This visit was conducted with Donna Church. Lead teacher. Your program currently operates with a FIVE (5) Star license effective 10/21/19. Restrictions include 1st shift daytime care; meets enhanced ratios; meets enhanced space: School age playground equipment cannot be used by preschoolers; meets all enhanced requirements and reduced ratios minus one. This one (1) NC Pre-K classroom is at Mt. Pleasant Elementary School, a public school in Wilkes County. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children having outdoor play, having morning snack, and having free choice center play. I observed materials and activities that were available to the children in the classrooms. Your facility uses Creative Curriculum with Teaching Strategies Gold. The last annual compliance visit was conducted on 4/1/25. A sanitation inspection was completed on 11/12/25 with a Superior classification and two (2) demerits. The last fire inspection was conducted on 11/12/25. A copy of this inspection was received during today’s visit. Program records and required postings were monitored. A fire drill was conducted on 2/10/26 at 12:40 pm with fifty-six (56) seconds to evacuate eighteen (18) children and two (2) adults by A. Byers. A lockdown drill emergency was documented on 1/9/26 at 2:00 pm with one (1) minute, thirty (30) seconds by A. Byers. An outdoor inspection was documented on 3/2/26 at 9:45 am by D. Church. No new staff files, one (1) substitute file, one (1) teacher assistant file, and two (2) children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Hazardous items were stored in locked closets. Storage and administration of medication were monitored. Medication authorization was monitored. Today I observed one Epi Pen for a peanut and tree nut allergy. The required medication paperwork was available for review. The program does provide transportation through Wilkes County school system buses. Today I monitored the Off-Site Transportation Records Verification signed by Eric Barker on 8/5/25. I observed the Transportation Policies for Parents of children who are transported by buses, the school bus drivers Responsibilities and Procedures Orientation forms for three (3) bus drivers and the Center Director Responsibilities and Procedures form signed by A. Byers on 8/28/25. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC Pre-K staff are lead teacher, Donna Church and teacher, Chrystal Parsons. Today Ms. Parsons was absent and substitute D. Harris was present. A selection of files was monitored for complete health assessments and developmental screenings. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum with Teaching Strategies Gold. You stated you completed the Dial screenings in March at the Preschool screenings for all children and as new children are enrolled, they are screened at that time. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress and documentation was viewed today. The checkpoint assessments are conducted three (3) times per year: at the beginning, middle and end of the school year. The first day of school was 8/28/25. The classroom operates from 7:30 a.m. to 3:05 p.m. Parent conferences are held two (2) times per year or more often as needed or requested. You stated that you use Remind App, notebooks, phone calls, and face to face communication to communicate with parents. In addition, a daily and weekly report is sent home to the parents which includes any information about the child’s day and any reminders, along with calendars and newsletters. You verified that you incorporate family engagement by parent meetings, P.T.O. meetings, family night, and four (4) parenting sessions per year. The environment rating scale score received on 10/17/19 was 6.17 points. The following violations were cited during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. Today I observed the fence around the outdoor playground is rusting and needs repair. I observed a sharp edge at the bottom of the wire fencing at the storage building that could be hazardous to the children if touched. 10A NCAC 09 .0601(a) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. This NC Pre-K classroom staff is not registered in the ABCMS portal as of 3/9/26. G.S. 110-90.2 & .2703(r) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send me documentation verifying compliance on or before Monday, March 23, 2026. This procedure is to ensure that all staff are registered in the ABCMS (Automated Background Check Management System) for the children’s safety. Refer to Childcare rule 10A NCAC 09 .2703(r) or GS 110-90.2. • Rust should be removed from all surfaces for the safety of the children. The timeline would be to go through Pathway 1 in the Fall of 2026 after the 3-month self-studies are complete. Today you stated that you are planning to do Pathway 1 since you are an NC Pre-K program. Beginning in April, you may start to apply for your reassessment if you are ready and any