The director, Molly Little, assisted me with today’s visit. We conducted a walk-through of the indoor and outdoor learning environments. I observed groups of children engaged in outdoor play and other groups washing hands, preparing for lunch or eating lunch. RATED LICENSE STATUS The program operates with a Notice of Compliance issued on 7/15/24 INSPECTIONS The last sanitation inspection was conducted on 11/10/25. The last fire inspection was conducted on 5/5/25 The last documented fire drill was conducted on 12/16/25. The last documented shelter-in-place or lockdown drill was conducted on 8/13/25. The last documented playground inspection was conducted on 11/5/25. The status of the owners, Fuquay Varina Baptist Church., is listed as current and active on the NC Secretary of State website. Limited monitoring of childcare requirements was assessed during today’s visit. I reviewed emergency medications, 2 new staff files and confirmed that all staff have current CPR, First Aid as required. The program does not transport children or enroll children under 2 years; therefore, these requirements were not assessed. The following violations were observed and recorded during today’s visit. Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last documented playground inspection was conducted 11/5/25. .0605(q) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). Four staff members have current qualification letters on file, but they were not renewed prior to the expiration date. G.S. 110-90.2(b) & .2703(n)&(o) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last emergency drill was conducted 8/13/25. .0604(u);.0302(d)(8) The above violations must be corrected immediately. By 1/27/26 please email me a signed letter of compliance. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 97%. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. TECHNICAL ASSISTANCE Criminal records are completed every 5 years to ensure that all staff meet the qualifications to work in child care. It is crucial that qualification letters remain current by renewing them before the expiration date. Four staff members have current qualification letters in their files, but all the letters were renewed at least one month past the expiration date. Staff can renew their criminal record qualification at least 6 months in advance. There are several ways to help staff remember when their qualification letters are due for renewal. Periodically, you can review the staff and training worksheets to see who is coming due, or you could set a calendar reminder or develop a separate document that tracks this information. Emergency drills are conducted to ensure that all staff and children can quickly respond in the event of an emergency that requires the children to shelter in place or the center needs to be locked down. These drills should be co=P0mpleted at least once every 3 months. It may be helpful to set a calendar reminder to help you track when the next drill is due. CONSULTATION minimal All childcare facilities are required to test their drinking and cooking water for led every three years. Your water was last tested, 4/14/24. See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. You stated that you had started a new drill log in August, but you were unable to locate this log, so I was unable to verify that any fire, shelter in place or lock down drills had occurred in this new school year. One staff members qualification letter expired 9/30/24. You stated that you had missed that when you were updating staff files. It may be helpful to keep a log or spreadsheet that shows when important staff dates are due to be renewed. This staff member should begin right away renewing her criminal background check. If she does not have a valid qualification letter on file within 14 calendar days, she will be unable to return to work until this is completed. Mulch is needed around the climbing structures on all three playgrounds. Sufficient protective surfacing helps provide cushioning in the event that a child falls from a climbing structure. The mulch should be at least 6 inches in depth and extend 6 feet around the structures. Michele Dunn, Nutrition Specialist accompanied me during a walk-through of the facility. During the visit, the director was not present. The allegations were not discussed. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 108 The operator made an effort to falsify information. On May 16, 2023, the center director provided inaccurate information to the Child Care Consultant regarding a child being left unsupervised. G.S. 110-91(14) 303 Children were not adequately supervised at all times. On April 18, 2023, a four-year-old child was left alone in a classroom for (19) nineteen minutes. Additionally, on an undisclosed date during the week of July 10, 2023, a three-year-old child was left alone in the classroom while in the bathroom for less than one minute. .1801(a)(1-5) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On May 5, 2023, a staff member slammed a two-year-old child onto a chair. On the same date, the same staff member pulled the same child across the floor while holding the child's ankles. G.S. 110-91(10) 904 Child was handled roughly. On May 5, 2023, a staff member grabbed a two-year-child by the ear and arm and lifted the child from the floor. The staff member then escorted the child from the classroom while holding the child's ear and arm. .1803(a)(1) 908 Discipline was not appropriate for the child's age and development. On April 18, 2023, a four-year-old child was placed in time-out for (19) nineteen minutes. .1803(b) 1810 There was a substantiation of child maltreatment. Michele Dunn, Nutrition Specialist accompanied me during a walk-through of the facility. During the visit, the director was not present. The allegations were not discussed. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 108 The operator made an effort to falsify information. On May 16, 2023, the center director provided inaccurate information to the Child Care Consultant regarding a child being left unsupervised. G.S. 110-91(14) 303 Children were not adequately supervised at all times. On April 18, 2023, a four-year-old child was left alone in a classroom for (19) nineteen minutes. Additionally, on an undisclosed date during the week of July 10, 2023, a three-year-old child was left alone in the classroom while in the bathroom for less than one minute. .1801(a)(1-5) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On May 5, 2023, a staff member slammed a two-year-old child onto a chair. On the same date, the same staff member pulled the same child across the floor while holding the child's ankles. G.S. 110-91(10) 904 Child was handled roughly. On May 5, 2023, a staff member grabbed a two-year-child by the ear and arm and lifted the child from the floor. The staff member then escorted the child from the classroom while holding the child's ear and arm. .1803(a)(1) 908 Discipline was not appropriate for the child's age and development. On April 18, 2023, a four-year-old child was placed in time-out for (19) nineteen minutes. .1803(b) 1810 There was a substantiation of child maltreatment. On October 10, 2023, the Division substantiated maltreatment. GS 110-105.6(a) 1877 A child was restrained as a form of discipline and the child's safety or the safety of others was not at risk.
