This documentation was reviewed with you, but I was having internet issues and was unable to type my report. I will email the report to you by the end of today. In addition to Ashley, her husband, John and her mother, who is visiting, were also present in the home during today’s visit. Upon arrival, I was greeted by the center operator, Ashley Gage, and I conducted a general walk through of the indoor and outdoor learning environments. When I arrived RATED LICENSE STATUS The program operates with a 4-star rated license issued on 6/21/19. They received 2 points in program standards, 7 points in staff education and one quality point through the programmatic option limiting infant capacity to no more than 2 infants under one year of age. The program’s “held harmless status” has been extended until the new QRIS is implemented. This means that rated license visits will not be processed until the new rated license process is completed unless the program requests to proceed with a new rated license. INSPECTIONS The last documented fire drill was conducted 10/1/24. The last documented lock down or shelter-in-place drill was conducted 11/5/24. The last documented playground inspection was conducted 11/4/24. MONITORING During this visit a full assessment of Child Care Requirements was conducted. The facility was assessed using items noted on the Annual Compliance for Centers checklist. The program is privately owned, so I did not conduct a search of the NC Secretary of State’s website. Staff on-going training was assessed from 12/1/23 – 11/30/24. You will need to complete 2 more training hours by the end of this month, to meet licensing requirements. The following violation was observed and recorded during today’s visit Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. One child's health assessment was not in their file. GS 110-91(1); 10A NCAC 09.1721(a)(1) The above violation must be corrected immediately. By 11/27/24, please email me a signed letter of compliance. Before the letter is sent, all violations should be corrected; please contact me if additional time is needed to correct a specific violation. Include the following in your letter: facility ID number, list the item number describing how and when the violations were corrected and tell me how you plan to maintain compliance in the future with each item. If you prefer to mail your letter, you may send it my attention at 2201 Mail Service Center Raleigh, NC 27699-2201 If the compliance action letter is not received by the due date another visit to the center may be made. COMPLIANCE HISTORY Prior to today’s visit, the compliance history score for the center was 97%. TECHNICAL ASSISTANCE The medical report was missing from the file for one child, who was enrolled in July 2023. Student health assessments and immunization records should be in the child’s file within their first 30 days of enrollment. This document may provide vital information about the child’s medical history in the event of an emergency. You stated that you would give the form to the parent when they picked the child up today. . CONSULTATION Child Care facilities are required to test their drinking and cooking water for lead every three years. You stated that you had completed the testing in the summer of 2024, but there were no records in your file. Please email documentation to me confirming the date the water testing was completed. For information you should contact Clean Water for Carolina Kids. Contact www.cleanwaterforcarolinakids.org for more information. In addition, child care facilities are now required to test for lead and asbestos hazards in their buildings. The Clean Classrooms for Carolina Kids program is designed to identify and eliminate exposure to lead and asbestos hazards in building infrastructure, while providing child care facilities with the support needed to meet all rule requirements. The program is free and the first step is to watch one of the pre-enrollment webinars, which are available at www.cleanwaterforUSkids.org/carolina or paste https://rtiorg.zoom.us/webinar/register/WN_vmlhG6iRS26BdkV8SZSaDw#/registration into your web browser. Additional information can be found in the RaiseNC newsletter sent on December 7, 2023. You stated that you had completed this information and your home was exempt for testing, but there was no records in your file. Please email documentation to me confirming this information. Training in all the required health and safety topics must be completed at least every 5 years. Your training was last completed in 9/28/22. Health and Safety training is available, at no charge, on the DCDEE website, in Moodle. Medication Administration is also a required health and safety training topic. This training is available through Moodle as a separate training. Staff should use their individual NCID user name and password to access this training. Today's visit was conducted with Ashley Gage, Owner. Children were observed participating in routine activities in their rooms and playing outside. A partial assessment of applicable childcare requirements was conducted. The last annual compliance visit was conducted on 11/27/23. The operation currently has a 4 star permit which was issued on 6/21/19. The background check for the primary caregiver expired on 4/16/24. Background check for Ms. Gage's suse expires June 2024. CPR expires 10/31/25. ITS-SIDS is currently expired but the course was stated to have been completed but a certificate was not received. No children under 1 year old are currently enrolled nor planned to be enrolled. Ms. Gage stated she will reach out again to the trainer to try to obtain the certificate. A fire drill was last conducted 5/6/24. The last shelter-in-place drill was conducted 3/10/24. A lockdown drill was completed 4/15/24. No enrolled children are currently on any medications. All required postings were observed. Today I observed 1 violation regarding childcare requirements listed below and these violations were discussed with you in detail during the visit. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The background check for the primary caregiver expired 4/16/24 and has not been renewed. G.S. 110-90.2 & .2703(n) & (o) The violations documented above must be corrected immediately. A signed and dated letter of compliance must be mailed by June 6, 2024 stating that all violations have been corrected. The letter can be emailed to scott.moore@dhhs.nc.gov or mailed to: Scott Moore 5 Seawell Ct Durham, NC 27703 If you state in the compliance letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by June 6, 2024, you must contact me with a proposed timeline of the corrections. In some cases the timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today's visit may negatively impact your compliance score.
