The previous Annual Compliance Visit was completed July 16, 2025, and prior to today’s visit, the eighteen-month compliance history score was at 94%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Notice of Compliance issued August 6, 2019. The information on record with DCDEE was verified as correct by Kay Matthews and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior complete January 23, 2026. A sanitation inspection was received during today's visit and updated in the Regulatory system. Fire Classification of approved completed March 4, 2026. Kay Matthews, Director, was present and assisted with monitoring both the indoor and outdoor areas. Upon my arrival, there were 59 children in care, ranging in age from one to five years. In space #102, children were cleaning up and participating in a group activity on the carpet, specifically “Ring Around the Rosies.” Children in spaces #107 and #108 were observed outdoors running, playing with balls, and using the slide. In space #110, I observed sanitation practices and handwashing; children were seated on the carpet, and positive communication was evident between the teacher and children. For example, the teacher stated, “If you hear me, wiggle your fingers.” The group discussed the letters M and N, identified classmates whose names begin with M, and practiced the /m/ sound. In space #111, children were seated at tables using ditto sheets and crayons to trace and color the letter N and objects beginning with that letter. In space #119, children were reviewing sight words, and in space #121, children washed their hands and sat at tables for morning snack. Snack consisted of Ritz cheese crackers. Each child’s lunch was provided by their parent. At the completion of the visit, the Visit Summary was reviewed with K. Matthews. The following violations were observed. Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two staff members were found to have expired First Aid training certifications. Please refer to the Staff Training Worksheet for details regarding the affected individuals and expiration dates. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Three staff members were found to have expired CPR training certifications. You also stated that you will begin scheduling two CPR and First Aid training sessions per year to ensure ongoing compliance and to prevent future lapses in certification. While scheduling is a necessary step, certifications must always be valid and on file. A lapse represents non-compliance with the rule. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Upon my arrival, there were 116 children in care. Children in space #119 participated in a group activity. They discussed the calendar and Rhyming words. Children were asked about words that rhymed. Children raised their hands and gave responses like, door/floor and boy/toy. The teacher gave affirmation and encouragement to children by saying, “good one”. Children were observed eating morning snack in several classrooms. Breakfast and snack are provided by parents. Therefore, each classroom had a different snack. I observed preparation for outdoors. Children put on their coats and formed a line. Once outdoors, children climbed up and down the play equipment. Some children were observed swinging, sliding, and running. Children were also observed, playing with manipulatives (colored magnetic windows, stacking blocks). Children read books and used ditto sheets like, coloring a boat and the letters N and J. Teachers were observed assisting children and asking questions like, what color is that?” I observed toileting and hand washing. Lunch consisted of spaghetti, green beans, mixed fruit, roll, and milk. At the completion of the visit, the visit summary was reviewed with K. Matthews. The following violations were observed: Violation Number Comment Rule 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Respectful language was used, such as addressing each child by their name and moments were taken to check-in with each child during activities to see how they were doing and what their needs are. The previous Annual Compliance Visit was completed August 2, 2023, and prior to today’s visit, the eighteen-month compliance history score was at 96%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Notice of Compliance issued August 6, 2019. The information on record with DCDEE was verified as correct by Kay Matthews and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed May 2, 2024. Fire Classification of approved completed March 1, 2024. There are concerns that an employee’s family member has been left alone with children of all ages to provide supervision during bathroom breaks for teachers. Clair Shea, Assistant Director, was present and assisted me while observing the indoor and outdoor areas. Upon my arrival, there were 62 children in care, ages one through four years old. I observed outdoor play, hand washing, group time, morning snack, and lunch. Morning snack consisted of assorted chips and water. Lunch was ordered from Chic-Fila. It consisted of chicken nuggets, fries, cookies, and milk. Lunch was missing the vegetable and fruit components. The allegation was discussed with the Assistant Director. I gathered information and interviewed the Assistant Director and two teachers. It was stated that a teacher’s daughter (who was not an employee) was standing in the hallway when another teacher asked her if she could stay with her class while she goes to the bathroom. The daughter agreed and went to the classroom. A staff member, who is the mother overheard the teacher asking the daughter. She went to the classroom and told the daughter that she could not be alone with children because she is not an employee. The teacher of the daughter stayed with the children until their teacher returned. The daughter, an unqualified person, was with the children alone for approximately two minutes. The daughter is currently an employee, her date of employment was May 22, 2024. The employee who asked an unqualified person to stay with her classroom was no longer employed. Based on the information gathered and observations made, the allegation regarding supervision is unsubstantiated. Kay Matthews, Director, was on vacation during the visit. I spoke with her via telephone. She authorized me to review the visit summary with Clair Shea. At the completion of the visit, the Visit Summary was reviewed with C. Shea. The following violations were observed: Violation Number Comment Rule 501 Meals/snacks did not comply with the Meal Patterns for Children in Child Care Programs. Lunch served was from Chik-Fil-A and included chicken nuggets, fries, cookies, and milk. Children were not served a supplemental grain and vegetable/fruit component. 10A NCAC 09 .0901(a) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. A current menu was not posted. 