The owner and provider, Rosa Nino, assisted me with today’s visit. The second caregiver, Rosa’s daughter, Lena Bernal arrived while I was conducting the visit. We conducted a walk-through of the indoor and outdoor learning environments. There were 4 children present today. When I arrived, the children were engaged in free play in their indoor learning environment. Shortly after my arrival, the children were served morning snack. RATED LICENSE STATUS The program operates with a 4-star rated license issued on 10/30/19. They received 2 points in program standards, 7 points in staff education and 1 quality point through the educational option of having a BS/BA or higher in Early Childhood Education. During today’s visit we discussed the new QRIS pathways and you were given an opportunity to ask questions. You stated that you planned to pursue Pathway We discussed challenges and resources needed to be successful in your selected pathway 2. You are currently using Creative Curriculum and will need to select a formative assessment and begin training for implementing the child focused assessment. The last documented shelter-in-place or lockdown drill was conducted on 12/15/25. The last documented playground inspection was conducted on 1/5/26. A partial assessment of child care requirements was conducted today. I confirmed that the provider’s CPR, First Aid and ITS SIDS training was current. I also assessed the program’s postings were compliant with child care requirements and assessed for general health and safety requirements. It was icy, so I was unable to assess the programs outdoor learning environment. The program does not transport children, so these requirements were not assessed. The following violation was observed and recorded during today’s visit. Violation Number Comment Rule 1405 Authorization for up to 12 months to apply over-the-counter medications, topical ointment, topical teething ointment or gel, insect repellents, fluoridated toothpaste, lotions, creams, and powders; such as sunscreen, diapering creams, baby lotion, and baby powder, to a child, when needed, was not in writing and did not contain information required by rule .1720(b)(7). The topical medication permission forms were dated more than 1 year ago. 10A NCAC 09 .1720(c)(4) The above violations must be corrected immediately. By 2/18/26 please email me a signed letter of compliance. Before the letter is sent, all violations should be corrected; please contact me if additional time is needed to correct a specific violation. Include the following in your letter: facility ID number, list the item number describing how and when the violations were corrected and tell me how you plan to maintain compliance in the future with each item. If you prefer to mail your letter, you may send it to 2201 Mail Service Center Raleigh, NC 27699-2201 If the compliance action letter is not received by the due date another visit to the center may be made and additional violations may be added. COMPLIANCE HISTORY Prior to today’s visit, the program’s compliance history was 95%. According to NC General Statute 110-90 (4) (d) all facilities must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months. REMINDER Your well water is due to be tested this year for bacteria. Please email a copy of the results for this testing when it is completed. TECHNICAL ASSISTANCE One child’s emergency medication permission form expires this week. Please ask the parent to update this document as soon as possible. Topical cream medication permission forms must be renewed at least annually, or when the medication dosage changes. This ensures that the parent still wants you to administer this medication and that the dosage of the medication remains the same. It may be helpful to set calendar reminders to track when the medication permission forms are due to be updated. CONSULTATION All childcare facilities are required to test their drinking and cooking water for led every three years. Your water was last tested, 8/3/24. For more information, you should contact Clean Water for Carolina Kids. Contact www.cleanwaterforcarolinakids.org for more information. In addition, childcare facilities are now required to test for lead and asbestos hazards in their buildings. The results of the lead-based paint and asbestos testing was negative. Training in all the required health and safety topics must be completed at least every 5 years. It may be time for you to complete this training again. Health and Safety training is available, at no charge, on the DCDEE website, in Moodle. There is a link to this training on Moodle. Medication Administration is also a required health and safety training topic. This training is available through Moodle as a separate training. This documentation was reviewed with you, and a copy of the visit summary documentation was given to you at the end of the visit. Upon arrival, I was greeted by the owner, Rosa Nino Alfonso, I conducted a general walk-through of the indoor and outdoor learning environments. There were four children present today, ranging in age from infant to four years. RATED LICENSE STATUS The program operates with a 5-star rated license issued on 10/30/19. They received 2 points in program