The previous Annual Compliance visit was completed November 25, 2024, and prior to today’s visit, the eighteen-month compliance history score was 97%. Your program currently operates with a three-star license issued March 26, 2020. The License was based on the program earning four points in education standards, two points in program standards, and one quality point by having an infant capacity restricted to no more than two children under one year of age. The information on record with DCDEE was verified as correct by Chadawn Grant. Current Inspections are as follows: Sanitation Classification of Superior completed May 23, 2025. Upon my arrival, I observed three children in care with the operator, Chadawn Grant. I conducted observations of both the indoor and outdoor learning environments. Due to ongoing renovations to the backyard, the outdoor play area is currently located at the front of the facility. The lower-level bathroom is also under renovation; therefore, children must use the bathroom facilities on the second floor for handwashing and toileting. The operator ensures that children are safely escorted up and down the stairs as needed. During the visit, I observed handwashing, toileting procedures, sanitation practices, and learning activities. Children participated in number recognition activities using large flashcards displayed on the table. Ms. Grant asked questions about each number, and the children demonstrated their knowledge by counting sequentially from 1 to 29. The children also engaged in a musical movement activity. Music was played while the children marched around a table and sang along to the song. During free play, children sat at a table and explored sand, construction vehicles, figurines, spoons, and bowls. Ms. Grant encouraged language development and critical thinking by asking open-ended questions about their play. For example, Ms. Grant asked children questions related to making squares and other structures. Lunch will consist of chicken hot dogs, wheat bread, corn, peaches, and milk. The visit summary was reviewed with Ms. Grant. The following violations were observed during today’s visit: Violation Number Comment Rule 701 All indoor and outdoor areas used by the children were not kept clean, orderly, and free of items which are potentially hazardous to children including removal of items a child can swallow; the removal of loose nails or screws and splinters on inside; and use of outdoor equipment that is too hot to touch. There were several nails protruding from multiple steps on the staircase leading to the second-floor bathroom. .1719(a)(1)&(17) 707 Indoor and outdoor stairs with more than two steps that are used by the children were not railed. The staircase leading to the second-floor bathroom did not have a handrail. 10 A NCAC 09 .1719(a)(29) 709 Equipment and toys were not in good repair and developmentally appropriate. On the outdoor area where children play, the blue and white play equipment had visible cracks on both the top portion of the structure and the slide. 10 A NCAC 09.1720(a)(7) 716 Electrical outlets not in use were not covered. There were four electrical outlets in the child care space that were not covered with protective safety caps and two uncovered electrical outlets in the area leading to the kitchen. 10A NCAC .1719(a)(27) 729 Appliances with heating elements, such as bottle warmers, crock pots, curling irons, and electrical cords were accessible to children. Two electrical cords are accessible to children and hanging in the space leading to the kitchen. 10A NCAC .1719(a)(28) 1544 The child care operator did not comply with all State laws, federal laws and/or local ordinances that pertain to child health, safety, and welfare, as required by General Statute 110-91. The facility's last sanitation inspection was May 23, 2025. GS 110-91 The violation(s) must be corrected immediately. Mail or email a written, dated, and signed statement to me by 5:00 pm on June 15, 2026, describing how and when the violations were corrected and how the facility plans to maintain compliance in the future. If the compliance letter is not received by the due date, an additional visit will be conducted to verify that violations have been corrected. You stated that you were unable to provide a projected completion date for the renovations because you do not currently have a contractor. You have worked with several contractors; however, issues with follow-through and completion of the work resulted in delays. At this time, your husband is completing some of the renovation work. The operator will continue to provide updates regarding the progress of the renovations and any changes that may affect the child care environment. The facility’s last sanitation inspection was on May 23, 2026. Please contact Environmental Health to check the status of your inspection. Refer to child care rule G. S. 110-91. A violation was cited for several nails protruding from multiple steps on the staircase leading to the second-floor bathroom. The exposed nails present a potential safety hazard to children and adults using the stairs. The operator was informed of the observation and advised to ensure the nails are repaired or removed to prevent injury. Refer to child care rule .1919(a)(1) &(17). I observed that the staircase leading to the second-floor bathroom did not have a handrail. Children are