D. Muldrow, administrator, arrived shortly after the visit began and assisted. Currently, this center operates with a Five-star License issued November 6, 2018, earning five (5) points in the Education Component, seven (7) points in the Program Component and one (1) Quality point. The last annual compliance visit was conducted on December 17, 2025. The last sanitation inspection was completed March 6, 2025, earning a Superior rating. The last fire inspection was completed March 6, 2025, and the facility was approved for daytime and nighttime care. You confirmed that there are currently no children enrolled on second shift and transportation is not offered. Children were observed participating free choice activity center play and a teacher directed art painting snowmen. I observed interactions to be nurturing in nature between teachers and children. FACILITY PROFILE This facility is owned by Donathan & Delillian, LLC. The corporation is listed as current/active in the North Carolina Secretary of State corporation’s database. The corporation that owns this facility is required to always maintain a current/active status. Failure to do so could result in Administrative Action. I reviewed the contact information for the owner and the facility with you. You verified the current information listed with DCDEE is correct. The following violations of child care requirements were documented. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. Several ant hills were observed on the front play area and the large play area in the back of the building. G.S. 110-91(6); .0601(b) 1824 The trained staff did not review the EPR Plan annually or when information in the plan changed to ensure all information was current. The EPR plan was revised 11/5/25 and the previous date was 1/11/24. .0607(e) The violations documented above must be corrected immediately. A signed and dated letter of compliance can be mailed or emailed to me and must be received by December 18, 2025. If mailing, you must submit two (2) signed and dated copies. My mailing and email addresses are noted at the bottom. The letter of compliance must detail how you corrected the violations. Please be aware that any written information you submit regarding the correction of the violation is legal documentation. Therefore, it is important that all submitted information is accurate and truthful. If it is determined that submitted information was inaccurate or willfully falsified, an Administrative Action, including Revocation of the facility’s license could be issued. COMPLIANCE HISTORY Prior to today’s visit your compliance history was 94% over an eighteen (18) month period. CHILDRENS RECORDS I reviewed ten (10) percent of children’s records, and no violations were observed. STAFF RECORDS I reviewed ten (10) percent of staff records, and no violations were observed. NC PRE-K This facility has one (1) NC Pre-K class in operation. NC Pre-K requirements were monitored today and found to be in compliance. The long-term teacher continues to meet requirements. The program continues to use creative curriculum for planning and implementing lessons. Brigance II was used as the developmental screening and completed well within the required ninety (90) days after enrollment. The formative assessment Teaching Strategies GOLD is also used by this program. I recommend reaching out to you NC Pre-K sponsoring agency as L. Harris has an exit date of 11/16/25 but continues to operate as the NC Pre-K long term substitute lead teacher. OUTDOOR PLAY AREA While monitoring the outdoor play area I observed several ant hills along the fence on the front play area. The majority were observed on the large play area in the back near the left side. You stated you would have the ant hills sprayed. I also observed a yellow tricycle on the large playground that did not have handlebar covers. Handlebar covers are required to ensure the children do grasp hot metal and that they cannot place their fingers in the handlebar holes. You informed me that you have ordered replacement covers. EPR PLAN While the EPR plan was reviewed on 11/5/25 the last review was documented as 1/11/24. Child Care Rules require that EPR plans be revised, or reviewed if no changes are needed, every twelve (12) months. This is verified by printing the entire plan, the cover page (current date printed) or page 28 which lists all dates the plan was reviewed and revised. TECHNICAL ASSISTANCE & REMINDERS: Technical assistance was discussed regarding ensuring that the second page of the playground inspection forms are completed each month. Monitor your current evacuation crib in space #3 to replace when needed. Continue to remind staff that cords should not be accessible to children in spaces where infants and/or toddlers are in care. When implemented staff must continue to document all screen time for the applicable age groups. Screen should be limited to no more than thirty (30) minutes daily and logs kept for the week to confirm no more than 2.5 hours weekly. Please note that violations may be documented if observed to be out of compliance. QRIS RATED LICENSE TRANSITION