During today’s visit, Leeann Millwood, Administrator, accompanied me during a walk-through of the facility. The NC Secretary of State website was viewed before the visit and Alexander Baptist Church, Inc. was current/active as of January 20, 2026. The facilities contact information was reviewed and confirmed with you. This program currently operates with a Notice of Compliance effective September 4, 2013. The permit restrictions include 1st shift and uses corporal punishment. The program’s compliance history was 100 percent prior to today's visit. You must maintain at least 75% Compliance History for each 18-month period as required by G.S. 110-90(4)(c). Please note any violations cited today and during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action being recommended per child care requirements. During the visit, I discussed the information with you, Leeann Millwood, Administrator and two additional staff members. You reported forty-five children were in attendance during the visit. Limited monitoring of child care requirements occurred during today’s visit. You and other staff members stated that on 1/12/2026 at 4:10pm, one child three years of age was left on the playground unsupervised and unattended for one minute. The child came to the door in space #1 and was let inside by a staff member who was performing cleaning duties within the facility. The child was then returned to his classroom without injury. You stated that the child’s parent was notified of the incident via telephone call at 4:25pm. Staff reported that each classroom has a notebook that is used to account for the children during transitions. I reviewed camera footage for January 12, 2026, and confirmed the timing of the supervision incident. Based on staff interviews, video footage, and documentation provided about the incident, the self-report regarding supervision was confirmed. The following violation was confirmed and cited during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On 1/12/2026 at 4:10pm, one child three years of age was left on the outdoor space #1 playground unsupervised and unattended for one minute. .1801(a)(1-5) The violations documented must be corrected immediately. On or before February 4, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when each violation was corrected, and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit, and license ID number in the letter. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, it may be considered falsification of information. If sufficient information is not received by the due date, an unannounced follow-up visit may be conducted. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Adequate supervision means that: (1) staff must be positioned in the indoor and outdoor environment to maximize their ability to hear or see the children at all times and render assistance; (2) staff must interact with the children while moving about the indoor or outdoor area; (3) staff must know where each child is located and be aware of the children's activities at all times; (4) staff must provide supervision appropriate to the individual age, needs, and capabilities of each child; and (5) staff must be able to see and hear children aged birth to five years old while the children are eating. I suggested you review this rule with all staff members at this time, and annually at a staff meeting, that you consider implementing a head count checklist to ensure all children are accounted for during transitions. All staff members should be aware of the number of children they are responsible for at all times. Additionally, I suggested you remind staff to be extra cautious during transitions to ensure all children are accounted for and complete a head count before, during and after transitions. Upon arrival you, Leann Millwood, Administrator, assisted me with the visit. A checklist was used to note the requirements I monitored today. Your program currently operates with Notice of Compliance issued September 4, 2013. The permit restrictions were in compliance including 1st shift and uses corporal punishment. The Secretary of State website was checked on April 8, 2024, and Alexander Baptist Church, is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. You conducted the walkthrough of the facility with me. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to in compliance. Children throughout the facility were participating in group story time, free play in activity areas, transitions and outdoor gross motor time. Infants were engaged in tummy time and napping. The caregivers were interacting and meeting the developmental needs for each of the children. The center's compliance history was at 97% prior to today’s visit. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. A sample of seven children’s files were reviewed. Seven new staff files were reviewed. A sample of three existing staff files were reviewed. The last monthly fire drill was practiced on 3/26/24. The last lockdown drill was practiced on 1/11/24. The last monthly playground inspection was completed on 3/13/24. The most recent sanitation inspection for your facility was conducted on December 12, 2023. A superior sanitation classification was issued with eight demerits. The most recent fire inspection for your facility was conducted on August 7, 2023. The facility was approved for day time care. The following violation(s) were observed and discussed with you today: Violation Number Comment Rule 125 Daily records of arrival and departure times for children enrolled at the center were not maintained as children arrive and depart and/or were not made available for review. The top of the fence, less than six feet, was not free from protrusions. The outdoor play area had areas of the fence that measured 3 feet 6 inches high. You are going to discuss a plan of correction with the childcare board, but are unsure if you can complete this task in the next 2 weeks. If the fencing will not be in compliance within two weeks, please send me an email requesting an extension with specific information about how/when compliance will be achieved. Item #840 Hazardous Products: All hazardous items with multiple warnings must be kept in locked storage, including all aerosol products. Items labeled “keep out of reach of children” with no other warnings can be stored five (5) feet off the ground. Have teachers check the labels on all items to assure they are properly stored. 