required to use this staircase to access the bathroom for handwashing and toileting during the lower-level bathroom renovation, the absence of a handrail presents a potential safety hazard. You are advised to address this concern to help ensure the safe use of the stairs by children and adults. Refer to child care rule .1719 (a)(29). I observed a blue and white outdoor play structure in the children's play area. The play equipment had visible cracks on both the top portion of the structure and the slide. The damaged equipment may present a potential safety hazard to children during use. We discussed repair or removal of the equipment to ensure a safe outdoor play environment. Refer to child care rule .1719(a)(21). There were four electrical outlets in the child care space that were not covered with protective safety caps. In addition, two uncovered electrical outlets were observed in the area leading to the kitchen. The uncovered outlets present a potential safety hazard to children. Ensure that all accessible electrical outlets are properly covered to help prevent injury. Refer to child care rule .1719. OBSERVATION SUGGESTIONS: Giving children choices and letting their interests determine what activities you do, you do them, how long you do them; helping children learn to care for themselves and take on responsibility. This is important because children must be interested in an activity in order to learn from it. If you let children choose activities, you can feel confident they are ready to learn. Strategies are: • Let children choose where and how they want to play and when they want to change activities; asking children to share their activities with you and incorporating those ideas into activities. • Not letting your rules for behavior or agenda for the day get in the way of children’s learning; children’s freedom to move and talk if is not disrupting other children; allowing children to join and leave activities based on their interest. (adjust pacing for individual children, allow movement and talking). • Giving children responsibilities in the classroom and as much independence within activities and tasks as possible; helping children learn to resolve their own conflicts by giving them words to say and encouraging them to consider other children’s perspectives. Two additional children arrived during the visit.The children were observed sitting at a table playing with sand. Ms. Grant interacted with the children through positive communication and participating in making things out of sand. The children made ice cream using ice cream cones. They made boulders using different type of trucks like (dump trucks, tractors, etc.). They talked about food trucks, ice cream trucks, using a plastic knife safely, and elephants. One child was redirected when shouting. There was a discussion about, “yolk”. Ms. Grant explained to a child where yolk comes from. I observed toileting, hand washing, preparation for outdoors and outdoor play. I observed, children’s arrival, sanitation, toileting, and hand washing. Today’s breakfast consisted of cheerios, apple slices, and milk. Lunch will consist of chicken nuggets, carrots, fruit cocktails, and milk. The visit summary was reviewed with C. Grant. The following violations were observed during today’s visit. Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. A lawn mower was observed on the outdoor area accessible to children. .1719 (a) (1) 1953 Operator did not ensure child's hands were washed upon arrival at the home each day. One child did not wash his hands upon arrival at the home. .1725(a)(4) The violations must be corrected immediately. Ms. Grant greeted the parents and children by name. She reminded the other children to say good morning. Two children were rocking dolls to sleep in the dramatic play area. Another child was pushing cars on a car track. Next, it was clean-up time. After cleaning up, children sat in chairs and participated in a group activity. The discussed the letter, “H” and the word, “Hot”. Ms. Grant asked the children questions like, “Tell me something that is hot?” The children responded with, stove, iron, and food after cooking it. I observed toileting, hand washing, preparation for outdoors and outdoor play. Outdoor play was in the front yard during today’s visit. Upon returning inside, the children washed their hands and sat at a table to do a writing activity coloring a circle. Lunch will consist of pasta, shredded cheese, apple sauce, bread, and milk. The visit summary was reviewed with Ms. Grant. The following violations were documented during today’s visit. Violation Number Comment Rule 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. The operator's pet (Buddha) rabies vaccination expired 8/12/23. .1719(b)(1) 1848 A Division representative was denied access to inspect the child care facility. During an attempted visit on November 21, 2023 a representative of the DCDEE was denied access to inspect the facility. Upon the representatives arrival, the operator and two children were observed in the child care space window. No one answered the door or phone. A visit summary was completed and left on the front door entrance. On November 23, 2023, the operator emailed the consultant with, "I wasn't ready the other day and don't want any violations". My hours are 7:30 - 6pm. GS 110-105 .1709 The violations must be corrected immediately.