You attended the QRIS: Pathway to the Stars meeting that was conducted by myself and another consultant on 9/17/25. You informed me that you are unsure if you use pathway #1 or #2 to maintain a five (5) star rating. We reviewed the capacity, family/community engagement, CQI plan, Curriculum and formative assessment, and education requirements for a five (5) star license. I showed you the NCRLAP website and where to find information on the outreach assessments and self-study guidance/forms. We also reviewed the DCDEE website, under the “QRIS Modernization” tab to find the handouts detailing the Pathways and all requirements. I showed you where to find the details related to coaching and mentoring in section .3205 (e)(11). We have a tentative plan to submit the application for a rated license assessment in March 2026. This will give you time to finalize your decision and decide on what family/community engagement standards, curriculum, formative assessment and CQI standards you will choose and implement. Please contact me if you have any additional questions or need clarification. The form documenting the program’s transition plan was completed and signed today. A copy was left with you. ABCMS PROVIDER PORTAL TRAINING Upon checking the ABCMS website there is a roster available for your program. This confirms that you have completed the required ABCMS provider portal training. This roster must be maintained so be sure to add and remove caregivers as applicable. ANNUAL LICENSING FEES Invoices for the Annual License Fee were emailed to facilities subject to license fees on November 3, 2025. Payments were due by December 3, 2025. If you have not already, ensure you pay your program’s fee as soon as possible. All payments must be submitted through our website using a Mastercard or Visa debit/credit card or via e-check only. Please be aware that failure to pay the fee may result in the revocation of your child care license. Unpaid license fees will also incur a 10% late fee and interest as established by NCGS § 147-86.23. ANNUAL CHILD CARE IMMUNIZATION REPORT The 2025-2026 Annual Child Care Immunization Reporting Portal was due to be submitted electronically by midnight, November 15, 2025. Instructions and worksheets were available for download and may be used to collect data needed before entering your individual school information online. MARKET RATE SURVEY All NC child care facilities, please take time to complete the 2025 survey on the rates you charge for child care. Your survey answers provide valuable information to the Division of Child Development and Early Education (DCDEE). Survey results support DCDEE’s recommendations to the General Assembly for establishing rates for subsidized child care that reflect the current market. Information about cost and fees also helps the Division recommend fair payment rates for child care providers offering higher quality care who participate in the subsidized child care program. To ensure your responses are kept confidential, DCDEE has partnered with the Center for Urban Affairs and Community Services (CUACS) at North Carolina State University. If any additional information is needed regarding your completed survey, CUACS staff will follow up with you directly. D. Muldrow, administrator, was present and assisted with the visit. Currently, this center operates with a Five-star License issued November 6, 2018, earning five (5) points in the Education Component, seven (7) points in the Program Component and one (1) Quality point. The last annual compliance visit was conducted on December 17, 2024. The last sanitation inspection was completed March 6, 2025, earning a Superior rating. You noted that there are currently no children enrolled on second shift, and you do not offer transportation. I observed children completing lunch, toileting and handwashing routines, and napping safely in the facility. The following violations of child care requirements were documented. The administrator has yet to complete the required ABCMS provider training to set up the employee qualification letter roster. STAFF/CHILD INTERACTION & OBSERVATIONS I observed current activity plans and schedules in each space used for care. Schedules were followed as written during the visit. School-aged children completed lunch and toileting routines prior to cleaning up from lunch to return to their classroom in the modular unit. Children were napping safely in each space and the school-aged children were engaging in quiet activities at the tables and those not interested in playing rested their heads on the table. I observed a variety of learning materials in each space and materials needed to implement the activity plans were present. Activities noted on the plans engage children in a variety of developmental domains and offer chances for gross and fine motor development. If parents fail to sign and note the child’s arrival or departure time, then a staff person is required to document the time and either initial or sign their name. Arrival and departure times must be documented as they occur and not at the end of the operating day or left blank. You