The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, activity plan, daily schedule, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of NC Child Care Law were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. A sample of existing staff files were reviewed and the Staff and Training worksheets was completed. Please complete the worksheet for all other staff members and send to me with your compliance letter by 5-28-26. A sample of children's records were reviewed and the worksheet was completed. The last fire drill was documented as conducted on 5-1-26 at 9:15 am with an evacuation time of three minutes. The last outdoor inspection was documented as conducted on 5-6-26. A shelter in place drill was conduct on 2-4-26 at 9:30 am. An approved fire inspection was completed on 1-16-26. A sanitation inspection was last completed on 3-12-26 with seven demerits and a superior classification. The children were observed playing outside, and during free play. A meal or snack was not observed during the visit. A three star child care center with four points in education, two points in program standards and one quality point was issued on 6-11-24. During your routine unannounced visit, we reviewed the changes to the rated license and you indicated that you would like to have your license transitioned to the new rated license under the Classroom and Instructional Quality Pathway. We reviewed the documents you will need to submit, along with the Application for a Rated License. You agreed to submit your application in August. I monitored the indoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival and departure procedures, first aid chart, menus, activity plans, daily schedules, staff/child ratios, emergency medical care plan, emergency telephone numbers and a summary of child care law were posted. No new staff members were hired since the 5-27-25 annual compliance visit. Therefore, no staff files were reviewed. The storage of medication and hazardous products was also monitored for compliance. A fire drill was documented as conducted on 2-4-26 at 9:30 am with an evacuation time of three minutes. An outdoor inspection was documented as conducted on 2-4-26. An approved fire inspection was completed on 1-16-26. A sanitation inspection was completed on 7-16-25 with five demerits and a superior classification. The facility is currently licensed as a three star child care with four points in education, two points in program standards and one quality point. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One outlet in the lobby of the facility was not covered or in use. 10A NCAC 09 .0604(c) You attended the Director's Meeting held by DCDEE on 10-7-25 during which the new child care requirements in section .3200 pertaining to two through five stars were reviewed. You stated today that your goal is to maintain your current three star license, with the possibility of increasing to a four star license. I reviewed the pathways with you and you stated that you are planning to apply to the Classroom and Instruction Quality Pathway, known as pathway 2. We reviewed the requirements for Family and Community Engagement and the Continuous Quality Improvement (CQI) for the facility and each individual staff member, including a review of these forms. The best way to locate these forms is to utilize the links from the Rated License Application, located on the Provider's Forms page. I encouraged you to become familiar with these forms and to share the individual CQI form with the staff members. I recommended that you have all staff members review their information in the WORKS database to determine if the information is up to date. If not, they will need to submit new official transcripts to the Workforce Unit. Please have them contact this until directly for questions pertaining to the their WORKS account. As we discussed, the goal will be for you to apply for a new rated license between today's visit and August, 2026. All application documents can be found on the DCDEE website. The incorporation status of Kidz Konnection LLC was reviewed prior to the visit using the Secretary of State's website. Based on the information available, you converted the LLC to a corporation in 2019. When I asked, you stated that you were not certain if this change was reported to DCDEE for the two centers you own in Four Oaks, NC. Kidz Konnection, LLC is listed as the owner for these two locations and the licenses were issued to Kids Konnection, LLC. I will contact you once I have additional information on next steps for updating this in our system. The children were observed playing outside, and during free play. Five children, ages eleven months to two years of age were grouped together. A violation was documented as children less than one year of age cannot be grouped with children two years of age and older, unless it is during the first or last operating hour of the day. You arrived as the facility and I explained the violation to you. Before the conclusion of the visit, you moved all children one year or age and older to another classroom and you provided care to the eleven month old. Therefore, the violation was marked as corrected. This type of violation is particularly serious as it directly impacts the safety of the children while in your care.