15A NCAC 18A .2820(b). An aerosol can of Zambee’s Soothing Saline Nasal Mist was removed from the classroom. This violation was corrected during the visit. Item #1321 Each child must have a completed medical exam on file for review within thirty days of enrollment GS 110-91(1). Item #1882 There must be a signed authorization giving a caregiver standing permission to apply over-the-counter topical ointments to a child with the all required information. 10A NCAC 09 .0803(7) Consultation or other topics discussed today: We discussed that outdoor play trucks that have rust need to be spray painted to prevent rust from being exposed. Per 10A NCAC 09.0304, The operator shall submit the original of the approved annual fire inspection report to the Division within one week of the inspection visit on the form provided by the Division. If the fire inspection report is not submitted to your Child Care Consultant within one week after the fire inspection is conducted, a violation will be cited during the facilities next monitoring visit. Name of Operation: ALEXANDER BAPTIST DAY CARE CENTER Facility ID: 8159002 Consultant: BETH ARCHER Operation Type: Center Case Number: Visit Date: 10/25/2023 Number Present: 58 Completed Date: 10/25/2023 Age: From 0 To 5 Total Minutes: 215 Time In: 09:40 AM Time Out: 01:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced routine unannounced visit was to monitor your compliance with all applicable child care requirements, including health and safety. Upon arrival, the purpose of today’s visit was reviewed with you, Leeann McGinnis, Administrator. Your program currently operates with a GS 110-106 license effective September 4, 2013. The permit restrictions were in compliance including 1st shift and uses corporal punishment. The Secretary of State website was checked on October 25, 2023, and your business Alexander Baptist Church, is active and in good standing. If any changes to the corporation need to be made or you decide to sell your business, then you must notify me, your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location, or inherited. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. You conducted the walkthrough of the facility with me. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in free play in activity areas and outdoor play time. Infants were engaged in tummy time and napping. The caregivers were interacting and meeting the developmental needs for each of the children. The center's compliance history prior to today’s visit was reviewed with the operator. The program’s compliance history was 97 percent as of October 25, 2023. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Five new staff have been hired since the annual compliance visit. New staff files were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. The most recent sanitation inspection for your facility was conducted on June 23, 2023. A supervisor sanitation classification was issued with ten demerits. The most recent fire inspection for your facility was conducted on August 17, 2023. The following violation(s) were observed today: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The fire inspection completed August 17, 2023 was not on the DCDEE approved childcare fire inspection form. 10A NCAC 09 .0304(a) 815 Electrical cords were accessible to infants and toddlers. In Space #6, a CD player with an electrical cord was accessible to toddlers. In Space #7, two bottle warmers with electrical cords were accessible to infants. 10A NCAC 09 .0604(f) 1067 Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. Two employees, one hired 9/11/23 and one hired 10/2/23, had not completed any hours for orientation since hired. .1101(a)(b) The violation(s) documented must be corrected immediately. On or before November 8, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. The Division of Child Development may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. Technical assistance on violations was reviewed with you. Areas discussed included: Fire Inspection: We discussed that fire inspection must recorded on the original approved childcare fire inspection form. I emailed you a current copy of the form discussed. You plan to contact the fire inspector about completing the childcare required form and will return it to me when complete. 10A NCAC 09 .0304(a) Electrical Cords: Electrical cords must not be accessible to infants and toddlers. We discussed moving items with electrical cords to cabinets that are five feet from the floor. The bottle warmers and CD player were moved out of the classroom spaces. This violation was corrected during the visit. 10A NCAC 09 . 0604(f) Staff Orientation: Each new employee must complete six clock hours of training in required topic areas within the first two weeks of employment and a total of sixteen clock hours within the first six weeks. You reported that due to having to be in the classroom frequently, the two newest employee’s orientations have not been completed. You plan to meet with both staff members missing orientation today and begin the required orientation training hours. NCAC 09.1101(a)(b) Consultation or other topics discussed today: We discussed the capacity requirements for outdoor space. I will plan to return and re-measure space as the outdoor space drawings are outdated and may have changed since last calculated within the boundaries of each individual space. We discussed information pertaining to COVID-19.