informed me that no new staff have been hired since the last visit. Therefore, staff records were not monitored. Fire drills, shelter-in-place/lockdown drills and playground inspections were observed to be up to date during this visit. RATED LICENSE ASSESSMENT I reminded you that the hold harmless state has been extended related to the rated license assessments. You be updated as more information is available. Programs that were listed in cohorts 1 and 2 will only go through the Environment Rating Scales or rated license assessment if they volunteer to do so. I encourage you to refer to the NCRLAP website at https://ncrlap.org and Franklin Granville Vance Smart Start, for more information. You can find information on the new ECERS-3 has been in use since February 1, 2025, to assess applicable programs. There are training courses and books available for review and purchase. Please contact me if you have any questions. ABCMS-CRMINAL BACKGROUND CHECK PORTAL The Division sent out an email several months ago regarding every administrator/owner completing the ABCMS Moodle training. You will need to use your business NCID. If you do not have a business NCID you do not create one. If you have not completed the ABCMS Moodle training, please complete it immediately. Once you have completed the training you will receive an access code to complete the Roster set-up, please wait 48 hours prior to using the access code (it takes about 2 days before you can use it). As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. You have yet to complete this required Moodle training. Therefore, a violation was documented. CLEAN CLASSROOMS FOR CAROLINA KIDS I reviewed the website https://www.cleanwaterforuskids.org regarding the programs lead water, lead paint and asbestos testing. I observed that the program is exempt from lead paint and asbestos testing after completing enrollment in the program and submitting required documents. The program completed its second round of lead water testing in April of 2024. Therefore, you are within compliance with those sanitation requirements. The program continues to use creative curriculum for planning and implementing lessons. Brigance II was used as the developmental screening for this school year. ENVIRONMENTAL RATING SCALES You were reminded that the hold harmless state has been extended. Therefore, programs that were in cohorts 1 and 2 do not have to proceed with the Environmental Rating Scale assessments (ERS). ERS is voluntary at this time, and you will be notified when this changes. TECHNICAL ASSISTANCE & REMINDERS Technical assistance given related to reviewing each child’s application when submitted to ensure that parents/guardians have included the name and phone number of their hospital preference. Remind parents not list “closest,” they will need to write the name and number for that specific hospital so that it may be quickly referenced should a medical emergency occur. Continue reminding staff that they must document the time that children leave the facility, and initial, at the time it occurs if parents/guardians fail to do so. An adult must sign the child out and their correct departure must be noted in case this information is ever needed. These items could result in a violation if they are not kept in compliance. Licensing fee invoices were emailed last month. Ensure that your email address remains current and once received that the payment is made online, on the Division website, by December 31, 2024. Please be reminded that failure to pay the license fee is a violation of General Statutes 110-90(1a) and will result in a penalty assessment, accrued interest, and issuance of an Administrative Action. The reporting period for child care immunizations is now open. Reporting for 2024-2025 has been extended due to Hurricane Helene. Children were given a grace period from requirements until November 30, 2024, and reports will be accepted until January 15, 2025. The Clean Water for Carolina Kids program has expanded to the Clean Classrooms for Carolina Kids program. This is a legislatively mandated effort to address lead and asbestos hazards in North Carolina public schools, licensed child care centers, and licensed family child care homes. Participation allows facilities to meet all rule requirements for identified lead and asbestos hazards, as required by law. Rules 10A NCAC 41C .1001 through .1007 require all programs to be free of lead and asbestos hazards. RTI is coordinating all the lead in water, paint and asbestos testing and remediation. Currently operating child care facilities are required to test for lead in paint and asbestos once unless the consultant notes a concern during a visit and refers the program to environmental health. After reviewing the website, I noted that your program has completed your second round of lead water testing. The program is also noted as exempt from the lead paint and asbestos testing. Due to active precipitation the playground was not monitored today, but will be monitored at the next visit. Children were observed participating in teacher directed coloring related to Martin Luther