The visit was conducted with you, Keirah Dawkins, Administrator. You provided me with applicable program, staff, and children’s records for review. The NC Secretary of State website was viewed before the visit and Western Carolina Community Action, Inc. was current/active as of October 15, 2025. The facilities contact information was reviewed and confirmed with you. This program currently operates with a Five star rated license effective May 10, 2023. The permit restrictions include 1st shift, meets enhanced ratios, meets enhanced space, and meets enhanced ratios minus one. The program’s compliance history was 96 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A checklist was used to note the requirements I monitored today. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios in compliance. Children and staff were participating in outdoor gross motor play, free play in activities areas, lunch, and rest time. The last monthly fire drill was practiced on September 23, 2025. The last lockdown drill was practiced on August 27, 2025. The last monthly playground inspection was completed on September 29, 2025. The Emergency Preparedness Response Plan was reviewed and published on March 19, 2025. A sample of three children’s records were reviewed. Three new staff files and one existing staff file was reviewed during today’s visit. The most recent approved fire inspection for your facility was conducted on October 10, 2025. The most recent sanitation inspection for your facility was conducted on March 18, 2025. A superior sanitation classification was issued with 0 demerits noted on the grade card. The most recent lead water test results were completed on February 14, 2023. Lead testing must be completed every three years. Healthy and Safety Training: Trainings must be completed in all eleven Healthy and Safety topics areas within one year of employment. The staff member employed on 4/3/2024 did not have verification of completed training in two of the eleven required topic areas. You reported that the certificates will be added to the virtual staff file within the next 2 weeks. We discussed that the health and safety training record may be used to help verify that all needed topic area trainings are completed and certificates are on file. Upon arrival you, Candice Freeman, Administrator, assisted me with the visit. A checklist was used to note the requirements I monitored today. Your program currently operates with a five star license effective May 10, 2023. The permit restrictions were in compliance including 1st shift, meets enhanced ratios, meets enhanced space and meets enhanced ratios minus one. The Secretary of State website was checked on October 10, 2023, and your business Western Carolina Community Action, Inc, is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. You conducted the walkthrough of the facility with me. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in outdoor playtime, lunch and rest time. Infants were engaged in outdoor playtime, feeding and napping. The caregivers were interacting and meeting the developmental needs for each of the children. The center's compliance history was at 97% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A sample of two children’s files were reviewed . Two new staff’s file were reviewed. One existing staff file was review. The last monthly fire drill was practiced on 10/4/23. The last lockdown drill was practiced on 9/13/23. The last monthly playground inspection was completed on 10/5/23. The most recent sanitation inspection for your facility was conducted on July 13, 2023. A superior sanitation classification was issued with three demerits. The most recent fire inspection for your facility was conducted on October 10, 2023. The facility was approved for day time care. The following violation(s) were observed and discussed with you today: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The most recent fire inspection was completed October 10, 2023. The previous fire inspection had been completed August 15, 2022. 10A NCAC 09 .0304(a) 428 A current activity plan was not posted for each group of children for reference. Areas discussed included: Fire inspection: Each operator must schedule and obtain a fire inspection within twelve months of the center's previous fire inspection. The fire inspection should have been completed before August 15, 2023. The fire inspection has been completed as of October 10, 2023. This violation was corrected during the visit. 10A NCAC 09 .0304(a) Activity Plan: I recommend that you check each classroom to ensure the current activity plan is posted. You may consider having staff to turn in activity plans on Friday for review and then post first thing on Monday morning. The current week’s activity plan was posted in Space 1 B during the visit. This violation was corrected during the visit. Hazardous Products: All hazardous items with multiple warnings must be kept in locked storage, including all aerosol products. Items labeled “keep out of reach of children” with no other warnings can be stored five (5) feet off the ground. Have teachers check the labels on all items to assure they are properly stored. 15A NCAC 18A .2820(b) The bottles were removed and placed in locked storage. This violation was corrected during the visit. Consultation or other topics discussed today: Qualifying letters are now valid for five (5) years from the date of issue. Please begin the criminal background re-check at least two (2) months prior to the expiration of the current qualifying letter.
