The visit was conducted with you, Hailey Sturgill, Administrator. You provided me with applicable program, staff, and children’s records for review. The NC Secretary of State website was viewed before the visit and Choice Care Playschool, LLC. was current/active as of August 6, 2025. The facilities contact information was reviewed and confirmed with you. This program currently operates with a Five star rated license effective May 29, 2019. The permit restrictions were in compliance including 1st shift, school age only, meets enhanced ratios, meets enhanced space, meets enhanced ratios minus one, and fireplace not used during operating hours. The program’s compliance history was 99 percent prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A checklist was used to note the requirements I monitored today. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios in compliance. Children and staff were participating in outdoor gross motor time, free play activities indoors, and lunch time. A sample of four children’s records were reviewed. A total of two staff files were reviewed including one new staff and one existing staff. The last monthly fire drill was practiced on July 30, 2025. The last shelter-in-place drill was practiced on July 30, 2025. The last monthly playground inspection was completed on July 30, 2025. The Emergency Preparedness Response Plan was reviewed and published on July 5, 2024. The Emergency Medical Care Plan was updated on July 23, 2024. The most recent fire inspection for your facility was approved on February 26, 2025 for day time care only. The most recent sanitation inspection for your facility was conducted on June 24, 2025. A superior sanitation classification was issued with 4 demerits noted on the grade card. The most recent lead water test results were completed on August 14, 2023. Lead water testing must be completed every three years. One employee hired 5/22/25 was not listed on the facility roster in ABCMS. G.S. 110-90.2 & .2703(r) The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance statement by August 21, 2025. I suggested you review the facility’s Emergency Medical Care Plan with all staff at the same time that you review the facility’s Emergency Preparedness and Response Plan to ensure it is reviewed with staff at least annually or as changes are made. You have created your ABCMS Provider Portal but had not updated the roster to include a new employee hired 5/22/25. We discussed that you must regularly review your roster to make sure that it is accurate and make needed changes within five days of change to ensure compliance. Upon arrival you, Hailey Sturgill, Administrator, assisted me with the visit. A checklist was used to note the requirements I monitored today. Your program currently operates with a five star license effective May 20, 2019. The permit restrictions were in compliance including 1st shift, meets enhanced ratios, meets enhanced space, meets enhanced space minus one, and fireplace not used during operating hours. The Secretary of State website was checked on August 21, 2024, and your business Choice Care Playschool, LLC, is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. You conducted the walkthrough of the facility with me. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios in compliance. Children throughout the facility were participating in snack time, outdoor play, and homework time. The center's compliance history was at 98% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A sample of five children’s files were reviewed. One new staff member has been hired and one staff member has transferred from another Choice Care location since the last monitoring visit. A sample of one existing staff file was reviewed. The last monthly fire drill was practiced on 8/14/24. The last lockdown drill was practiced on 8/13/24. The last monthly playground inspection was completed on 8/20/24. The most recent lead water testing results were completed on August 14, 2023. Lead testing must be completed every three years. The most recent sanitation inspection for your facility was conducted on June 10, 2024. A superior sanitation classification was issued with no demerits. The most recent fire inspection for your facility was conducted on February 1, 2024. The facility was approved for daytime care. The following violation(s) were observed and discussed with you today: Violation Number Comment Rule 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. One child enrolled 5/28/24, did not have emergency and identifying information in the vehicle that provided transportation for the child. Photographs for seven children were not in the vehicle that provided transportation for the children. 10A NCAC 09 .1003(d) 1325 Parent’s statement includes the child’s name and date of enrollment and the date the parent signed the statement. One child enrolled 6/10/24 and one child enrolled 6/24/24 did not have the date of enrollment for the child on the verification of the discipline policy review form. .1804(b) The violation(s) documented must be corrected immediately. On or before September 4, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. If you email the compliance letter, it must be sent from the email address on file with DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Areas discussed included: Emergency information including the child’s name, photograph, and emergency contact information must be in the vehicle that the child is being transported in. You added the needed information for the child that enrolled 5/28/24 to the transportation vehicle during the visit. We discussed that missing photographs for seven children will need to be added to the transportation vehicle within the next two weeks. The discipline policy must be given to the child’s parent and explained to each child’s parent at the time of enrollment. A statement attesting that the center’s written discipline policy was be given to, discussed with the parent, and be on file for review. The statement must include the child’s name, the date of enrollment and the date that the parent signed the statement. You reported that each parent is given a copy of the center's policy and asked to sign. This form includes all the required information needed. You also have parents to sign another verification form that includes multiple policies on one form. This form does not include the date of enrollment for the child. You may choose to continue to use a signed copy of your discipline