required self-studies are complete. Or you may apply early if you are already accredited and know your Pathway. • The Application for Assessment for a Rated License is now live on DCDEE’s website under Provider Documenters. You are welcome to view this form and see all the required forms for each pathway. You can access the applications below for a Center. In addition to gaining introductory knowledge about child development, early educators will learn about the many resources and professional agencies available in North Carolina to help with promoting a responsive and engaging learning environment to support all children served. I observed staff/child interactions today. I observed children having outdoor play, having morning snack, and having free choice center play. Indoors, Ms. Parson stated that new dramatic play furniture is on order, as well as new shelves that are to arrive before Thanksgiving. I observed children having free choice center time, including sand play, playdough station, block play dramatic play, and magnetic block play. No new staff was reported at this program. A playground inspection was recorded 11/1/23 with leaves needing blowing and grass needing cutting noted on the playground inspection checklist. A fire drill was recorded on 10/24/23 at 10 am with forty-seven seconds to evacuate sixteen (16) children and two (2) adults. A lockdown emergency drill was recorded at 10 am on 8/31/23 with twenty (20) seconds to lockdown by A. Byers. The most recent fire inspection was on 10/25/23. A copy of this inspection was received during today’s visit. A sanitation inspection was last conducted on 11/1/23 earning a Superior classification with eleven (11) demerits. Whitney Holbrook, lead teacher, assisted me with the visit. This is a one (1) NC Pre-K classroom program in the Wilkes Public School system. Your program currently operates with a FIVE (5) Star license effective 10/21/19. Restrictions include Daytime care only; preschool children cannot use School age playground equipment; Meets all enhanced requirements and reduced ratios minus one: meets enhanced ratios: meets enhanced space. The program’s compliance history was 95% prior to today’s visit. All programs are required to maintain at least 75% compliance. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. On the outdoor play area, screws and bolts were found in the mulch due to the ongoing construction at this school. The two (2) metal outdoor picnic tables are rusted. Ms. Holbrook stated they do gross motor activities in the classroom if the construction workers are outside their playground area. She also stated the children have been alerted not to touch, but to tell her, or the teacher assistant, Ms. Parsons if they see any screws, etc., on the playground. Ms. Holbrook stated that she or Ms. Parsons will do a walk-through of the play area prior to going inside the fenced area. Indoors, Ms. Parson stated that new dramatic play furniture is on order, as well as new shelves that are to arrive before Thanksgiving. Upon arrival, staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed positive interactions today. I observed children having free choice center time, including sand play, playdough station, block play dramatic play, and magnetic block play. No new staff was reported at this program. There was no medication observed nor reported. There were no allergies reported. Storage hazardous items was monitored today. A playground inspection was recorded 11/1/23 with leaves needing blowing and grass needing cutting noted on the playground inspection checklist. A fire drill was recorded on 10/24/23 at 10 am with forty-seven seconds to evacuate sixteen (16) children and two (2) adults. A lockdown emergency drill was recorded at 10 am on 8/31/23 with twenty (20) seconds to lockdown by A. Byers. The most recent fire inspection was on 10/25/23. A copy of this inspection was received during today’s visit. A sanitation inspection was last conducted on 11/1/23 earning a Superior classification with eleven (11) demerits. This Wilkes County School does provide transportation in the am and pm for the NC Pre-K children. I observed the Public-School Off-Site Records Verification form signed by Westley Wood, Assistant Superintendent of Wilkes County Schools on 8/22/23, the Transportation Requirements form signed by Eric Barker, Director of Transportation on 8/22/23/23, along with the Bus Driver Responsibilities and Procedures Orientation information form signed by three (3) bus drivers and Principal, Andrea Byers. The following violations were cited during today’s visit: Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. I observed screws, nuts, bolts, and nails on the playground area due to the ongoing construction going on at this school. I observed the ground cover showing through in the sandbox causing a tripping hazard for the children. I observed two (2) metal picnic tables that are rusted.