King and NC Pre-K children completed a writing activity. Two (2) staff files had annual evaluations and development plans dated 1/3/23 and 1/5/23 only. 10A NCAC 09 .0514(f) 1764 A health assessment was not on file at the NC Pre-K site within 30 days after a child entered the NC Pre-K program or the health assessment was more than 12 months old at the time of program entry. The health assessment on file for one (1) child enrolled in NC Pre-K on 8/30/23 did not complete the required health assessment until 10/30/23. .3005(a) COMPLIANCE HISTORY Prior to today’s visit your compliance history was 95% over an eighteen (18) month period. Child Care Rules require that children enrolled in NC Pre K have a health assessment within thirty (30) days of enrollment. A child enrolled 8/30/23, and noted on the children’s record form, did not complete a health assessment for NC Pre K enrollment until 10/30/23. STAFF RECORDS I also monitored ten (10) percent, or more, of staff records and noted two (2) violations. Two (2) staff hired 9/2/14 and 5/10/21 have annual evaluations and staff development plans dated for 1/3/23 and 1/5/23 on file. Child Care Rules require these evaluations and development plans be completed each year to ensure that staff remain effective and continue to improve any skills that need it. The staff are noted on the staff and training worksheet. At least two (2) staff have not met their annual on-going training requirements for this year. I recommend reviewing all staff files and ensuring they completed the remainder of their on-going training hours by 1/25/24 as that will make a year since the last annual compliance visit. Please email certificates for completed trainings for the two staff noted on the staff and training worksheet by that date. D. Muldrow, administrator, arrived shortly after the visit began and assisted. Currently, this center operates with a Five-star License issued November 6, 2018, earning five (5) points in the Education Component, seven (7) points in the Program Component and one (1) Quality point. The last annual compliance visit was conducted on January 25, 2023. The last sanitation inspection was completed June 29, 2023, earning a Superior rating. The last fire inspection was completed April 19, 2023, and the facility was approved for daytime and nighttime care. You noted that there are currently no children enrolled on second shift and transportation is not offered. Due to active precipitation the playground was not monitored today, but will be monitored at the next visit. Children were observed participating in teacher directed coloring related to Martin Luther King and NC Pre-K children completed a writing activity. Teachers were observed interacting with children in a nurturing nature. FACILITY PROFILE This facility is owned by Donathan & Delillian, LLC. The corporation is listed as current/active in the North Carolina Secretary of State corporation’s database. The corporation that owns this facility is required to always maintain a current/active status. Failure to do so could result in an Administrative Action. I discussed the contact information for the owner and the facility with D. Muldrow. You verified the current information listed with DCDEE is correct. The following violations of child care requirements were documented. Two (2) staff files had annual evaluations and development plans dated 1/3/23 and 1/5/23 only. 10A NCAC 09 .0514(f) 1764 A health assessment was not on file at the NC Pre-K site within 30 days after a child entered the NC Pre-K program or the health assessment was more than 12 months old at the time of program entry. The health assessment on file for one (1) child enrolled in NC Pre-K on 8/30/23 did not complete the required health assessment until 10/30/23. .3005(a) COMPLIANCE HISTORY Prior to today’s visit your compliance history was 95% over an eighteen (18) month period. CHILDRENS RECORDS I reviewed ten (10) percent of children’s records and noted one (1) violation. Child Care Rules require that children enrolled in NC Pre K have a health assessment within thirty (30) days of enrollment. A child enrolled 8/30/23, and noted on the children’s record form, did not complete a health assessment for NC Pre K enrollment until 10/30/23. STAFF RECORDS I also monitored ten (10) percent, or more, of staff records and noted two (2) violations. Two (2) staff hired 9/2/14 and 5/10/21 have annual evaluations and staff development plans dated for 1/3/23 and 1/5/23 on file. Child Care Rules require these evaluations and development plans be completed each year to ensure that staff remain effective and continue to improve any skills that need it. The staff are noted on the staff and training worksheet. At least two (2) staff have not met their annual on-going training requirements for this year. I recommend reviewing all staff files and ensuring they completed the remainder of their on-going training hours by 1/25/24 as that will make a year since the last annual compliance visit. Please email certificates for completed trainings for the two staff noted on the staff and training worksheet by that date.