policy and/or your own policy authorization form. The chosen form will need to include all required information in order to meet the discipline policy requirement. Consultation or other topics discussed today: Rake and fluff the sand product that is used around the fall zone for the castle play structure to ensure a safe play environment. We discussed that the manufacturer’s information must be read for all outdoor equipment to ensure proper usage. The Dirt Bike Big Kid Dually Tricycles are for ages 7 and up. Make a plan for how the tricycles will be used for the proper age group only and used for single riders, unless information can be provided to verify that the tricycle can be used for tandem riders. Hold harmless has been extended until the new QRIS system is implemented. Programs still have the option to choose to voluntarily complete the star rated license reassessment if they so desire. Lead & Asbestos Testing The Clean Classrooms for Carolina Kids Program is providing free facility-wide testing and inspections without any out-of-pocket cost to child care centers and public schools. Upon arrival you, Hailey Sturgill, Administrator, assisted me with the visit. A checklist was used to note the requirements I monitored today. Your program currently operates with a five star license effective May 20, 2019. The permit restrictions were in compliance including 1st shift, meets enhanced ratios, meets enhanced space, meets enhanced space minus one, and fireplace not used during operating hours. The Secretary of State website was checked on August 21, 2024, and your business Choice Care Playschool, LLC, is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. You conducted the walkthrough of the facility with me. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios in compliance. Children throughout the facility were participating in snack time, outdoor play, and homework time. The center's compliance history was at 98% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A sample of five children’s files were reviewed. One new staff member has been hired and one staff member has transferred from another Choice Care location since the last monitoring visit. A sample of one existing staff file was reviewed. The last monthly fire drill was practiced on 8/14/24. The last lockdown drill was practiced on 8/13/24. The last monthly playground inspection was completed on 8/20/24. The most recent lead water testing results were completed on August 14, 2023. Lead testing must be completed every three years. The most recent sanitation inspection for your facility was conducted on June 10, 2024. A superior sanitation classification was issued with no demerits. The most recent fire inspection for your facility was conducted on February 1, 2024. The facility was approved for daytime care. The following violation(s) were observed and discussed with you today: Violation Number Comment Rule 1124 Emergency and identifying information, including the child's name, photograph, emergency contact information and/or a copy of the emergency medical care information form was not in the vehicle for each child being transported. One child enrolled 5/28/24, did not have emergency and identifying information in the vehicle that provided transportation for the child. Photographs for seven children were not in the vehicle that provided transportation for the children. 10A NCAC 09 .1003(d) 1325 Parent’s statement includes the child’s name and date of enrollment and the date the parent signed the statement. One child enrolled 6/10/24 and one child enrolled 6/24/24 did not have the date of enrollment for the child on the verification of the discipline policy review form. .1804(b) The violation(s) documented must be corrected immediately. On or before September 4, 2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. If you email the compliance letter, it must be sent from the email address on file with DCDEE (this serves as your signature) and the following information must be included: name, position, facility name, and facility ID number. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit may be completed. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Areas discussed included: Emergency information including the child’s name, photograph, and emergency contact information must be in the vehicle that the child is being transported in. You added the needed information for the child that enrolled 5/28/24 to the transportation vehicle during the visit. We discussed that missing photographs for seven children will need to be added to the transportation vehicle within the next two weeks. The discipline policy must be given to the child’s parent and explained to each child’s parent at the time of enrollment. A statement attesting that the center’s written discipline policy was be given to, discussed with the parent, and be on file for review. The statement must include the child’s name, the date of enrollment and the date that the parent signed the statement. You reported that each parent is given a copy of the center's policy and asked to sign. This form includes all the required information needed. You also have parents to sign another verification form that includes multiple policies on one form. This form does not include the date of enrollment for the child. You may choose to continue to use a signed copy of your discipline policy and/or your own policy authorization form. The chosen form will need to include all required information in order to meet the discipline policy requirement. Consultation or other topics discussed today: Rake and fluff the sand product that is used around the fall zone for the castle play structure to ensure a safe play environment. We discussed that the manufacturer’s information must be read for all outdoor equipment to ensure proper usage. The Dirt Bike Big Kid Dually Tricycles are for ages 7 and up. Make a plan for how the tricycles will be used for the proper age group only and used for single riders, unless information can be provided to verify that the tricycle can be used for tandem riders. Hold harmless has been extended until the new QRIS system is implemented. Programs still have the option to choose to voluntarily complete the star rated license reassessment if they so desire. Lead & Asbestos Testing The Clean Classrooms for Carolina Kids Program is providing free facility-wide testing and inspections without any out-of-pocket cost to child care centers and public schools.
