The visit was conducted with you, Cynthia Logan, Operator. Your program currently operates with a Four star license effective January 8, 2021. The permit restrictions include 1st shift, 2nd shift, and 3rd shift. This program was also monitored for compliance with implementing an approved curriculum as required for all four and five star licensed facilities where four-year -old children are enrolled. The last annual compliance visit was conducted June 2, 2025. The most recent monthly fire drill was conducted on March 3, 2026 at 8:00am. The most recent quarterly shelter-in-place drill was conducted on March 16, 2026 at 9:00am. The facilities EPR Plan was reviewed on January 1, 2026. The most recent monthly outdoor inspection was completed on March 1, 2026. The program’s compliance history was 97 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A walk-through was completed today, all indoor and outdoor caregiving areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children were participating in outdoor gross motor activates, free play activities indoors, handwashing routines, lunch, and nap. The caregivers were interacting and meeting the developmental needs for each of the children. A checklist was used to note the requirements I monitored today. Program Standards: At all-star levels, programs have no enrollment restrictions in addition to licensed capacity. We discussed at the Four and Five-Star Levels, programs must have an approved curriculum when four-year-olds are enrolled. You reported that the program uses Learn Every Day Preschool Curriculum. The Self-study Verification Form for Family Child Care Homes was reviewed during today’s visit. The Thinking More About Activities for FCCERS-3 paperwork used for the three-month self-study was on file at the facility. The facility completed the request for the Environment Rating Scale was completed and signed during today’s visit. This is a 2nd annual compliance visit based on the yearly annual compliance visit that was completed July 6, 2023 of last year. Upon arrival, the purpose of today’s visit was reviewed with you, Cynthia Logan, Owner/Operator. A checklist was used to note the requirements I monitored today. Your program currently operates with a four star license effective January 8, 2021 . The permit restrictions were in compliance including 1st shift, 2nd shift and 3rd shift. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor learning environment and found supervision and staff/child ratios in compliance. Children were participating in a group game and induvial play in activity areas. One child was not feeling well and was resting during the visit. Ten children, one through nine years of age, were enrolled. Five children were present today. The center's compliance history was at 97% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The FCCH and Centers Located in a Residence Operator’s Statements of Responsibility form and the Verification of Required Information for Operator and Additional Caregivers form was completed and signed during today’s visit. Your signature on these forms served as verification the information provided was accurate and complete. Criminal Background Check Qualifying letters for you and household members were reviewed and were in compliance during today’s visit. Your staff records were monitored during the visit. Children’s files were reviewed. The last monthly fire drill was practiced on 6/3/24. The last lockdown drill was practiced on 6/3/24. Your lead water testing was completed October 25, 2022. The following violation(s) were observed today and discussed with you today: Violation Number Comment Rule 1893 Application did not include all required information including, but not limited to: child's full name, named to be called, child’s date of birth, any allergies, including symptoms and the type of response required, and any fears or behavior characteristics a child has. One child enrolled 11/29/23 and two children enrolled 3/8/24 did not have all required information on the application including fears or behavior characteristics. 10A NCAC 09 .1721 (a)(3)(A)(B)(C)(E) The violation(s) documented must be corrected immediately. On or before June 27, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. If you email the compliance letter, it must be sent from the email address on file with DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Areas discussed included: Item #1893 A child’s application must include all required information including fears or behavior characteristics a child has. 10A NCAC 09.1721(a)(3)(E). We discussed that parents often do not write anything on the application if the item does not apply to the child. I encouraged you to have parents to list non-applicable if there is not information needed on a particular question. Consultation or other topics discussed today: We discussed that medical action plans need to be updated at least annually. One child has a medical action plan that needed review by July 6, 2024. The parent was present during the visit and updated the medical action plan during the visit. I discussed the EDU 119 course at the community college with the your additional caregiver to increase knowledge in the early education field. Ms. Tutt stated she may consider this course in the near future. Lead & Asbestos Testing The Clean Classrooms for Carolina Kids Program is providing free facility-wide testing and inspections without any out-of-pocket cost to child care centers and public schools. Enroll today at www.cleanwaterforuskids.org/carolina. The program is free, and the first step is to watch one of the pre-enrollment webinars, which are available at www.cleanwaterforUSkids.org/carolina or paste https://bit.ly/3CK-webinar. Facilities that have not yet enrolled should complete the online process by November 1, 2024. Due to the volume of testing and inspections, completion of the online enrollment process in the program is considered temporary compliance with the testing and inspection requirement. You must enroll in both sections including lead based paint and asbestos testing to comply with the rule requirements. Family Child Care Homes have until January 1, 2025, to complete the required lead water testing, lead paint testing and asbestos testing. Resuming Star Rated License Reassessment Information: As a reminder your facility is in cohort 2 for resuming the star rated license reassessment. Your child care program’s prep year will begin on July 1, 2024, through June 30, 2025. Your child care program’s reassessment year will begin on July 1, 2025, through June 30, 2026. Cynthia Logan, operator, was on site and caring for two children. During the visit, I discussed the allegations with Ms. Logan. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 1005 Children were not attended to in a nurturing and appropriate manner, and in keeping with the child's developmental needs. The operator moved a cot away from the wall while a six-year-old child was still on the cot. The child said the child fell off the cot as the cot was moved by the operator.
