staff and training worksheet was completed for the new employee. The program currently operates with a four-star license issued March 12, 2024. During the visit I reminded you to make sure all staff have sent their education to WORKS to be evaluated. The hold harmless has been extended until further notice at this time. Your last fire drill and your last monthly outdoor playground inspections were completed 4/3/25. . Your last EPR drill was a shelter-in-place completed 3/20/25. I reminded you that an EPR drill must be completed every three months. Please keep all documentation on file for review. Your last fire inspection was completed 3/7/25 and a violation was documented. The following violations were documented during today's visit. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection was completed 3/7/25 and the previous fire inspection was dated 1/24/24. 10A NCAC 09 .0304(a) 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Two infant bottles were not labeled or dated. 15A NCAC 18A .2804(d) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. A checklist was used to note the requirements I monitored today. Today Ms. Fisher Owner/Administrator accompanied me on the walk through and assisted with today’s visit. Prior to the visit I reviewed your corporation status as current-active on the Secretary of State website. This program currently operates with a four-star license issued March 12, 2024. Please be reminded to have all staff’s education sent to WORKS to be evaluated. The hold harmless has been extended until further notice. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, summary of NC Child Care Law, were posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. The staff and children records were also reviewed today. Files for the staff were monitored. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. A random selection of children’s files was reviewed, and the children’s record worksheet was completed. The infants were observed on cushioned area with a staff member. The toddlers were observed during morning group time. The preschool children were observed playing outside on the playground. The school- aged children were not present at the time of the visit. The theme in the lesson plans was fall activities. The lunch menu for today consisted of fish sticks, bread, corn, sliced bananas, and milk. Your last fire drill was completed 10/9/24, and your monthly playground inspection was dated 10/13/24. Your last EPR drill was a shelter-in-place dated 9/27/24. I reminded you an EPR drill must be completed every three months and to keep all documentation on file for review. Your last fire inspection was dated 1/24/24. Your last sanitation inspection was completed 11/20/23. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed. Once your inspections have been completed mail or email them to me. I recommend that you place your due dates on a calendar or spreadsheet to help you maintain compliance. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. Technical assistance regarding the following items were reviewed with you today. All flexibilities expired, except for the flexibilities related to the star rated license. I reminded you the health and safety trainings have been relocated from Pro-Solutions to Moodle on the DCDEE website and they are free of charge. I discussed various other training resources, including those online and in-person. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded the health and safety training courses are required to be completed every five years. Today we reviewed the forms that need to be updated yearly, and you stated you would usually have the staff update the yearly forms at the beginning of the new year. I suggest checking on qualifying letters for the expiration date. During the visit I reminded you to make sure all sippy cups or bottles are to be labeled and dated daily upon arrival at the facility. We also reviewed incident reports and when you are to sent me a copy within seven days if a child seeks medical attention. DHHS Memo on Funding and Enrollment Public schools and licensed child care facilities in North Carolina are eligible to participate in the funding mechanisms to cover the costs of testing for lead in water and inspecting for asbestos and lead-based paint hazards beyond the May1, 2024 deadline. Facilities that have not enrolled should complete the online process by November 1, 2024. The Clean Classrooms for Carolina Kids Program is providing free facility-wide testing and inspections without any out-of-pocket costs. Due to the volume of testing and inspections, completion of the online enrollment process in the program is considered temporary compliance with the testing and inspection requirement and will allow facilities to participate in the funding mechanisms. Required water testing for child care centers under Rule 15A NCAC 18A .2816 is not affected by the November 1 deadline. During the visit you stated you would check on this. Annual Licensing Fee: Pursuant to GS 110-90(1a) the Division establishes an annual fee for licensed child care facilities. The invoices for the fees are assessed based on your licensed capacity and mailed after the 1st of November every year. You will only receive one invoice, and payment is due in FULL within 30 days. Please closely monitor your mail and be on the lookout for your invoice. Please don’t delay making your payment and be aware that Payment Plans are NOT available. In addition, please be aware that failure to pay a license fee within 30 days of receipt will result in late charges. If payments are made late and late charges are applied, the fee as well as the late charge must be resolved. Please be reminded that failure to pay a license fee could also result in the facility's child care license being revoked.