10A NCAC 09 .0901(b) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. One staff member (J.M.) did not have an emergency information form on file. .0701(a) 1041 Prior to employment a Criminal Background Check was not completed. Javianna Miller was left alone with children prior to completing the criminal background check and receiving a qualifying letter. This was corrected during the visit when it was determined Ms. Miller received the qualification letter dated 5/13/2024. G.S. 110-90.2(b) 1757 A valid qualification letter was not on file and available to review at the facility. The previous Annual Compliance Visit was completed August 2, 2023, and prior to today’s visit, the eighteen-month compliance history score was at 96%, which is at or above 75%, as required by North Carolina General Statute 110-90(4)(c-d). This program currently operates with a Notice of Compliance issued August 6, 2019. The information on record with DCDEE was verified as correct by Kay Matthews and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of superior completed May 2, 2024. Fire Classification of approved completed March 1, 2024. There are concerns that an employee’s family member has been left alone with children of all ages to provide supervision during bathroom breaks for teachers. Clair Shea, Assistant Director, was present and assisted me while observing the indoor and outdoor areas. Upon my arrival, there were 62 children in care, ages one through four years old. I observed outdoor play, hand washing, group time, morning snack, and lunch. Morning snack consisted of assorted chips and water. Lunch was ordered from Chic-Fila. It consisted of chicken nuggets, fries, cookies, and milk. Lunch was missing the vegetable and fruit components. The allegation was discussed with the Assistant Director. I gathered information and interviewed the Assistant Director and two teachers. It was stated that a teacher’s daughter (who was not an employee) was standing in the hallway when another teacher asked her if she could stay with her class while she goes to the bathroom. The daughter agreed and went to the classroom. A staff member, who is the mother overheard the teacher asking the daughter. She went to the classroom and told the daughter that she could not be alone with children because she is not an employee. The teacher of the daughter stayed with the children until their teacher returned. The daughter, an unqualified person, was with the children alone for approximately two minutes. The daughter is currently an employee, her date of employment was May 22, 2024. The employee who asked an unqualified person to stay with her classroom was no longer employed. Based on the information gathered and observations made, the allegation regarding supervision is unsubstantiated. Kay Matthews, Director, was on vacation during the visit. I spoke with her via telephone. She authorized me to review the visit summary with Clair Shea. At the completion of the visit, the Visit Summary was reviewed with C. Shea. The following violations were observed: Violation Number Comment Rule 501 Meals/snacks did not comply with the Meal Patterns for Children in Child Care Programs. Lunch served was from Chik-Fil-A and included chicken nuggets, fries, cookies, and milk. Children were not served a supplemental grain and vegetable/fruit component. 10A NCAC 09 .0901(a) 526 Menus for all meals and snacks were not current or posted where easily seen by parents and cook. A current menu was not posted. 10A NCAC 09 .0901(b) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. One staff member (J.M.) did not have an emergency information form on file. .0701(a) 1041 Prior to employment a Criminal Background Check was not completed. Javianna Miller was left alone with children prior to completing the criminal background check and receiving a qualifying letter. This was corrected during the visit when it was determined Ms. Miller received the qualification letter dated 5/13/2024. G.S. 110-90.2(b) 1757 A valid qualification letter was not on file and available to review at the facility. Javianna Miller did not have a valid qualification letter on file at the facility at the time she was left alone with children. This was corrected during the visit when it was determined Ms. Miller received the qualification letter dated 5/13/2024. The previous Annual Compliance Visit was completed August 11, 2022 and prior to today’s visit, the eighteen-month compliance history score was 92%, which is above the required 75%, as required by North Carolina General Statute 110-90(4)(c-d). Your program currently operates with a Notice of Compliance issued March 14, 2017. The information on record with DCDEE was verified as correct by Kay Matthews, Administrator and the owner’s corporation status was verified on the Secretary of State’s website as current-active. Current Inspections are as follows: Sanitation Classification of Superior completed May 26, 2023. Fire Classification of Approved completed March 1, 2023. Kay Matthews, Administrator, was present and accompanied me while I observed the indoor and outdoor areas. Upon my arrival, there were fifty-seven children in care. Children in space numbers 102, 103, 119, and 120 were observed on the outdoor area, running, climbing, pushing toys, swinging, and riding cars. Indoors, in space #107/108, children were observed playing with dramatic play materials like, unicorns, pretend foods, furniture and dolls. In blocks, a child was playing with a helicopter. A child was observed tracing the number eighteen with a crayon. A teacher acknowledged how well the child was tracing his numbers. In space #110, children were sitting at tables participating in an activity, coloring eight strawberries. Ms. Kay asked the children question about what they were doing. She asked, what they were coloring; how many strawberries were on the page; and what color are strawberries. The children answered with excitement. Today’s lunch consisted of cheese roll-up, rice, refried beans, carrots, apple sauce, and milk. Today’s lunch was ordered from Taco bell. The vegetable and fruit were provided by the program. The Annual Compliance Checklist for Child Care Centers was used during the visit. Six children files, two old staff files, and two new staff files were monitored. The Staff Training worksheet was received during the visit. At the completion of the visit, the visit summary was reviewed with K. Matthews. The following violation was observed: Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The facility's last shelter-in-place or lockdown drill were not practiced every three months. The last drill was conducted on 2/22/23. .0604(u);.0302(d)(8) The violation must be corrected immediately. Mail or email a written, dated, and signed statement to me by 5:00 pm on August 16, 2023, describing how and when the violation was corrected and how the facility plans to maintain compliance in the future. If the compliance letter is not received by the due date, an additional visit will be conducted to verify that violations have been corrected. The last shelter-in-place or lockdown drill was completed February 22, 2023. To maintain a safe and healthy environment, a shelter-in-place or lockdown drill shall be conducted every three months. Refer to child care rule 10A NCAC 09 .0102. We discussed strategies of putting the month (every three months) on the fire drill documentation log and scheduling drills on a calendar. Reminder: S.
