The director, Joann Feyen, assisted me with today’s visit. We conducted a walk-through of the indoor and outdoor learning environments. I observed groups of children engaged in free play in their classrooms or outdoors and one group eating lunch. RATED LICENSE STATUS The program operates with a 1-star rated license issued on 2/3/22. INSPECTIONS The last sanitation inspection was conducted 11/6/25. The last fire inspection was conducted on 9/6/25. The last documented fire drill was conducted on 12/22/25. The last documented shelter-in-place or lockdown drill was conducted on 12/23/25. The last documented playground inspection was conducted on 1/5/26. We also discussed the Pathway to the Stars. The center is currently licensed with one star and the director stated that they do not intend to pursue a star-rated license. The status of the owners, Geefour Enterprises, LLC., is listed as current and active on the NC Secretary of State website. Limited monitoring of childcare requirements was assessed during today’s visit. I reviewed emergency medications, 2 new staff files and confirmed that all staff have current CPR, First Aid, and ITS SIDS certificates, as required. The program does not transport children or enroll school-aged children; therefore, these requirements were not assessed. The following violation was observed and recorded during today’s visit. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One staff member's qualification expired 8/12/25 and was not renewed until 12/9/25. G.S. 110-90.2(b) & .2703(n)&(o) The violation is marked as corrected during the visit, so a compliance letter is not required. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 90%. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. TECHNICAL ASSISTANCE Confirming that center staff meet and maintain the basic requirement of successfully completing a background check and receiving a qualification letter ensures that all staff meet the minimum requirement of being able to work in a child care facility. It ensures that the individual has no pending or previous charges that may disqualify some from working with children in a licensed program. One staff member has a current letter that was renewed on 12/9/25, but their previous qualification letter expired 8/12/25. Staff should complete the renewal process prior to the expiration of their qualification letter. The renewal process can be completed up to 6 months prior to its expiration date. CONSULTATION The owners’ qualification letters expires in March and April 2026. All childcare facilities are required to test their drinking and cooking water for led every three years. Your water was last tested, 5/7/24. For more information, you should contact Clean Water for Carolina Kids. Contact www.cleanwaterforcarolinakids.org for more information. In addition, childcare facilities are now required to test for lead and asbestos hazards in their buildings. This center is determined to be exempt from testing for lead-based paint and asbestos. ACCESS TO THE ABCMS SYSTEM: Center directors can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: See the real-time background check status of staff members. Run a printable report of the staff roster to assist with compliance visits. See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. To access your staff criminal records, they must have your program listed as their place of employment in ABCMS. This requirement is indicated as Item #7 on the staff and training worksheet and replaces the Data Change form that was previously required to be submitted to the Criminal Records Unit. It is recommended that staff add your program as their place of employment during the first few days of orientation. This change should be documented and placed in their file. The documentation could be a screenshot after the change is made or documented on the Orientation Form in the Other section. The date the change was made should be indicated in Item #7 on the staff and training worksheet during the staff member’s first year of employment. Two medication authorization forms were expired today. 10A NCAC 09 .0803(1)(d) The above violations must be corrected immediately. By 2/24/25 please email me a signed letter of compliance. Before the letter is sent, all violations should be corrected; please contact me if additional time is needed to correct a specific violation. Include the following in your letter: facility ID number, list the item number describing how and when the violations were corrected and tell me how you plan to maintain compliance in the future with each item. If you prefer to mail your letter, you may send it to 2201 Mail Service Center Raleigh, NC 27699-2201 If the compliance action letter is not received by the due date another visit to the center may be made and additional violations may be added. The allergy care plan should be renewed annually, but the medication authorization forms are valid for 6 months. This ensures that the child is receiving the proper dosage of medication. I recommend designating someone to periodically check medications throughout the center to ensure compliance with these requirements. Applicable childcare requirements and the inspection was discussed with Administrator, Joann Feyen. Currently this facility operates with a 1 star license effective February 3, 2022. During the visit I monitored the indoor and outdoor spaces used for child care. I observed children during routine activities. I observed whole group activities and free play activities. Children were observed playing outside during outdoor play with adequate supervision. Age appropriate materials and equipment was accessible to all children. The facility was monitored for compliance using the annual compliance monitoring checklist. I also monitored, permit restrictions, posting requirements, training requirements, use of approved space, program records, and nutrition. 7 children's records were reviewed and staff records were reviewed along with the staff and training worksheet. The last fire inspection was completed 9/8/22. The last fire drill was completed on 6/15/23. The last lockdown drill was completed 4/18/23. The operation's EPR plan was last updated 7/14/22. The EPR plan was updated during inspection. Diaper creams and suntan lotions were observed to have current permissions in all rooms. Sleep checks were observed to be completed. Only 1 emergency medication was observed in the operation. The permission form for the emergency medication was not able to be located during inspection. A copy of the form was located and will be completed. Transportation is not provided by this facility. I observed 3 violations during this inspection. Violation Number Comment Rule 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. 7 staff members have no completed the required annual training hours for their last annual training period. .1103(a) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The EPR plan was last updated 7/14/22. This was updated during inspection. .0607(e) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Medication authorization was not avaialble for 1 emergency medication. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) The violations documented above must be corrected immediately. A signed and dated letter of compliance must be mailed by July 28, 2023 stating that all violations have been corrected. The letter can be emailed to scott.moore@dhhs.nc.gov or mailed to: Scott Moore 5 Seawell Ct Durham, NC 27703 If you state in the compliance letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by July 28, 2023 you must contact me with a proposed timeline of the corrections. In some cases the timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today's visit may negatively impact your compliance score. TECHNICAL ASSISTANCE All background checks are now valid for 5 years as of July 2022. Please amend your current letters on file by adding 2 years to the letter's listed expiration date. A reminder that medication permissions for emergency medications are only valid for 6 months and must be renewed every 6 months.