standards, 7 points in staff education and one quality point through the educational option of have a BA/BS in Early Childhood Education or Child Development. The program’s “held harmless status” has been extended until the new QRIS is implemented. This means that rated license visits will not be processed until the new rated license process is completed unless the program requests to proceed with a new rated license. During today’s visit we discussed the new QRIS pathways, and you were given an opportunity to ask questions. I also advised you to visit the DCDEE website to review the QRIS Modernization requirements and to visit the North Carolina Rated License Assessment Project at www.ncrlap.org for more information about the FCCERS 3. During the visit, Rosa’s daughter, Lina arrived, and she asked about the changes in capacity for FCCH. I briefly explained the rules related to capacity and advised that as we receive more guidance, there may be additional requirements to maintain an increased capacity. During today’s visit we discussed the new QRIS pathways and you were given an opportunity to ask questions. I also advised you to visit the DCDEE website to review the QRIS Modernization requirements and to visit the North Carolina Rated License Assessment Project at www.ncrlap.org for more information about the FCCERS 3s. In addition, Early Years, formerly known as Child Care Services Association may be able to provide technical assistance with your family child care home. INSPECTIONS The fire drill record was not available today. The lock down or shelter-in-place drill record was not available today. The last documented playground inspection was conducted 6/6/25 Child Care facilities are required to test their drinking and cooking water for lead every three years. Your water was last tested for lead on 5/7/24. Your program was inspected for lead based paint and asbestos and it was determined that no concerns were present at this time. MONITORING During this visit a full assessment of Child Care Requirements was conducted. The facility was assessed using items noted on the Annual Compliance for Centers checklist. The program is privately owned. Staff on-going training was assessed from 9/1/24 – 8/31/25. The following violations were observed and recorded during today’s visit: Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The last documented playground inspection was completed 6/6/25. 10A NCAC .1721(e)(5)(A-F) 903 When medication is administered, authorization for the operator to administer the specific medication according to the parent's or health care professional's instructions was not on file and available. The authorization form for one of the medications was expired and there was no authorization form for the other medication. 10A NCAC 09 .1720(c)(1&2) & .1721(a)(4) 1401 Prescribed medications were not stored in their original containers in which they are dispensed with the pharmacy label. Parents brought the prescription medication pre-loaded in droppers for the provider to administer to the children. 10A NCAC 09 .1720(c)(1)&(2) 1853 The operator did not conduct a monthly fire drill. No records were available for review today. .1719(a)(15) & .1721( e)(2) The above violations must be corrected immediately. By 8/18/25 please email me a signed letter of compliance. REMINDERS Rosa and Lina are due to complete CPR and First Aid by the end of August and ITS SIDS training by the end of September. Your staff health questionnaires should be updated by the end of August. TECHNICAL ASSISTANCE Fire drill and emergency drill records were not available today. To ensure that all staff and children are prepared to respond quickly to emergencies, these drills should be completed monthly. It may be helpful to set a calendar reminder and post the drill log so that you remember to conduct and document these drills as they occur. The last playground inspection was dated 6/6/25. Please complete these inspections monthly to confirm that the outdoor play area is safe. There is a child enrolled in the center who requires daily oral medication. For the provider’s convenience, the parent pre-measures the medication in droppers and brings them with their child each day. I spoke with the child’s mother today to explain child care requirements for administering medication. She stated that they have been bringing the medication in droppers because the child’s dosage changes according to the child’s weight. I advised that they should update the medication authorization forms as the dosage changes, or at least every 6 months, and that the medication must be stored in its original container. I told her that most pharmacies will provide a second labeled bottle for child care programs. To comply with child care requirements, the medication must be in a bottle labeled with the child’s name, dosage and other required information. CONSULTATION We will be hosting Family Child Care Providers training to review the new Rated License system in September. Be on the look out for the email regarding this important meeting. Training in all the required health and safety topics must be completed at least every 5 years. It may be time for your staff to complete this training again.
